IL-6在巨噬细胞向M2型极化过程中的诱导作用刘苗;唐荣;姜毅【摘要】目的将小鼠巨噬细胞系RAW264.7细胞和骨髓瘤细胞系KM3细胞共培养,探讨IL-6在巨噬细胞向M2型极化的过程的作用.方法取对数生长期细胞,分为3组:A组(KM3细胞组),B组(RAW264.7细胞组),C组(RAW264.7细胞+KM3细胞组).分别予以ACTA(IL-6特异性抑制剂激活素A)或rIL-6(重组人IL-6)处理后,检测肿瘤相关M2型巨噬细胞表达标志F4/80+ CD206+的比例.RT-PCR和Westernblot方法检测各组细胞因子CCL22、IL-10、IL-12、TNF-αmRNA和蛋白表达量.ELISA法检测各组细胞培养上清中IL-6的含量.结果 RAW264.7和KM3细胞共培养24 h后,与对照组相比,M2 型巨噬细胞比例显著增加(P<0.05);予以ACTA处理后M2型巨噬细胞表达明显下降(P<0.05);而予以rIL-6处理后M2型巨噬细胞表达明显上调(P<0.05).与B组(RAW264.7细胞组)相比,C组(RAW264.7细胞+KM3细胞)M2型巨噬细胞相关细胞因子CCL22,IL-10的mRNA和蛋白表达水平明显上调(P<0.05),而M1型巨噬细胞相关因子IL-12,TNF-α的mRNA和蛋白表达水平明显下调(P<0.05).与A组、B组相比,C组细胞上清液中IL-6含量在24 h、48 h、72 h时间点均明显上调(P<0.05).通过浓度梯度改变RAW264.7细胞或KM3细胞的数量,发现RAW264.7细胞数量的改变显著影响了IL-6的表达水平.结论将RAW264.7细胞和KM3细胞共培养后,后者能诱导RAW264.7细胞向肿瘤相关M2型巨噬细胞转化, IL-6在上述转化过程中发挥重要作用.%Objective To investigate the effects of IL-6 in polarization process of macrophages to M2 type,with macrophage cell line-RAW264.7 of mice being co-cultured with myeloma cell line KM3 in vitro.Methods The cells at exponential growth phase were divided into three groups: group A (KM3 cell),group B(RAW264.7 cell),group C (RAW264.7 cell +KM3 cell).After the cells were treated with ACTA (a specific inhibitor of IL-6) or rIL-6 (recombinant human IL-6),respectively,the proportion of F4/80+ CD206+ cells was detected.The expression levels of CCL22, IL-10, IL-12, TNF-α lpha were measured by RT-PCR and Western Blot, respectively.The content of IL-6 in culture supernatant was detected by ELISA.Results After RAW264.7 cells and KM3 cells were co-cultured for 24 hours, as compared with that in control group, the proportion of M2 type macrophages was significantly increased ( P <0.05),moreover after the cells were treated by ACTA, the expression of M2 type macrophages was obviously decreased ( P<0.05),however, after the cells were treated by rIL-6, the proportion of M2 macrophages was significantly increased ( P <0.05).As compared with those in group B, the expression levels of M2 macrophage related cytokines-CCL22 and IL-10 mRNA and protein were significantly up-regulated in group C ( P <0.05),but the expression levels of M1 macrophage related cytokines-IL-12, TNF-α lpha mRNA and protein were obviously down-regulated ( P <0.05).As compared with those in group A and group B, the levels of IL-6 in culture supernatant were significantly increased at 24h, 48h, 72h time points in group C ( P <0.05).When the cell counts of RAW264.7 cells or KM3 cells were changed by different concentration gradient, the expression levels of IL-6 were more easily influenced by the chages of RAW264.7 cell number.Conclusion After RAW264.7 cells are co-cultured with KM3 cells in vitro, the latter can induce the transformation of RAW264.7 cell into tumor-associated M2macrophages, moreover, IL-6 may play an important role in the transformation process.【期刊名称】《河北医药》【年(卷),期】2017(039)008【总页数】5页(P1125-1128,1132)【关键词】巨噬细胞;骨髓瘤;IL-6;极化【作者】刘苗;唐荣;姜毅【作者单位】430060 湖北省武汉市,武汉大学人民医院儿科;430060 湖北省武汉市,武汉大学人民医院儿科;430060 湖北省武汉市,武汉大学人民医院儿科【正文语种】中文【中图分类】R730.2白细胞介素-6(IL-6)是一种多功能的细胞因子,能调节多种生理功能,包括基因激活、细胞增殖和分化[1,2]。

关键词:白细胞介素,肺癌,化疗1. 引言白细胞介素家族(interleukins ,ILs)成员众多,该家族有多种生物学功能且作用广泛,其成员又分属于不同的细胞因子家族,如IL-2、IL-3、IL-4、IL-5、IL-6、IL-7、IL-9、IL-11、IL-13和IL-15属于造血因子家族,IL-8属于趋化因子家族等等,主要有T、B细胞产生,少数由单核巨噬细胞和组织细胞产生。
据目前研究表明,在白细胞介素家族中很多种白介素及其受体都与肺癌患者的预后有一定关系作用。
2. 白细胞介素-6、8、10许多细胞因子在肿瘤的发生、发展、转移及预后中起到十分重要的作用。
众多研究表明白细胞介素-6、8、10(IL-6、IL-8、IL-10)与肿瘤的免疫关系密切。
陈春莉等[2]选取了47例TNMⅡ~Ⅳ期的肺癌患作为研究对象,其中23例患者进行了以铂类药为主的化疗,在治疗前后一月用ELISA方法分别测定其血清IL-6、IL-8、IL-10水平。
另对7例死亡患者临终前检测血清IL-6、IL-8、IL-10于水平。
