护士日常工作应知应会 ★问：治疗护理过程中怎样进行身份识别 1.我们医院是通过2种方法进行患者身份识别的--（-这句话开头必说，不能漏） 2.第一种方法是核对腕带上的信息，有床号、姓名、性别、禁用药物等 3.第二种方法是如果患者是能识别自己身份的患者，我通过双向核对，让患者讲出自己的姓名来核对确认患者身份 如果不能识别自己身份的患者（昏迷、神志不清、无自主能力的重危病人及不能识别自己身份的精神病患者），我则通过核对床头卡等进行身份识别 ★问：口头临时医嘱：我们医院一般情况下，不执行口头临时医嘱及电话通知医嘱只有在紧急抢救病人时，护士才能执行---（这句话开头必说，不能漏），并做到如下要求： 1.抢救病人时医生的口头医嘱，执行者应复诵一遍无误经医生（开口头医嘱的医生）认定后方可执行 2.执行时，药物需二人核对无误后再使用（是与开口头医嘱的医生核对） 3.执行过程中应保留所有使用药物的外包装，事后经复核无误后方可弃去 4.抢救结束后，应及时补开医嘱（不得超过6小时）并准确补充记录 ★重点药物用药后观察要点： 一、精神科重点药物：氯氮平和碳酸锂 二、氯氮平用药后的观察要点： 1.病人主诉 2.药物不良反应的表现：直立性低血压（临床表现：头昏，头晕、视力模糊，站立不稳等；预防直立性低血压：嘱患者起床、走动、改变体位时动作宜慢）、流延、粒细胞缺乏症（临床表现：患者可有头晕、乏力、头痛、四肢无力、食欲不振、低热、；可有易感染倾向，如易患疖痈、肺炎、尿路感染等。）、心悸、便秘等 3.血常规及血药浓度 三、碳酸锂用药后观察要点 1.病人的主诉 2.药物不良反应的表现a早期不良反应：无力，疲乏，嗜睡，手指震颤，厌食、上腹不适，恶心，呕吐，腹泻，多尿，口干等b后期不良反应：持续多尿，烦渴，体重增加，甲状腺肿大及功能减退，粗大震颤，类似低钾血症的心电图改变，肾功能损害等c中毒症状：发热，共济失调，肌阵挛，肢体运动协调障碍，言语不清和意识模糊，重者昏迷、死亡 3.血锂浓度 重点药物用药后的不良反应或意外情况的处理措施 一、氯氮平 1.直立性低血压：立即帮助病人取平卧位或头低足高位并测量血压，必要时给与病人吸氧，建立静脉通道，遵医嘱用药 2.流延：及时更换床单位，嘱病人备毛巾垫于颌下，经常更换保持清洁 3.粒细胞缺乏症：嘱病人注意休息，预防感染，根据严重程度遵医嘱减药或停药，必要时给病人升白细胞药物 4.心悸：嘱病人注意休息，勿剧烈运动，必要时遵医嘱用药处理 5.便秘：嘱病人多饮水，多吃水果及含纤维丰富的蔬菜，适当增加运动量，如病人3天未解大便遵医嘱给与通便药，必要时可用开塞露纳肛或灌肠 二、碳酸锂 1.早期不良反应：遵医嘱减药，指导病人多饮盐开水 2.后期不良反应；遵医嘱停药，以后从小剂量开始 3.中毒症状：立即停用锂盐，遵医嘱给与大量生理盐水或高渗钠盐加速锂的排泄，或进行人工血液透析 关键流程交接程序注：关键流程是指门急诊重危病人住入病房的交接过程（在预案及处理流程红色标准册内） 急诊护士提前10分钟电话通知病区护士，简要告知患者病情----病区护士通知值班医生，根据患者病情准备床单位、急救器械等-----由急诊医师、护士携带重危病人急诊转入病区护理记录单，门诊病历卡护送病人（已带好腕带）至病
