液体疗法治疗重型颅脑损伤的应用研究

[摘要] 目的探讨液体疗法通过提高脑灌注压对重型颅脑损伤的治疗作用。方法选择2007年1月~2009年12月北京东方医院神经外科收治的重型颅脑损伤患者30例,将患者随机分为实验组和对照组,每组各15例。实验组患者给予平衡液+胶体液+脱水剂疗法,对照组给予常规脱水剂。测量两组患者治疗前后及治疗后脑灌注压(CPP)、GCS评分并进行比较,评估对重型颅脑损伤患者进行液体疗法的治疗效果。结果①实验组平均CPP升高了(10.80±4.84)mm Hg(1 mm Hg = 0.133 kPa),对照组平均CPP升高了(7.13±3.29)mm Hg。两组治疗后CPP均高于同组治疗前,差异均有统计学意义(P < 0.05);实验组治疗后

CPP[(70.13±4.52)mm Hg]高于对照组[(66.47±3.67)mm Hg],差异有统计学意义(P <

0.05)。②实验组平均GCS评分提高了(1.60±1.35)分,对照组平均GCS评分提高了

(1.13±0.64)分。两组治疗后GCS评分均高于同组治疗前,差异均有统计学意义(P <

0.05);两组治疗后GCS评分比较,差异无统计学意义(P > 0.05)。结论液体疗法能够有效地提高重型颅脑损伤患者CPP,对于重型颅脑损伤患者有积极的治疗作用。

[Abstract] Objective To investigate the liquid therapy treatment for severe craniocerebral trauma. Methods 30 patients of severe craniocerebral trauma in Dongfang Hospital of Beijing University of Chinese Medicine from January 2007 to December 2009 were selected and randomly divided into experiment group and control group with 15 cases in each group. The treatment group was treated by the prescription with balance liquid+colloidal liquid+dehydrating agent. The control group was treated with conventional dehydrating agent. CPP, GCS score in the two groups before and after treatment were measured and compared; the treatment effect of liquid therapy treatment for severe craniocerebral trauma was evaluated. Results ①The improvement of CPP in experiment group was (10.80±4.84) mm Hg (1 mm Hg = 0.133 kPa), the improvement of CPP in control group was (7.13±3.29) mm Hg. The CPP in the two groups after the treatment were all higher than those before the treatment in the same group, the differences were statistically significant (P < 0.05);the CPP in experiment group after the treatment [(70.13±4.52) mm Hg] was higher than that in