同时取30例健康成人作为对照组结果发现,47例肺癌患者不同病理类型之组间三组细胞因子水平无差异,但均明显高于对照组;肺癌患者血清IL-6、IL-8、IL-10水平随着临床分期的进展而升高,有远处转移的晚期患者的血清三者明显高于无转移的早期患者;16例化疗有效,病情稳定和缓解的患者血清IL-6、IL-8、IL-10于水平较化疗前明显下降,研究表明血清IL-6、IL-8、IL-10联合检测可作为辅助诊断和疗效观察的指标。

一项最新研究[1]显示,我国骨质疏松症的总患病率为13%,总人数已超过1.78亿。
老年人是骨质疏松症的重点人群,自1982年起,我国65岁以上老年人占人口比重不断升高,2019年我国65岁以上老年人占人口比重已达到12.6%[2]。
预计到2050年,我国骨质疏松症或骨密度低的患者将达到2.12亿[1]。
骨质疏松症使得骨质脆性增加、易于骨折,导致患者的生活水平急剧下降。
利用脂肪来源的间充质干细胞(adipose-derived mesenchymal stem cells ,ADMSCs )诱导成为成骨细胞治疗骨质疏松症是医学研究的新方向[1]。
ADMSCs 可以通过旁分泌功能,分泌一些生物活性分子,为组织修复建立良好的微环境,促进新生血管的形成和伤口愈合,并且减少组织的炎症反应。
ADMSCs 也可分泌促进血管生成和抗凋亡潜能的生长因子,如转化生长因子(transforming growth factor ,TGF )、胰岛素样生长因子(insulin growth factor ,IGF )、血管内皮生长因子(vascular endothelial growth factor ,VEGF )、肝细胞生长因子(hepatocyte growth factor ,HGF )[3]和骨形态发生蛋白(bone morphogenetic protein ,BMP )家族BMP-2、BMP-7等[4]。
ADMSCs 来源丰富,通过脂肪抽吸术易于获得,无免疫排斥。
平均每300 mL 脂肪组织可获得108个 细胞,每克动物脂肪可获得5 000个成纤维集落形成单位(colony forming unit-fibroblast ,CFU-F )[5]。

细胞因子是指主要由免疫细胞分泌的能够调节细胞功能的小分子肽,是体内细胞之间相互作用的主要介质,细胞因子的产生和相互作用对机体防御疾病和维持生理平衡具有重要意义。
肿瘤坏死因子α(tumor necrosis factor α,TNF-α)、白细胞介素 (interleukin,IL)6是重要的促炎因子和免疫调节因子,本文对TNF-α、IL-6在炎症性肠病发病机制中的作用进行综述如下。
1 TNF-α与炎症性肠病1.1 TNF-α的概述国外有学者于1975年发现,用卡介苗以及内毒素处理小鼠后,在其血清中发现了能够诱导肿瘤组织出血、坏死的蛋白物质,故将该血清蛋白命名为TNF,又称其为分化诱导因子或恶液质因子[2]。
TNF分为三类:TNF-α、TNF-β、TNF-γ,其中TNF-α是由活化的巨噬细胞、单核细胞及T细胞产生。

C反应蛋白对儿童感染性肺炎的临床价值【摘要】目的通过对细菌性和病毒性肺炎患儿进行治疗前后c 反应蛋白、白细胞计数及异常率对比研究,探讨crp对儿童感染性肺炎的临床价值。
方法选择在我院3-12岁感染性肺炎患儿192例,分为细菌组121例和病毒组71例。
分别对两组患儿治疗前后c反应蛋白、白细胞计数及异常率进行比较。
结论 c反应蛋白对儿童细菌性和病毒性肺炎的早期诊断和鉴别诊断、评价病情变化有重要的临床价值。
【关键词】c反应蛋白;白细胞;感染性肺炎;儿童肺炎是儿内科最常见的疾病之一,死亡率高,世界卫生组织2005年估计,全球每年约有160万人死于链球菌感染性疾病,其中70-100万是5岁以下的儿童 [1]。
儿童感染性肺炎最常见的病因是细菌和病毒,其确诊主要依赖于微生物学检查。
但目前诊断肺炎仍缺乏可靠的方法:肺穿刺培养是小儿肺炎病原学诊断的金标准,但此法对操作人员技术要求高,患儿及家长不易接受;从病变部位培养病毒是诊断病毒感染的金标准,但传统的病毒分离与鉴定操作复杂、实验条件要求高,检测时间长,阳性率极低[2]。
寻求敏感快速简便的诊断方法对儿童感染性肺炎早期诊断和鉴别诊断,以及评估病情发展具有重要的现实意义。
本研究通过对192例感染性肺炎患儿的c反应蛋白(crp)、白细胞计数(wbc)及异常率进行对比研究,发现crp对于儿童感染性肺炎早期诊断和鉴别诊断具有重要临床价值。
1 资料与方法1.1 一般资料 2010年8月至2012年10月在我院住院的3-12岁感染性肺炎患儿192例,诊断标准参照文献[3],以咽拭子细菌培养和病毒血清学检测为依据,分为细菌组121例(男78例,女43例,平均年龄8.3±4.1岁)和病毒组71例(男45例,女26例,平均年龄8.2±3.9岁)。
入选者均排除肿瘤、免疫性疾病、严重器质性疾病等。
两组别在年龄、性别构成无差异。
1.2 方法患儿入院时和治疗后1周分别抽取静脉血,检测crp 和wbc计数。

根据儿童高关注物质(CHCC) 2012年血管炎分类新命名,可分为:大血管炎[巨细胞动脉炎(GCA)、Takayasu动脉炎(TA)]、中血管炎[结节性动脉炎(PAN)、川崎病]、小血管炎[抗中性粒细胞胞浆抗体(ANCA)相关性血管炎、免疫复合物血管炎(AAV)]、变异性血管炎(白塞病、COGAN综合征)、单脏器血管炎(皮肤白细胞破碎性血管炎、皮肤动脉炎、原发性中枢神经血管炎、孤立性主动脉炎)、系统性疾病相关性血管炎(SLE血管炎、RA血管炎等和类肉瘤血管炎)、可能病因相关性血管炎。
由于血管炎的种类繁多、治疗手段较少,且疗效及个体差异相对其他疾病较大。
虽然现阶段生物制剂在治疗难治性系统性疾病相关性血管炎方面取得了很好的效果,但大部分是基于临床个案报导及少数病例对照研究。

were determined in37patients with esophageal squamous cell carcinoma who received identical neoadjuvant preoperative chemoradiotherapy regimens and underwent surgical resection.Histological response to this ther-apy was assessed in surgically resected specimens.The serum soluble interleukin-6receptor(sIL6R)level was significantly higher in30patients who failed to achieve a histological complete response(P=0.005).Multivariate analysis revealed that the increased level of sIL6R was one of several significant inde-pendent predictors of an unfavorable outcome(hazard ratio, 2.87;P=0.017).The increased level of this cytokine in patients who did not obtain a complete response was reproducibly observed in an independent cohort of34patients.Esophageal squamous cell carcinoma patients with an increased serum level of sIL6R are predicted to respond poorly to preoperative chemor-adiotherapy,therefore,their exclusion from this treatment may be considered.Persistent systemic inflammation is implicated as a possible mechanism of resistance to this therapy.(Cancer Sci 2013;104:1045–1051)E sophageal cancer is one of the leading causes of cancermortality worldwide,accounting for>300000deaths annually.