2.受委托人有权代表委托人与检验机构进行沟通协调,确保申请流程的顺利进行。
3.受委托人有权代表委托人领取船舶修理质量证明书,并确保其内容准确无误,符合相关法律法规的要求。
四、委托期限
本委托书自双方签字盖章之日起生效,至船舶修理质量证明书成功办理完毕,或双方协商终止本委托关系时止。
五、费用承担
1.受委托人在办理委托事项过程中产生的合理费用,包括但不限于检验费、差旅费等,应由委托人承担。
4.2受委托人应在发生费用时,向委托人提供费用清单,并按照双方约定的方式支付。
第五条保密条款
5.1受委托人应对在办理船舶修理质量证明书过程中获得的委托人的商业秘密、个人隐私等信息严格保密。
5.2除非依法应当向相关机构提供,否则未经委托人书面同意,受委托人不得向第三方披露前述信息。
第六条法律责任
6.1受委托人在办理委托事项过程中,如因故意或重大过失导致委托人损失,应承担相应的法律责任。
1.2受委托人负责收集、整理并提交船舶修理质量证明书所需的所有文件和资料。
1.3受委托人负责跟进船舶修理质量证明书的办理进度,并及时向委托人报告相关情况。
1.4受委托人在办理船舶修理质量证明书过程中,有权代表委托人签署相关文件和承诺。
第二条委托权限
2.1受委托人有权在委托人授权范围内,代表委托人办理与船舶修理质量证明书相关的所有事宜。

1.受委托人应代表委托人,就委托人所拥有的船舶(以下简称“该船舶”),向具有资质的船舶修理质量检验机构(以下简称“检验机构”)申请并取得船舶修理质量证明书。

• 是船舶修理工程的基本技术文件,反映了船方对修理工程 提出的内容、范围及要求;也是船厂估工、估料、编制预 算、报价、确定船期、签订修船合同的依据; • 对公司机务部门而言,修理单是安排修船计划、分配修船 费用和签订修船合同的依据。 • 船舶主管人员(大副、轮机长)必须深入实际,仔细调查 研究,全面掌握船舶实际技术状况,特别是船体构件、动 力设备、装卸设备、通信设备等关键问题心中有数。广泛 征求各设备主管人员的意见,形成初稿,在各部门会议上 讨论定稿。力求做到技术合理、部位清楚、情况正确、以 保证修船工程顺利进行和达到规定的质量标准。

• “主要工程摘要单”是机务部门、船厂安排下一年度修船 计划和申请机电设备、特殊材料及制造预制件的依据。如 下一年要修理的船舶应在当年的6月1日前编写完毕交机务 审定,“主要工程摘要单”应一式四份,机务审定后留一 份,交船厂二份,退回船上一份。 • “主要工程摘要单”只是表达出主要工程项目及范围轮廓。 编写时要简明扼要,不写零星工程、易耗部件、经常修理 工程、工艺方法、技术要求及因修理引起的附带工程。只 需写明主要工程、特殊材料、机电设备和特殊要求。在船 舶小修时若没有主要工程项目,而且不需要机电设备和特 殊材料,只要在项目栏内写明“一般工程维修单”即可。
3. 船舶修理单的内容
• 1)注明修理部位和名称 • 凭修理单自己可以找到各修理项目的位置。船 壳外板可参照外板展开图来说明需换钢板的部 位。如“PC4需换新“是左舷船底C列第4块 板需换新。 • 2)写明损坏的范围和程度 • 根据实际情况,写明属于何种损坏及损坏的范 围、程度,如锈蚀、锈死、弯曲、变形、折断、 裂缝等。
2. “船舶修理单”的编写
• “船舶修理单”是船方和机务部门向船厂提 出修理工程的正式文件。“船舶修理单” 应一式四份,机务审定后留一份,交船厂 二份,退回船上一份。一般小修船舶应在 进厂前60天送机务部门,机务部门审定后 在进厂前45天送船厂;检修船舶应在进厂 前4个月送机务部门,机务部门审定后在进 厂前80天送船厂。
• 4)修船标准:应根据本次修船目的和船龄而定, 凡能维持到下次修理的,本次一般不列修理项目, 尤其高龄船舶应以维持为基本原则。 • 5)修理单的编排:一般都在半年前就做准备,应 做全面、仔细的工作,减少追加工程,以防超出 预算和打乱船期。 • 6)修理单的编制应按工程类别,分门别类,重点 突出,层次分明,工程完善。 • 文字力求准确、明了、严谨。一般应遵循从前到 后,从上到下,按甲板层次,货舱顺序逐项编制。
4.编制修理单注意事项
• 1)修理单按坞修工程(轻载水线以下)、甲板工 程、轮机工程及电气工程四个 • 部分分别立账编制,按规定的份数及时间送机务 部审核。 • 2)自修项目所需的物料和备件列出清单。船员计 划性的自修项目应单独列出清 • 单上报。 • 3)工程项目的确定:在保证船舶安全运转、保持 船级、保证船舶适航状态下, • 厉行勤俭节约的原则,添装与大的改装一般不予 审报。
• 3)写明修理方法,根据损坏情况提出具体要求根 据实际情况提出修理方法,如局部割换、覆补、 校正、换新等。例如变形校正要写明是拆装校正 还是现场校正。 • 4)写明工程修理时所需的材料的名称、规格、型 号及数量对某些特殊的设备备件应标明厂家。特 殊的构件应画出图样,注明各尺寸和何种质材, 有利于厂方提前准备,缩短修船期。 • 5)写明因修理工程所引起的附带工程和船舶一些 测试和试验工作。
