1 •此表为医疗机构向设置审批的卫生行政部门申请分类核定性质时专用.一式三份,分别留主管部门.申报单位.设置审批的卫生行政部门各一份。
没有主管单位的.一式二份。
2、医疗机构简况由申请人据实填写。
(1)设置单位:指政府机关、事业单位、企业、社会团体和其他社会
组织及个人。
(2)主管部门:是指与医疗机构具有直接行政隶属关系的上级主管部门或单位,没有的可不填写。
(3)资金总计指申报时上一年度固定资产与流动资金总额,包括各种
借贷资金在内。
(4)投资渠道来源和性质:投资渠道来源指政府机关.事业单位、企
业、社会团体和其他社会组织及个人。
资金性质指财政投入■法人和个人投资.社会捐赠.贷款。
(5)收支结余的使用或盈余分配方式,须填写分配依据的政策•制度;
依据合资合作协议分配的要注明分配办法;没有具体依据的填写"自主分配使用"。
3.审核意见由医疗机构的主管单位.所在地的市、县(区)级卫生行
政部门和设置审批卫生行政部门分别填写。
(1)没有上级主管单位的医疗机构.不填"主管单位意见"栏。
(2)政府办医疗机构中.核定与注册登记权限在上一级人民政府卫生行政
部门的,由举办医疗机构的政府卫生行政部门在"主管单位意见"栏中填写本级初审意见,再报上一级卫生行政部门核定性质。
(3)政府办医疗机构变更经营性质,或转为其他非营利性医疗机构的,
主管和核定性质的卫生行政部门应分别会同同级计划、财政部门共同核电并分别在备注栏签署意见和单位盖章。
(4)其他社会组织和个人投资举办的医疗机构■申请开办非营利性医疗机构的.主管和核定性质的卫生行政部门应分别会同同级计划.财政部门共同核定.并分别在备注栏中签署意见并盖单位公章。