(1)Surgical resection is one of the most reliable meth-ods for local management of the disease,but lymph node metastasis and invasion to neighboring organs,such as the lung,trachea,and large vessels,often hamper curative resec-tion of the tumors.To improve resectability,various trials of PCRT have been attempted,(2–6)and a recent large randomized phase III clinical trial clearly indicated that it improved the overall survival of patients with potentially curable esophageal or esophagogastric-junction cancer.(7)However,PCRT does not always improve the survival of patients with ESCC.We previously showed an unfavourable outcome of patients with ESCC that did not response to PCRT.(3)Similar results have been reported by other investiga-tors.(8–10)The combination of chemotherapy with radiation enhances the degree of toxicity.(11,12)Therefore,if PCRT proves ineffective,patients potentially would merely have suffered more severe adverse events without receiving any of the anticipated benefits.For this reason,development of a new diagnostic method that would reliably predict the response of every patient to the treatment is clearly desirable.(11)The biological behavior of cancer may be determined,or at least influenced,by the tissue microenvironment.Cytokine gene expression signatures in non-cancerous tissues have been reported to predict the outcome of patients with hepatocellular carcinoma and lung adenocarcinoma.(13,14)Chemoradiation induces cancer cell death through tumor antigen-specific T-cell responses.(15)Based on these observations,we assumed that a certain type of host reaction might influence the efficacy of chemoradio-therapy.Here we report the comprehensive profiling of serum cytokines in ESCC patients who received an identical protocol of neoadjuvant PCRT,which revealed a correlation between serum sIL6R and the histological response to PCRT that to our knowledge has not been reported previously.Materials and MethodsSerum samples.Serum samples from a total of218ESCC patients in three retrospective cohorts(PCRT-discovery [n=37],PCRT-validation[n=34],and PCT[n=100])and two prospective cohorts(prospective PCRT[n=26]and prospective PCT[n=21])were analyzed.The diagnosis of primary squamous cell carcinoma was con-firmed histologically in all cases by pretreatment endoscopic biopsy.Patients were staged clinically according to the Inter-national Union Against Cancer’s TNM classification of malig-nant tumors(6th edition).(16)Serum samples were collected before the initiation of any treatment and kept frozen until analysis.Patients received PCRT(PCRT-discovery,PCRT-validation,and prospective PCRT Cohorts)or preoperative combinational chemotherapy(PCT and prospective PCT Cohorts)and underwent standard esophagectomy and lympha-denectomy with curative intent.Histological responses to treat-ments were classified into Grades0–3(G0,G1,G2,and G3) according to the9th edition of the Japanese Classification of Esophageal Cancer(Table S1).(17)Individuals who had previously undergone therapy for esophageal cancer or chemoradiotherapy for other malignan-cies,or had histories of other active a retrospective cohort of 37stage II –IVa ESCC patients randomly selected from among those who consecutively received neoadjuvant PCRT at TMU between 2000and 2005(Table 1).