♦概述:属于糖蛋白类抗原,MW约400000D。

1984年Hilkens等人从人乳脂肪球膜上糖蛋白MAM-6制成小鼠单克隆抗体(II5-DB);1984年,Kufu等从肝转移乳癌细胞膜制成单克隆抗体(DF-3),由这些抗体识别出CA15-3o主要用于乳腺癌的监测和筛选,以及用于评价患者预后情况的指标。

♦临床应用:乳腺癌1)乳腺癌患者30-50%增高,有转移灶者增高可达80%;2)发现癌转移的敏感性比癌胚抗原和组织多肽抗原高,且早于临床发现转移;3)CA15-3是监测病人术后复发情况,特别是癌症转移患者的术后监测的重要指标, 血清CA15-3水平增高,提示乳腺癌的局部或全身复发,且增高早于核素检查和临床检查;4)用于对治疗的随查评估及早期发现转移灶,CA15-3的增高比临床诊断出转移灶要早几个月时间;5)可与CA125联合检查,用于卵巢癌复发的早期诊断;•检测结果由机器自动从标准曲线上查出。

此曲线由仪器通过2点定标校正,由从试剂条形码扫描入仪器的原版标准曲线而得。

试齐小M:链霉亲和素包被的微粒(透明瓶盖),1瓶,6. 5mlo含防腐剂。

R1:生物素化的抗CA153单克隆抗体(灰盖),1瓶,10ml。

含防腐剂。

R2: Ru(bpy)32+标记的抗CA抗3单克隆抗体(黑盖),1≡, 10ml o含防腐剂。

储存和稳定性:存放在2-8度,切莫倒置。

未开封,可稳定至标明的保质期。

开封后,2-8度,12周;放在Elecsys2010, 8周;(室温20 — 25度;开盖时间累计不超过20小时)。

标本采集和准备:血清:按标准常规方法采集。

血浆:EDTA-K3或柠檬酸钠抗凝。

(不要使用肝素抗凝血浆或草酸盐/氟化物抗凝血浆。

确保标本不被肝素污染。

)标本在2 —8度可稳定24小时,-20度可稳定12个月。

只可冻融一次。

含沉淀的标本使用前需离心。

标本和质控品禁用叠氮钠防腐。

标本、定标液和质控品在测定前的温度应与室温平衡;放入仪器后应在2小时内测定以避免蒸发的影响。

CA153定量测定-检验科免疫室作业指导书CA153定量测定1.原理抗原或抗体包被的微粒子,是由多孔高分子粒子制成,具有很好的亲水性,悬浮性极佳,微粒子可与玻璃纤维不可逆结合,从而提高了反应的特异性。

标本与微粒子以一定比例混合,标本中被检物质与微粒子上包被的抗体进行一定时间的反应,以反应终了后,反应液的一部分被移到玻璃纤维上,洗去未反应的被检物质与其它的不要成份,加入基质液,基质液被碱性磷酸所分解生成Methylumbelliferone。

当该物受荧光照射后就产生荧光,测定荧光强度的分化率,从而决定被测物质的浓度。

2.标本采集:2.1标本采集前病人准备:受检者应空腹。

2.2标本种类:血清或血浆。

2.3标本要求:采集血清样本,取被检者静脉血,用无菌取血针抽取病人静脉血3ml,收集干燥试管中,室温放置不超过8小时,2000转/分离心20分钟备用.采集血浆样本,用无菌取血针取被检者静脉血3ml,收集于含有EDTA作抗凝剂的试管中,室温放置不超过8小时,2000转/分离心20分钟备用。

3.标本储存:待测样本室温不超过8小时,4-8℃不超过72小时,-20℃可长期保存,避免反复冻融。

4.标本运输:室温运输。

5.标本拒收标准:细菌污染,严重溶血或脂血标本不能作测定。

6.试剂:6.1试剂名称:CA153诊断试剂盒。

(1) When a ship is lying alongside a quay or some other vessel for the purpose of the processes, there shall be safe means of access for the use of the workers at such times as they have to pass to or from the ship, unless the conditions are such that they would not be exposed to undue risk if no special appliance were provided.
(b) dangerous footways over bridges, caissons and dock gates shall be fenced to a height of not less than 2 feet 6 inches (75 cm.) on each side, and the said fencing shall be continued at both ends to a sufficient distance which shall not be required to exceed 5 yards (4 m. 50).

用途:
用免疫学方法定量测定人血清或血浆中的CA15-3II。

辅助乳腺癌病人的治疗监测。

与其它临床和诊断措施相结合,CA15-3II系列动态测定有助于II期和III期乳腺癌病人治疗后再复发的早期发现;监测乳腺癌转移病人对
治疗的反应性。

电化学发光免疫测定试剂,适用于罗氏Elecsys1010、2010和E170免疫测
定分析仪。

概述:
CA15-3测定值由夹心法中所用的二种单克隆抗体(MAb)115D8和DF3决定。

115D8特异性针对人乳脂球膜,而DF3特异性针对人转移性乳腺癌的膜提取成份。

能与115D8和DF3反应的决定簇存在于一种称为MAM-6的糖蛋白分子上,该
种抗原属于唾液酸化的糖蛋白亚类,又称多态性上皮粘蛋白(PEM)。

正常情况下,PEM只存在于腺体细胞腔的分泌物中,不出现在血循环中。

当细胞恶变时,基底细胞膜渗透性增强,PEM可在血清中由CA15-3方法检测出来。

原理:采用双抗体夹心法原理,整个过程18分钟完成。

· 第1步:20ml预先用Elecsys通用稀释液作1:10稀释的标本、生物素化的抗CA15-3单克隆抗体和钌(Ru)标记的抗CA15-3单克隆抗体混匀,形成夹心复合物。

