设计服务合同 甲方： 联系人： 联系方式： 乙方： 联系人： 联系方式： 根据《中华人民共和国合同法》、《中华人民共和国广告法》和有关法律规定，甲乙双方本着平等互利的原则，经充分的友好协商，就甲方委托乙方承担其项目的广告设计推广事宜，一致达成如下协议，并承诺共同遵守。 一、服务内容和范围 甲方委托乙方承担XXXXX 设计工作，具体服务内容详见本合同附件一。 二、合同期限 本合同有效期为（X个月），自2018年X月X日至2018年X月X日止。合同结束之界定以完成XXX工作为准，但总体工作认定时间不得超过合同期限，若有相应延迟，以双方口头商定时间为准。 三、服务费用及支付方式 （一）服务费用：为确保项所服务之品牌形象的完整性和系统性，同时保证双方利益，合同采用分阶段计费方式。 合同服务费用，总计人民币：XXXX元整（￥XXXX元） （二）1、付款方式：甲方以现金或银行个人转账形式向乙方支付。 2、付款时间： 本合同签订之日起3个工作日内，甲方须向乙方支付项目启动金，计人民币XXXX元整（￥XXXX元）整。 项目VI系统设计完成，甲方须想乙方支付合同剩余尾款：计人民币XXX元整（￥XXXX元）整。乙方在收到相应款项后，将所有涉及之设计文件移交于甲方，合同结束。 四、甲方的权利和义务 1、甲方有权及时对乙方所提交的广告方案、设计稿和其他书面工作文件提出口头或书面修改意见和建议，乙方据此进行修改、调整。 2、甲方应积极配合乙方工作，按双方约定的项目进度计划及时提供乙方服务所需的各种图片、文字原形资料和相关信息，并对以上资料的合法性、真实性负责，如因使用甲方提供的资料而引起乙方的任何法律纠纷和违规行为，一切责任均由甲方承担，乙方不承担由此产生的直接责任或者间接责任。 3、甲方应出具授权委托书确定甲方的对口衔接人，全权代表甲方对委托项目的总体设计风格及功能要求进行控制负责，并有对所有稿件签字确认的权利和义务。 4、甲方对委托项目各阶段整体设计方案、创意稿等有权提出书面或口头修改意见和建议，并对乙方修改后最终认可的设计方案予以书面确认。 5、甲方在各阶段整体设计方案确定后，有权提出局部修改完善意见，但每次提出的修改意见不应超出上次提出的原则性修改意见。乙方的工作成果经甲方签字认可即为合格。 6、为配合乙方工作，经双方协商一致后，以清单形式列出阶段工作及