(3,18)The PCRT consisted of low-dose CDDP (5mg/m 2/day,5days weekly for 4weeks;total 100mg/m 2)and 5-FU (350mg/m 2/day,5days weekly for 4weeks;total 7000mg/m 2)plus concurrent radiation (10-MV linear accelerator,2Gy/day,5days weekly for 4weeks;total 40Gy).Surgical resection was carried out 4weeks after the completion of PCRT.2PCRT-validation Cohort was a retrospective cohort compris-ing 34serum samples obtained from the remaining stage II –III ESCC patients and those who received neoadjuvant PCRT using the same protocol as that for the PCRT-discovery Cohort at TMU between 2006and 2009(Table 1).(3,18)3Prospective PCRT Cohort was a prospective cohort of serum samples from 26stage II –III (excluding T4)patients who were enrolled in a phase II clinical trial of neoadjuvant PCRT at the National Cancer Center Hospital and the National Cancer Center Hospital East between 2010and 2011.(19)These patients underwent two courses of protracted infusion of 5-FU (1000mg/m 2/day)on days 1–4and days 29–32,and 2-h infusions of CDDP on days 1and 29(75mg/m 2),along with adequate hydration and antiemetic coverage,plus concurrent irradiation (1.8Gy/day).Surgical resection was carried out 6–8weeks after the completion of PCRT.4PCT Cohort was a retrospective cohort of serum samples from 100stage II –III (excluding T4)patients who received neoadjuvant combinational chemotherapy at the NCC between 2003and 2010.The combinational chemotherapy comprised 5-FU (800mg/m 2/day)on days 1–5and days 22–26,and 2-h infusions of CDDP (80mg/m 2)on days 1and 22,with adequate hydration and antiemetic coverage.The treatment was repeated three times at 3-week intervals.5Prospective PCT Cohort was a prospective cohort compris-ing serum samples from 21stage II –III (excluding T4)patients who were enrolled in a phase II clinical trial of neoadjuvant combinational chemotherapy at the National Cancer Center Hospital between 2009and 2010.The chemotherapy regimen consisted of docetaxel (70mg/m 2)on day 1,5-FU (750mg/m 2/day)on days 1–5,and 2-h infusions of CDDP (70mg/m 2)on day 1,with adequate hydration and antiemetic coverage.The treatment was repeated three times at 3-week intervals.Multiplexed immunobead-based assay.The levels of 84cyto-kines (listed in Table S2)were measured in the sera of the 37patients in the PCRT-discovery Cohort using nine multiplex kits:an acute-phase 4-plex panel Technology,Boston,MA,USA), as described previously.(18,20)Statistical analysis.Survival curves covering the period from the date of surgery to the date of death or last follow-up were plotted using the Kaplan–Meier method,and differences between the curves were assessed with the log–rank test.Stu-dent’s t-tests and the Cox proportional hazards regression model were carried out using the StatFlex statistics package (version5.0)(Atiteck,Osaka,Japan)and tools available in the R statistical package(/).(18,21,22)Differ-ences having P-values of<0.05were considered to be statisti-cally significant.ResultsCirculating cytokines associated with response to PCRT.The levels of84cytokines in serum samples from37patients that were obtained prior to PCRT(PCRT-discovery Cohort)were measured using the multiplexed immunobead-based assay.All of the patients received the same protocol of chemoradiotherapy (CDDP plus5-FU and concurrent irradiation)and underwent esophagectomy.Histological examination of the resected speci-mens revealed that viable tumor cells had completely disap-peared in seven(19%)patients(pathological complete response or G3),whereas residual viable tumor cells were detected in the remaining30(G1or G2).No case was graded as G0(no recognizable effect).There was no significant differ-ence in age,gender,tumor location,clinical stage,or serum CRP level between these two sets