Traditional methods of cervical cancer detection involve regular cytological screening, followed by referral to colposcopy for the evaluation of smear-detected abnormalities. The colposcopist inspects the cervix using low-powered magnification, with contrast enhancement between normal and abnormal epithelium achieved via the topical application of agents such as aqueous acetic acid and Lugol’s iodine solution. A biopsy may be indicated, and further follow-up and treatment is based on the colposcopic examination and the pathology result. Although the traditional screening, referral and treatment process has proved very effective in reducing overall cervical cancer incidence if implemented within a coordinated programme (Quinn et al., 1999), the process is not a fail-safe method of detecting and treating all cervical abnormalities. At the screening stage, the cytological smear is known to suffer from sensitivity limitations (Nanda et al., 2000). Diagnostic assessment inevitably relies on the subjective interpretation of the magnified and illuminated colposcopic image, despite the implementation of standardised training, accreditation and quality assurance systems in some jurisdictions (Department of Health UK, 2001; see Chapter 25). This leads to variability in the colposcopic diagnosis and in management decisions (Etherington et al., 1997). A consequent level of inaccuracy in the localisation of biopsy site contributes to a significant false negative rate for cervical biopsy (Skehan et al., 1990). Several approaches to improving the accuracy of cervical disease detection have been considered. For cervical screening, technological improvements to cytology include liquidbased cytology and automated cytological evaluation systems. A number of new approaches to cervical screening and diagnosis have also emerged and include HPV DNA testing, the use of molecular markers and real-time devices. Real-time devices utilise in vivo measurement techniques and computerised analysis to provide immediate feedback to the clinician. They incorporate optical and/or electrical biosensors, which are linked to software processing systems designed to classify tissue types. The technology relies on changes in primary tissue characteristics, such as cellular composition, cellular morpho-

Real-time devices for the screening and diagnosis of cervical neoplasia

INTRODUCTION
logy and vascular structure, which can be used to distinguish between various types of normal and abnormal cervical epithelium. Secondary (or derived) effects of these primary characteristics, which can be measured directly, include tissue reflectance and absorbance, tissue autofluorescence, the intensity and direction of backscattered light and the electrical properties of the tissue. Two major applications have been proposed for real-time devices. The first is in cervical cancer screening, where the device is used by the health practitioner for routine screening. In this situation, real-time technology is likely to be introduced initially as an adjunct to cervical cytology although, in the longer term, the technology has the potential to operate as an autonomous screening test. The second major application is as a tool for the colposcopist, with the device used as an aid to identifying and classifying abnormal tissue locations on the cervix. Depending on the clinical setting, this information can potentially be used either to guide the choice of biopsy site or to facilitate see-and-treat protocols. One of the major benefits associated with real-time devices is the automation, and consequent standardisation, of the test process. In effect, these devices have the potential to bring the expertise of the specialist gynaecologist to a wider group of practitioners. The second major advantage is in the provision of instantaneous feedback to the clinician or screener. For screening applications, realtime feedback allows immediate counselling, management and referral. In situations where loss-to-recall is a concern, the technology could potentially allow the screening, diagnosis and treatment of cervical disease to take place within a single clinical session. The accuracy of real-time devices for classifying cervical disease is device dependent, and is intrinsically dependent on two major factors. The first is the specific choice of biosensor, or combination of biosensors, used for assessing cervical tissue. In general terms, the chosen biosensor must have several key characteristics. It must be capable of reliable and reproducible measurement, and it must discriminate between the various types of normal and abnormal cervical tissue. The second major factor in real-time device accuracy is the processing software used to convert the information obtained f:48 AM Page 387
