In this paper, based on the research achievements of previous scholars, respectively from the epidemiology, clinical manifestation, diagnostic criteria, clinical examination, and clinical treatment and so on were summarized. 【Key words】Eosinophils; Gastroenteritis; The diagnosis 嗜酸性粒细胞性胃肠炎（EG）属于临床较为少见的疾病，通常表现为腹泻、腹痛或消化不良等病症，临床可见胃肠道内广泛或局部嗜酸性粒细胞浸润。一般自食管到直肠均具有浸润表现。据相关学者的研究可知，该病在任何年龄均具有一定的发病率。 1.流行病学分析 嗜酸性粒细胞最初是由Kaijser等人于1937年提出，据相关数据提示，在所有住院患者中，EG的发病率为1/100000，然而随着环境的不断变化，人们的饮食习惯也发生了较大的改变，EG的发病率也大大提高[1]。 EG患者具有十分复杂的发病机制，大部分患者具有食物过敏、哮喘、湿疹、过敏体质或季节性过敏等既往病史，过敏对患者的发病具有十分关键的作用。 2.临床表现及诊断标准 2.1 临床分型 根据患者病症类型的不同，一般可表现出不同的临床病症。依据其病症的严重程度，浸润程度以及病变部位，通常可将其分为局限型以及弥漫型。 ①局限型 局限型多见于中老年患者，且不同性别间的发病率无显著差异，通常发病较急，表现为恶心呕吐、上腹部痉挛性疼痛等病症，但病程较短，未表现出明显的过敏史。 ②弥漫型 弥漫型属于常见的病症类型，在男女间的发病率具有显著差异，且多发于中年患者。其中大部分患者发病时会出现恶心呕吐等胃肠道反应，但临床病症并无规律可循，一般在使用抗酸解痉剂后无法起到较好的缓解作用。再依据其病症反应的不同，又可将其分为黏膜型、肌层型以及浆膜型。黏膜型通常表现为腹泻、消化道出血、缺铁性贫血等，临床最为常见。据学者研究发现，该类患者有一半以上曾出现过过敏史、哮喘或荨麻疹等病症。肌层型通常表现为逐渐增厚并变硬的胃小肠壁，可累及肌层，但临床并不常见。浆膜型则更为罕见，发病时可引起胸水或腹水，并含有大量的嗜酸性粒细胞。 2.2 诊断标准 EG病症首先可通过观察临床表现来对病情进行初诊，进而还可通过血象分析、内镜检查以及放射学检查等进行评估判断。临床通常采用两种诊断标准，其一是Talley在Klein的基础上于1990年提出的诊断标准，共表现在三个方面：①胃肠道反应明显；②经病理学检查可见食管到肠道存在嗜酸粒细胞浸润，在放射学检查下可见异常增多的嗜酸粒细胞在结肠周围显示。③出现了结缔组织病、淋巴瘤、嗜酸粒细胞增多症等因嗜酸粒细胞异常增多而发生的疾病。其二是由Leinbach提出的关于EG的诊断标准，共表现在三个方面：①异常的胃肠道反应出现在食用某类特殊食物后。②出现异常增多的外周血嗜酸粒细胞。③经病理组织学结果可见胃肠道浸润或存在异常增多的嗜酸粒细胞 [2]。 3.临床鉴别及检查 对EG病症的检查除了观察患者的临床反应外，还可对其进行血象检查、CT检查、粪便检查、X射线检查或腹腔穿刺检查等辅助检查措施。也可根据检查的类型将其分为以下三类： 3.1 实验室检查 血象检查、粪便检查等均属于实验室检查。经血压检查可见患者的嗜酸性粒细胞明显增多，且高于600/L时具有EG病症的可能，且根据细胞计数的多少还可将其分为轻度、中度以及重度的病症类型。然而经不少学者的研究发现，该病所引起的嗜酸性粒细胞增多并未直接引起并导致患者的胃肠道病变。也有学者研究发现，大部分患者均可能引起嗜酸性粒细胞的增多，然而并不能将该诊断结果作为该病的金标准，还需要进一步辅助内镜检查以及影像学检查确诊。 3.2 内镜检查 内镜检查通常观察患者的黏膜病症程度，评估水肿、充血、溃疡、糜烂或肿块的严重程度。 3.3 影像学检查 CT检查、X射线检查均为影像学检查，经CT检查可发现患者的胃小肠肠壁存在不规则增厚或出现结节样改变，然而此类影像学特征缺乏一定的特异性，其他多类疾病也具有上述表现，例如克罗恩病、淋巴瘤等疾病。还可通过超声检查进行诊断，对于存在消化道内有假肾征或萎缩的患者，一般可作为EG的支持诊断。
