附件一：《学生综合素质评价“品德表现”评价要求和评价标准》
一、评价要求
1.主要反映学生在践行社会主义核心价值观、弘扬中华优秀传统文化及学生道德认知和行为表现等方面的情况。包括社会责任感、诚实守信、合作友善、自尊自信、遵守纪律和人生态度等六个方面的表现。
2.评价内容
（1）社会责任感
爱党爱国爱人民；珍视国家荣誉;勤劳笃行,积极完成班级的工作，乐奉献；自己事自己做，主动分担家务；履行社会责任，乐于参加志愿服务。
（2）诚实守信
诚实守信，保持言行一致；明礼讲德，不说谎不作弊；借东西及时还；做到知错就改。
（3）合作友善
主动帮助同学，不闹矛盾，会合作共处；孝敬父母尊敬师长；热爱集体，配合老师工作；虚心接受批评。

自尊自爱，能正确评价自己，尊重自己；自律自信，严格要求自己，自我约束和自觉规范，相信自己，有自己的学习目标和人生目标；自强，严格要求自己，无论顺境还是挫折，都不放弃自己的目标。
（5）遵守纪律
遵守国法校纪，不违纪违法；自觉礼让排队；保持公共卫生；爱护公共财物。
（6）人生态度
红灯停绿灯行；防溺水不玩火，会自护懂求救；积极锻炼身体；文明绿色上网；乐观开朗向上；不比吃喝穿戴，爱惜花草树木，节粮节水节电，低碳环保生活。
3.评价标准
（1）评价从社会责任感、诚实守信、合作友善、自尊自信、遵守纪律和人生态度等六个方面进行评价。每点5分。
（2）具体要求
每一点总体能做到，有具体生动的案例，得5分；每一点总体做到较好，有具体生动的案例，得4分；每一点总体做到较好，案例不具体的明确，得3分；每一点总体能做到，无案例的，得1-2分。
4.评价要求
（1）学生自评根据上述要求并填写《学生综合素质评价“品德表现”学生自评表》（附件二），在班级中展示一周，接受同学和老师的监督。
（2）学生互评根据《学生综合素质评价学生互评表》（附件三）。
（3）教师评价《学生综合素质评价“品德表现”教师评价表》（附件四）。
5.学生自评、互评、教师评价，都要坚持事实其实的原则，做到客观、公正、合理。具体见《学生综合素质评价信息录入要求》（附件十九）。

第周第课时上课时间月日（星期）累计教案1个课题（内容）：介绍乐高积木第 1 课时 教学目标： 1、学生初步了解乐高积木，培养兴趣。 2、介绍乐高积木的有关历史。 教学重难点： 学生初步了解乐高积木，培养兴趣。 板书设计： 初识乐高积木 教学过程： 1、欣赏导入、激发兴趣： 同学们，一张白纸可以绘出美丽的世界，那一块白布可以做些什么呢？在大家看看桌上的材料后，应该可以猜出个大概了吧。请大家先来看看老师的几幅作品。展示乐高积木图片(小组传阅，部分贴在黑板上)，现在大家知道我们今天所要实践的到底是什么呢？（引出课题）今天我们所要学习的是乐高积木，它是积木的一种。同学们知道乐高积木的起源吗？ 2、介绍乐高积木的历史来源： 乐高公司创办于丹麦，至今已有85年的发展历史，追本溯源，还得从它的金字招牌LEGO说起。商标“LEGO”的使用是从1932年开始，其语来自丹麦语“LEg GOdt”，意为“play well”（玩得快乐），并且该名字首先迅速成为乐高公司在Billund地区玩具工厂生产的优质玩具的代名词。 多年来，“LEGO”图标也变化了多种形式，最新的图标是1998年制作，它是在1973年的版本基础上稍作调整而成，使之更便于在媒体上传播和识别。第一个生产地在丹麦的一所红房子中，那里就是乐高开始的地方。近日，乐高乐高官方推出的一座大型主题空间，坐落在乐高的总部所在地丹麦比隆（Billund），占地 12000 平米，离公司总部也不远，LEGO Experience Center 将于今年正式开放，目前计划中的具体时间是在九月份。(图片展示)