of7and30patients (Table1).The seven patients who obtained a G3response showed a markedly favorable postsurgical outcome in compari-son with the G1and G2cases(P=0.014,log–rank test) (Fig.1a).Through multiplex protein profiling we found that the base-line serum levels of six cytokines,including MIP1B (P=0.002,t-test),sIL6R(P=0.005),MIP1A(P=0.027), insulin(P=0.031),interferon-a2(P=0.048),and MMP3 (P=0.049),were significantly decreased in the seven patients who showed a complete pathological response(Table2).The differences did not remain statistically significant after Bonfer-roni adjustment for multiple testing,(23)probably due to the small number of patients assessed,especially those who obtained a G3response.Correlation coefficient analysis revealed no significant mutual association(correlation coefficient≥0.70)among the six cytokines(Table S3).Association of high sIL6R with poorer OS.To further select the most critical factor,the37patients in the PCRT-discovery Cohort were classified into two groups according to their levels of each cytokine,and OS was compared between the groups.Among the six cytokines,we found that only the serum level of sIL6R was significantly associated with patient outcome.Nineteen patients with sIL6R higher than20.5ng ⁄mL(median value for37patients)had significantly worse OS than the remaining18(P=0.008,log–rank test)(Fig.1b).We adopted here the median values for the37patients in order to avoid introducing any selection bias.Univariate analysis by the Cox proportional hazards model(Table3)revealed that only clinical stage(specifically,stages III–IV vs stage II;hazard ratio,3.06;P=0.008)and serum sIL6R(>20.5ng⁄mL vs≤20.5ng⁄mL;hazard ratio, 3.20; P=0.008)were significantly correlated with patient outcome. Multivariate analysis indicated that sIL6R(hazard ratio,2.87; P=0.017)and clinical stage(hazard ratio,2.50;P=0.034) were independent predictors(Table3).Validation in an independent cohort.The relative unrespon-siveness of individuals with a high sIL6R level to PCRT was further validated in the independent cohort of30patients who received the same PCRT protocol(PCRT-validation Cohort). Consistently,the serum level of sIL6R had no significant correlation with the OS of patients in the PCT Cohort(Fig.3c) (P=0.865,log–rank test).Thesefindings suggested that the pathways of tumor cell killing in response to chemotherapy and radiotherapy might differ.DiscussionIn the present study we showed for thefirst time that an increased serum level of sIL6R was correlated with a relatively poor response to PCRT in patients with ESCC.Several reports have documented the prognostic significance of serum CRP and IL6,but no biomarker that can predict the efficacy of PCRT for ESCC has yet been found.(24,25)The absence of any significant correlation with OS or pathological complete response in patients receiving chemotherapy alone(Fig.3) indicated that sIL6R is not a prognostic biomarker.Squamouscell carcinoma is the predominant histological type of esopha-geal cancer in Asian countries(26)and accounts for>90%of surgical cases in Japan.It is generally accepted that the effects of PCRT are more apparent among patients with squamous cell carcinoma than in those with adenocarcinoma.(7,11) Interleukin-6receptor(also known as CD126)is a cell mem-brane cognate receptor for the inflammatory cytokine IL6.(27) Soluble IL6R is the alternatively spliced or proteolytically cleaved form of IL6R that lacks the transmembrane domain. Secreted sIL6R binds to IL6,and the sIL6R⁄IL6complex evokes intracellular trans-signaling through binding to the gp130receptor(CD130)expressed on the surface of various cells.