沙库巴曲缬沙坦联合达格列净对2_型糖尿病肾病患者的疗效分析

方法选取2022年1月—2023年1月如东洋口医院收治的68例2型DN患者为研究对象，采用随机数表法分为对照组和观察组，每组34例。
对照组予以达格列净单一治疗，观察组予以达格列净和沙库巴曲缬沙坦联合治疗。
比较两组的临床疗效、肾功能指标、血糖变化及氧化应激指标。
结果观察组治疗总有效率高于对照组，差异有统计学意义（P<0.05）。
治疗后，两组空腹血糖、糖化血红蛋白水平均呈下降趋势，且观察组下降更明显，差异有统计学意义（P均<0.05）。
治疗后，两组尿素氮、血肌酐、24 h尿蛋白定量均呈下降趋势，肾小球滤过率呈升高趋势，且观察组改变更明显，差异有统计学意义（P均< 0.05）。
治疗后，两组总抗氧化能力、超氧化物歧化酶、谷胱甘肽过氧化物酶呈升高趋势，且观察组升高幅度更大，差异有统计学意义（P均<0.05）。
结论合用沙布巴曲缬沙坦与达格列净，在2型DN患者治疗中可取得确切疗效，改善肾功能，并实现血糖的良好控制，减轻炎症反应，提高预后。

Methods 68 patients with type 2 DN admitted to Yangkou Hospital of Rudong from January 2022 to January 2023 were selected as the study objects and were divided into con⁃trol group and observation group by random number table method, with 34 cases in each group. The control group was given single treatment of daglipzin, and the observation group was given combined treatment of daglipzin and sakuba⁃tril valsartan. The clinical effects, and renal function indexes, blood glucose changes, and oxidative stress indexes of the two groups were compared. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, fasting plasma glucose (FPG) and glyeosylated hemoglobin A1c (HbA1c) in both groups decreased, and the increase were more significant in the ob⁃servation group, and the difference was statistically significant (all P<0.05). After treatment, blood urea nitrogen (BUN), serum creatinine (Scr) and 24-hour urinary protein quantity (24 hUTP) in both groups showed a decreasing trend, the glomerular filtration rate showed an increasing trend, and the change was more obvious in the observation group, and the difference was statistically significant (all P<0.05). 方法方便选取2021年3月—2023年5月贵州省纳雍县人民医院收治的68例老年CHF患者作为研究对象，按照简单随机抽样法分为两组。
对照组（n=34）给予环磷腺苷葡胺治疗，观察组（n=34）加用沙库巴曲缬沙坦。
对比两组心率变异性、心功能指标、不良反应。
结果治疗后，观察组的RR 间期平均值标准差[（80.95±8.14）ms]、全部窦性心搏RR间期的标准差[（86.51±6.44）ms]、相邻NN之差>50 ms 的个数占总窦性心搏个数的百分比[（12.41±1.19）%]均高于对照组，差异有统计学意义（t=3.947、2.894、8.281，P均<0.05）；观察组的左心室收缩末期容积[（141.21±14.99）mL]、左心室舒张末期容积[（214.88±19.73）mL]均低于对照组，左心室射血分数[（43.02±4.05）%]高于对照组，差异有统计学意义（t=3.419、2.040、6.623，P均< 0.05）；两组不良反应发生率对比，差异无统计学意义（P>0.05）。
结论在老年CHF患者中应用沙库巴曲缬沙坦联合环磷腺苷葡胺治疗效果显著，能够改善心功能指标，控制心率变异性，且药物安全性良好。

The heart rate variability, cardiac function indexes and ad⁃verse reactions were compared between the two groups.Results After treatment, the mean standard deviation of RR in⁃terval in observation group [(80.95±8.14) ms], the standard deviation of RR interval in all sinus beats [(86.51±6.44) ms], and the percentage of difference between adjacent NN >50 ms in total sinus beats [(12.41±1.19) %] were higher than those in control group. The differences were statistically significant (t=3.947, 2.894, 8.281, all P<0.05). 【摘要】采用反相高效液相色谱法测定了枇杷叶中科罗索酸的含量.将样品在90%乙醇溶液中于90 ℃下提取3.0 h,提取3次,浓缩溶液后加水沉淀,将沉淀溶解于甲醇溶液后在反相色谱柱上分离,以甲醇-1.0%醋酸水溶液(体积比为88∶12)为流动相,流速0.8 mL/min,在波长215 nm处进行测定.结果表明:安徽黄山产枇杷叶中科罗索酸含量(以质量分数计)为0.36%,科罗索酸平均加标回收率为99%.该方法方便快速,结果准确可靠.
