诊断与鉴别诊断
诊断标准
主要标准
包括多发性神经病、脏器肿大、内分 泌异常、M蛋白血症和皮肤改变。满 足以上3条主要标准和至少1条次要标 准即可诊断为POEMS综合征。
次要标准
包括水肿、异常出汗、杵状指(趾) 、肺动脉高压、限制性肺病、血栓性 静脉炎、性功能减退、腹泻、低热、 视乳头水肿、脑脊液蛋白增高等。

视盘水肿 颅压增高及视盘水肿比例高,
脑脊液压力增高,脑脊液蛋白增高者占 97% ,有蛋白细胞分离现象。一般无剧烈 头痛、呕吐。

其他症状 病人可有低热(70%)、多汗
(66%),半数病人伴有杵状指(趾),少数可 有白趾(甲),这是本综合征的又一特点。 另外,也可见到肺动脉高压、肾脏病变、 心肌缺血与动脉闭塞,以及红细胞沉降率 加快、血压高和蛋白尿等临床病症。
血浆置换 能缓解症状,但不持久。随病
情发展疗效减弱,早期多次治疗可能会获 得较好的近期效果。

手术治疗 如有孤立性浆细胞瘤者,
可进行局部手术切除。
放射治疗 发现骨髓有孤立性浆细胞
瘤者或手术切除后,可行局部放疗。

自体外周血造血干细胞移植 近年来,国
内外的专家和学者们对自体外周血造血干细胞 移植治疗POEMS综合征进行了研究,并取 得了一定效果。干细胞移植治疗POEMS综 合征的机制还不十分清楚,但是,成功的治疗结 果为造血干细胞移植在神经科的应用开创了一 个新领域,有待于进一步的探讨和研究。

预后
本病能早期被发现并积极进行上述治疗 则有较好效果,确诊较晚再治疗者,其效果 欠佳。5年生存率约为60%,病死者的中位生 存时间为97个月。但据解放军总医院神经科 观察,最早被诊断并得到积极治疗者已痊愈, 至今已12年未见复发。

次,连续3日,或氢化可的松200mg,每日1次, 连续7-10日,或地塞米松20mg,每日1次,连续 7-10日,之后改口服泼尼松50mg,并每3-5日 减5mg,直至10mg时一直维持至3-6个月。如 复发时,可再次类似应用。

免疫抑制剂 在应用激素治疗无效或效
果不佳的情况下,可加用一种免疫抑制剂, 如硫唑嘌呤,他莫昔芬,氟尿嘧啶等。