(28)Soluble gp130,or sgp130,a soluble form of gp130,(29)competes with gp130for binding to sIL6R⁄IL6.We measured the serum levels of IL6and sgp130in the PCRT-discovery Cohort,but found that they were not significantly correlated with response to PCRT(data not shown).A high level of sIL6R has been reported in patients with several chronic inflammatory and autoimmune diseases,as well as various malignancies.(27)Secretion and expression of IL6and IL6R have been reported in esophageal cancer cells,(30,31)but the source of the increased sIL6R production in patients with esophageal cancer remains undetermined.Although we were unable to reproduce those previous results in the present study cohorts(Fig.2c and data not shown), some reports have documented that serum CRP and VEGF are correlated with the sensitivity of ESCC to PCRT.(31–33)C-reac-tive protein is produced by hepatocytes in response to inflam-matory cytokines,such as IL6.The production of VEGF isMakuuchi et al.Cancer Sci|August2013|vol.104|no.8|1049also regulated by IL6signaling.(34)As well as sIL6R,we found that the levels offive cytokines(MIP1A,MIP1B,inter-feron-a2,insulin,and MMP3)were also significantly increased in patients who failed to achieve a complete pathological response(Table2).Both MIP1A(also known as CCL3)and MIP1B(also known as CCL4)are cytokines released from macrophages in response to inflammatory stimuli.Alterna-tively,an increase in the levels of a variety of cytokines is considered to reflect a persistent systemic inflammatory status. Suchi et al.(35)reported that overexpression of IL6in ESCC cells induced resistance to cisplatin.The sIL6R⁄IL6signal is transduced into the nucleus through JAK-mediated activation of STAT1⁄3.(27)Efimova et al.(36)reported that radiation-resis-tant human squamous cell carcinoma cells showed constitutive activation of the STAT1pathway.Leu et al.(37)reported that IL6inhibited apoptosis in human esophageal carcinoma cells through activation of the STAT3and MAPK pathways.A small-molecule inhibitor of JAK2(TG101209)has been reported to affect the expression of survivin,thus sensitizing lung cancer cells to radiation.(38)Here we observed that the nuclear expression of pSTAT3was correlated with an increased level of circulating sIL6R(Fig.2e).Based on these experimental and clinicalfindings,it would appear that active sIL6R⁄IL6⁄JAK⁄STAT signaling may be involved in the resis-tance of ESCC to PCRT.However,there were certain limitations to this study in the context of potential clinical application.We observed that a sub-stantial proportion of patients with a low level of sIL6R did not achieve a G3response.Although patients with a high level of sILR6may be potentially excluded from PCRT,we were unable to determine a safe cut-off value,mainly because of the small number of cases examined and the differences in therapy proto-cols among the cohorts.A substantially high proportion of patients in the Prospective PCRT Cohort achieved a complete response,and this included some patients with a high sIL6R level;this outcome was probably attributable to the intense nat-ure of the therapy protocol.It will be necessary to carry out a large prospective study to evaluate the clinical applicability of our presentfindings.We are now planning a phase III clinical trial to compare the efficacies of preoperative combinational chemotherapy and chemoradiotherapy for ESCC.AcknowledgmentsThis work was supported by a Grant-in Aid for Scientific Research from
