There are many people who think that wealth is better than health. I used to think so until one day I read a story about Howard Hughes. He was an American billionaire who got everything he wanted. However, in the last twenty years of his life,his health began to deteriorate and he was miserable. He had the best doctors and nurses. However, he could still find no relief. I realized that health is worth all the money in the world. If you have millions of dollars but your health is poor, you will not be able to do what you want to do. So I would like to say don't hurt yourself and try to make money.Instead, take care of your body and be happy at what you do. Health is more important.

摘要:某房地产开发公司总经理马先生因身体状况不佳, 到医院体检中心体检并寻求健康管理。我们采用常规体检和健康问卷相结合的方式采集了健康管理的基本资料; 对其健康状况进行了评估,列出了健康的风险评估数据;总结了健康危险因素;个体健康风险程度报告; 制定了健康教育计划; 干预的短期和长期目标; 干预的过程及效果评价。在本人的密切配合下, 通过对其生活方式的管理和疾病治疗相结合的方式, 使其健康状况有较大改善, 患脑中风、糖尿病、冠心病的风险有所降低。

一、一般情况:马某,男, 1961年6月4日出生。汉族,籍贯陕西,大学文化,目前为某房地产开发公司总经理,工作繁忙,压力大,经常加班,每天工作时间长达12小时. 平日很少有运动,经常是以车代步,吸烟(2包/日。因工作需要经常需要应酬,经常饮白酒。日饮酒>100ml。喜吃羊杂碎等高脂和高胆固醇食品,口味重,平时喜吃拉面

等高盐饮食高盐饮食, 不喜欢食用蔬菜和粗粮。其父亲患有高血压和冠心病。自觉近来身体素质下降, 常有力不从心之感,偶有心前区憋闷。于2007年4月10日进行初次健康健康咨询

和体检。二、体检资料:

根据马先生的情况,安排了一次较为全面的体检,包括内科、外科、胸部X 检查、眼底检查、心电图检查、腹部 B 超、血生化检查(血糖、总胆固醇、甘油三酯、尿常规检查。结果如下:

三、健康评估:

1. 综合上述各项指标进行健康风险评估,该客户的主要健康问题包括:

1 高血压:客户血压测量为145/98 mmHg,已经可以诊断为高血压。

2 高血脂:甘油三酯高达2.4mmol/l, 总胆固醇为6.8mmol/l,均超出正常范围。

3 超重:体重指数27.64kg/m2, 属于超重,但已接近肥胖水平。

4 空腹血糖受损:空服血糖:6.6mmol/L

5 脂肪肝:B 超诊断为中度脂肪肝。

2. 主要危险因素为:

1 饮食结构不合理:高脂肪、高热量食物、酒精摄入过多。高盐饮食。蔬菜,水果及粗纤维食物摄入过少。

2 运动量太少:客户的体力活动较少。

3 有家族史

3. 综合体检和问卷情况及家族史调查资料, 用相关疾病风险评估工具进疾病危险分数评估。马先生属于冠心病、糖尿病和肥胖的高危人群。

四、健康干预

由于马先生的健康问题多数与生活方式不健康有关, 所以应该采用以生活方式管理的干预手段。

1, 健康教育:首先把体检和健康评估的结果告诉马先生, 并告之他的身体状况不佳。已经患了几种疾病, 另外还处于冠心病、糖尿病和肥胖的高危状态, 如果不注意, 健康有进一步变坏的可能。希望他能积极配合我们改善和维护好自己的健康。马先生表示愿意配合。并告之有高风险并不意味着一定就会发病, 慢性疾病的发生、发展往往需要几年甚至十几年乃至几十年的时期, 在此期间进行有针对性的干预, 有可能成功的阻断、延缓甚至逆转疾病的发生和发展, 从而实现健康的目的。首先, 我们把与马

先生相关的几种病的发病机理和防治方法对其进性了教育。

(1 高血压的危害因素和控制:不可改变的危害因素包括年龄、性别、遗传因素,可改变的危害因素包括高盐饮食、超重和肥胖、过量饮酒、缺乏体力活动、长期精神紧张,减少高血压危险因素的措施包括限盐、控制体重、经常进行体力活动, 多吃水果和蔬菜、减少脂肪摄入、保持心理健康等。尤其是肥胖与高血压密切相关, 高血压是由于心输出量和外周阻力关系混乱导致血流动力学异常,引起的以动脉和舒张压持续增高为主要表现的临床综合征。而肥胖是由人体内的脂质代谢混乱在皮下和脏器周围堆积所致。监测指标是体重指数(BMI 及腰围(WC 计算,我国卫生部发布的《中国成人超重和肥胖症预防控制指南(试用》中规定的中国的分类标准是:BMI 28为肥胖[1]。据统计我国高血压患者中肥胖人是常人3倍, 肥胖者高血压的患病率是标准体重人患高血压的26倍, 肥胖者高血压时以收缩压增高为主。

