体 周 围骨组 织情 况 、 植 体 周 软组 织 与 修 复体 情 况 、 者满 意 度 等指 标 。结 果 ： 种 患 １例 即刻种 植 修 复 后 １年 ， 侧 牙 龈 唇

Straumann，牙种植系统，口腔修复Straumann牙种植系统在口腔修复的应用【摘要】目的探讨Straumann牙种植系统在口腔修复中的特点及临床效果。
方法牙或牙列缺失患者42例,植入ITI Straumann牙种植体98枚,其中2例为即刻种植，2例为下颌肿瘤术后种植体支持的功能性重建。
6周～3个月后行上部结构修复。
定期随访口腔卫生、种植体周骨结合状况及骨吸收速度、种植体周软组织与修复体情况。
结果种植成功率98.98%。
1枚即刻种植体第1年唇侧牙龈退缩2.5 mm,其余种植体第1年周围骨吸收距离(0.93?0.27)mm；第2年后种植体周围骨均稳定,无吸收。
4枚种植体植入后分别出现愈合帽松脱及上部结构松动,病人对舒适度及咀嚼效率评价良好。
结论Straumann牙种植系统有负载时间早、潜入与非潜入种植皆宜、手术次数少、疗程短以及成功率高等特点。
【关键词】牙种植体牙缺失义齿修复术各种原因引起的牙缺失导致牙列缺损可引起咀嚼功能下降,影响患者生活质量。
传统的活动及固定修复方式各有局限性,难以达到完美的修复效果。
种植修复因其结构类似正常牙、对余留牙无损伤、咀嚼及美观效果恢复良好等优点而受到人们的青睐,种植技术的日渐完善及成熟种植系统的开发应用，使得种植修复已经逐渐成为牙颌系统重建的主要方式。
目前国际上主流的种植系统如Brankmark、ITI、Lifecore等各有优缺点,在应用上也各有特点[1]。
笔者2003年2月～2007年4月使用ITI Straumann牙种植系统重建缺牙患者的牙颌系统,取得满意效果。
1对象与方法1.1对象42例中,男性28例,女性14例,年龄(46?4.3)岁(18～65岁),共植入种植体98枚;其中2例为即刻种植,2例为下颌骨肿瘤手术后种植体支持的功能性重建,9例进行不同类型的软硬组织成型手术。
种植体部位及数量:上颌前牙区12枚,双尖牙区15枚,后牙区16枚；下颌前牙区6枚,双尖牙区17枚,后牙区32枚。

All of the crowns were CAD/CAM all-ceramic crows using cement-retained mode.During the follow-up of 1 to 2 years,mPLI and mSBIand other clinical indicators were monitored and compared with the health status of the soft tissue,while papilla positions were recorded to evaluate the gingival shaping situation.Results:After 1～2 years of follow-up observation, there was no significant difference between custom titanium abutments and finished abutments in gingival health(P>0.05).However, the gingival shape was better and the position of papillae was more stable after using custom abutment.There were significant differences between the two groups(P<0.05).Conclusion:Compared with the finished abutment, the custom abutment can be designed according to the height and shape, so as to provide the most suitable transgingival height for patients, and achieve the perfect effect in aesthetics,biocompatibility,soft tissue remodeling and restoration.It is a good way for posterior implant prosthesis.【Key words】Posterior implants; Custom abutment; Finished abutment; Gingival papilla近年来，随着种植义齿的广泛运用，人们对种植修复的效果要求不再局限于远期的成功率和恢复咀嚼功能，希望能够得到更加自然、舒适、美观的修复效果[1-3]。

Straumann® SmartSmart 产品说明种植体骨内段直径和色码常规颈 (RN) 和宽颈 (WN) 平台型美学种植体有三种骨内段直径可选：∅ 3.3 mm、∅ 4.1 mm 和∅ 4.8 mm。
统一的色码方便识别器械和种植体。
色码●黄色种植体骨内段直径 3.3 mm●红色种植体骨内段直径 4.1 mm●绿色种植体骨内段直径 4.8 mm螺距美学 (RN/WN) 种植体上的螺距在∅ 3.3 mm 种植体上为 1 mm，在∅ 4.1 mm 和∅ 4.8 mm 种植体上为 1.25 mm。
种植体长度Straumann® 美学 (RN/WN) 种植体有 6、8、10、12 和 14 mm 五种长度。
6 mm 美学种植体的可用性取决于种植体直径和种植体材料。

【病例解析】Straumann®BLT联合Variobase®基台进行即刻种植即刻修复拔牙后即刻植入种植体可能是延期植入的可行替代方案。
然而，它需要仔细选择病例。
对于唇线高的患者，种植体位置和种植体周围的可用软组织变得越来越重要。

患者的唇线位置很高，牙龈暴露，所以，她对治疗效果的美观性期望非常高（图3）。
我们决定应用即刻种植体代替上颌左中切牙。
（图 2）（图 3）外科手术局部麻醉后，完成不翻瓣的微创拔牙以保留近中龈乳头及剩余颊侧骨板和舌侧骨板。
仔细检查拔牙窝是否开裂和穿孔，并使用刮匙清除残留的牙周纤维。
牙槽窝清创术后，在21 牙位植入Straumann®Bone Level Tapered Implant 士卓曼骨水平锥柱状种植体（Regular CrossFit™连接组件Ø4.1mm SLA®14mm）（FDI）（图 4）。

当用35 Ncm 的扭力植入时，种植体表现出极佳的初期稳定性。
然后，放入定制的临时钛柱（临时基台），手动拧到种植体上，使用定位导板修整，以确保高度正确（图 6）。
