[摘要]目的探讨不同呼吸锻炼方法对胸腔积液患者胸腔积液日平均引流量、引流管留置时间的影响,为制定最佳的呼吸锻炼方法提供依据。方法将南阳市中心医院 2013年7月~2014年3月收治的胸腔积液患者60例随机分为对照组、干预Ⅰ组、干预Ⅱ组各20例,对照组采用常规腹式呼吸锻炼,干预Ⅰ组采用常规呼吸锻炼﹢缩唇呼吸锻炼,干预Ⅱ组常规呼吸锻炼﹢吹气球锻炼。比较三组胸腔积液患者胸腔积液量、引流管留置时间等指标的差异性。结果三组中心静脉导管行胸腔闭式引流留置时间分别为:对照组 5.00±1.94 天,干预Ⅰ组4.45±1.56 天,干预Ⅱ组 3.60±1.75 天,三组比较差异有统计学意义(P <0.05);进一步两两比较,仅干预Ⅱ组与对照组间差异有统计学意义(P<0.05),干预Ⅱ组留置时间缩短 1.40 天。三组置管后不同时间引流量比较,差异均无统计学意义(P>0.05);结论对胸腔积液患者实施吹气球呼吸锻炼能缩短引流管留置时间,更有利于患者康复,改善患者气体交换状态且操作简单方便,可视性好,易于开展和推广使用。

Abstract: Objective:Explore different methods of breathing exercises in patients with pleural effusion, pleural effusion daily average drainage, drainage tube indwelling time of impact , provide the basis for the development of the best breathing exercise method . Methods The patients with pleural effusion Nanyang Central Hospital in July 2013 - April 2014 were treated 60 cases were randomly divided into control group, the intervention group Ⅰ ,group Ⅱ 20 cases the intervention and control group using conventional abdominal breathing exercise intervention Ⅰ group using conventional breathing exercises + pursed lips breathing exercises , breathing exercises routine intervention group ⅱ + blower exercise. Pleural effusion compare three groups of patients with pleural effusion , drainage tube indwelling time and other indicators of diversity .Results Three groups of central venous catheter thoracic

drainage retention time were as follows: control gr oup 5.00 ± 1.95 days, the intervention group Ⅰ 4.45 ± 1.57 days, the intervention group Ⅱ 3.60

± 1.76 days, the difference between the three groups was statistically significant (P <0.05 ) ; further pairwise comparisons , only the intervention group and c ontrol group ⅱ between statistically significant (P <0.05) differences in the intervention group Ⅱ shorter retention time 1.40 days . Three different times after catheter drainage , the difference was not statistically significant (P >0.05)Conclusion:For blowing balloons for patients with pleural effusion breathing exercises can reduce drainage tube indwelling time , more conducive to the rehabilitation of patients , improve patient gas exchange status and the operation is simple and convenient , good visibility, easy to carry and promote the use of .