(2 糖尿病的危害因素和控制:糖尿病的发生与遗传因素、超重和肥胖、缺乏体力活动、膳食不平衡都有密切关系。糖尿病是属于代谢性异常的慢性疾病, 。它最可怕的不是疾病本身, 而是在并发症上, 易并发心血管、肾脏、视网膜及神经等慢性疾病。2型糖尿病发病率,

与超重和肥胖有密切相关性,肥胖的身躯会使胰岛素的需要量增大,而分泌胰岛素的胰脏β细胞为应付此情形会增殖变的肥大,长期下来β细胞便产生障碍而无法分泌足量的胰岛素, 进而造成糖尿病。所以控制自己的体重在理想体重内,对糖尿病的预防很好的效果。

(3冠心病和脑卒中:高血压治疗的最终目标是预防冠心病和脑卒中。血糖、血脂是上述两种疾病进行风险评估时的重要参数,所以也要控制在正常范围。

(4中国居民膳食指南指出,每日每人油脂不超过25g ,家用白瓷调羹2 调羹,盐不超过6g (1 个啤酒瓶盖。根据我国实际情况最多不超过10g 。并提倡多吃水果蔬

菜, 每天不少于500 克[2]。

2、膳食处方:根据马先生身高体重及工作性质, 计算出每日所能摄入的总热量62×25=1552 千卡, 其中碳水化合物供热占65%,脂肪供热占20%,

蛋白质供热占15% 早、中、晚餐供热比例为3:4:3,每周提供一次参考食谱,并建议少食动物内脏,吃拉面时不要将面汤一起喝光, 炒菜时少放油和盐, 可以用辛辣香料改善菜口味过淡的问题。少食腌鱼、腌肉、香肠,尽量少吃味精、酱油,以减少钠的摄入。

3.运动处方:考虑到马先生工作繁忙,且没有运动习惯,建议其每日步行代替乘车上班。每日快走30~40分钟,达到1万步(佩戴记步器。周末参加球类或登山运动。

4.戒烟限酒:建议其逐步减少,直至完全戒烟。在交际应酬时,如果一定要喝酒建议喝干红葡萄酒。而且,总量不要超过四量。

5.遵医嘱药物控制高血压。

6.将干预措施告知了马总的秘书、司机以及妻子和孩子,请大

家给予支持、配合和督促。五、干预的短期目标:提高马先生对高血压、肥胖及糖尿病的知晓率、服药率、控制率以及依从性。

六、干预的长期目标:控制高血压、减轻体重,改善糖代谢,降低冠心病、糖尿病的发病风险,提高生活质量。

七、追踪随访:每两周定期对马的用药、饮食、运动情况进行随访, 根据实际情况随时调整。

八、干预的过程及效果评价:通过方案的制定及干预措施, 应用上述方案实施3个月, 已与患者沟通6次,通过饮食和运动干预,能自觉控制吸烟50ml/天,每天快走30分钟,食盐量所改善,走路、爬楼都轻松多了。对今后的健康管理充满信心。

九、今后措施及建议

马先生经过三个月的健康管理和综合干预过程, 虽然血压和空腹血糖基本正常, 但仍然没有达到理想水平。体重和血脂仍然不正常。马先生属于高血压、糖尿病、肥胖等疾病的高危人群。鉴于此情况,

建议马先生:

1.每年至少定期到医院做健康检查一次,包括一般的内外科检查,血、尿、粪便常规检查, 肝肾功能检查,血糖、血脂检查,胸部X 线检查、心电图及腹部B 超检查。重点需要注意的是:每周个月测量一次血压,最好能自备血压测量仪,控制在140/90mmHg以下;每半年检测空腹血糖一次,控制在3.9-6.0mmol/L之间. 并同时检测糖化血红蛋白和做糖耐量实验。争取全面达标。

2.继续加强生活方式的管理,保持平衡膳食,适量运动,良好心态

巩固第一阶段健康管理的成果。

通过本例健康管理的案例我们体会到,健康管理完全可以有效地阻断、延缓甚至逆转疾病的发生。同时, 健康管理是一个不断循环往复的过程, 只有持之以恒, 才能达到促进健康, 维护健康的目的。
