4. New Affidavit of support (Moody)

Moody Bible Institute U.S. GOVERNMENT REQUIREMENTS

All F-1 students are required to provide documentation of financial support before an I-20 document can

be issued. If the student will be supported by a sponsor(s), the sponsor must sign the Affidavit of Financial Support below AND have it notarized by a notary public. In addition, sponsor(s) must provide

their official bank statement showing the availability of the funds in liquid assets. Bank statements and

financial affidavits cannot be older than 6 months.

WHAT DOES THIS AFFIDAVIT MEAN?

By completing this affidavit, you are swearing that you will provide this student with a specific amount of

money for the specific duration of time the student is going to study at Moody Bible Institute. Also, with

the attached official bank statement, you are proving that you are financially capable of providing this support.

Before signing this affidavit, you must understand that you are making a financial commitment to the student. This commitment should not be broken. Sponsor(s) who fill out this form will be responsible for

the students’ expenses for the duration of time he/she has promised. Do not expect that the student will

be able to help support his or her educational and living costs through employment. Employment opportunities are typically limited to jobs on campus and are strictly controlled by the Department of Homeland Security regulations.

NOTE: * Additional documentation may be required as evidence of sponsor’s ability to provide financial support.

*** This form is valid for six months from the date notarized.

**** Notary services may be provided at a local bank and/or law firm, depending on the country you reside.