· 第2步:加入链霉亲和素包被的微粒,让上述形成的复合物通过生物素
与链霉亲和素间的反应结合到微粒上。

· 第3步:反应混和液吸到测量池中,微粒通过磁铁吸附到电极上,未结
合的物质被清洗液洗去,电极加电压后产生化学发光,通过光电倍增管
进行测定。

·检测结果由机器自动从标准曲线上查出。

此曲线由仪器通过2点定标校
正,由从试剂条形码扫描入仪器的原版标准曲线而得。

试剂:
M:链霉亲和素包被的微粒(透明瓶盖),1瓶,6.5ml。

粒子浓度0.72mg
/ml,生物素结合能力: 470ng生物素/mg粒子。

含防腐剂。

R1:生物素化的抗CA15-3单克隆抗体(灰盖),1瓶,10ml。

浓度1.75mg
/l,磷酸缓冲液0.02mol/l,pH6.0。

含防腐剂。

R2:Ru(bpy)32+标记的抗CA15-3单克隆抗体(黑盖),1瓶,10ml,浓度
10mg/l,磷酸缓冲液0.1mol/l,pH7.0。

含防腐剂。

储存和稳定性:存放在2-8度,切莫倒置。

未开封,可稳定至标明的保质期。

开封后,2-8度 12周;放在Elecsys2010或E170上,8周;放在Elecsys1010上,4周(使用后放回2-8度 ;如置20-25度,约20小时)。

标本采集和准备: 血清:按标准常规方法采集。

血浆:肝素(锂、钠、铵),EDTA-K3抗凝。

标本在2-8度可稳定5天,-20度可稳定3个月。

含沉淀的标本使用前需离心。

不要加热灭活标本。

标本和质控液禁用叠氮钠防腐。

标本、定标液和质控品在测定前的温度应与室温平衡。

检查试剂与消耗品是否充足。

使用前需混匀微粒。

仪器通过扫描试剂盒条形码自动输入测试所需的特异性参数,不需手工输入。

如果特殊情况下仪器无法阅读条形码,可以手工输入15位数字。

E170/Elecsys2010:将冷藏试剂预温到20oC后放置于仪器的试剂盘上,避免产生泡沫。

仪器
自动控制试剂温度和开/关试剂瓶盖。

Elecsys1010:
将冷藏试剂预温到20-25度后放置于仪器的样品/试剂盘上(室温20-25
度,避免产生泡沫。

使用前手工打开试剂瓶盖,使用后手工关闭试剂瓶盖
并放回2-8 度。

定标:
每批CA15-3II试剂有一条形码标签,含有该批试剂定标所需的特殊信息。

应用CA15-3II CalSet定标液定标。

定标频率:
每批试剂必须用新鲜试剂(试剂经仪器注册24小时以内)标定一次,
如再次标定即根据下列要求:
E170/Elecsys2010:
· 一个月(同一批号试剂)
· 7天(放置仪器上的同一试剂盒)
Elecsys1010:
· 每一试剂盒做一次· 7天(室温20-25 度)
· 3天(室温25-32 度)各种分析仪均适用的情况:·根据要求进行标定:如质控结果超出范围时。

质控:Elecsys肿瘤标志物质控品(Precicontrol Tumor Marker)1和2以及其它合适的质控品。

各浓度区域的质控至少每24小时或每一次定标后测定一次。

质控间隔期应适用于各实验室的具体要求。

检测值应落在确定的范围内,如出现质控值落在范围以外,应采取校正措施。

计算:
对每一个标本,仪器会自动计算CA15-3含量,单位是U/ml或kU/l。

干扰因素:
该方法不受黄疸(胆红素<65mg/dl)、溶血(血红蛋白<3.0g/dl)、脂血(脂质1500mg/dl)和生物素<100ng/ml干扰。

接受高剂量生物素(>5mg/天)治疗的病人,至少要等最后一次摄入生物素8小时后才能采血。

不受类风湿因子干扰(1500U/ml)。

28种常用药物经试验对本测定无干扰。

CA15-3浓度高达20000u/ml也不出现钩状效应。

接受过小鼠单抗治疗或体内诊断的病人可能会出现假阳性反应。

Elecsys CA15-3II测定结果应结合病人病史、临床其他检查结果综合起来进行诊断。


检测范围:1.00─300 U/ml
稀释:高于检测范围的标本可用通用稀释液进行自动预稀释。

建议1:10稀释。

稀释后的标本CA15-3II含量必须高于30U/ml。

如用手工稀释结果应乘上稀释倍数。

如果是机器自动稀释,机器会自动计算结果。

 
正常参考值:
≤25U/ml(第95%位数,n=272)。

如有必要,各实验室应自己测定一个正常值范围。
