山东省实验中学学籍管理规定一、招生(一)我校根据省市教育主管部门下达的招生计划,主要面向济南市招生。

中美高中课程实验班计划单列。

(二)新生录取后须按规定时间到校办理入学手续,因故不能报到的,要由学生家长持有关证明及时向学校请假,请假时间最长不得超过一个月。

不按时报到且未及时办理请假手续的,除因不可抗力等事由外,取消入学资格。

(三)新生通过填写报名副表表达校区选择志愿。

报名副表中的校区选择志愿和学生住址是我校对新生分配校区的首要依据。

(四)新生实行电脑随机均衡编班,由班主任以抽签方式确定班级序号。

(五)新生使用全省统编学号注册。

二、转学(一)外地(校)学生需转入我校就读的,应持原就读学校出具的转学证明及本人学籍档案到校联系,经校长审核签字批准,然后由学籍管理员持学生转入申请表及转学材料及相关转学证明材料,到市级学籍主管部门办理转学手续。

学生由省内其他地市转入我校,还需同时提供本人原全省统编学号。

(二)我校学生转往外地的,由学生本人及家长持接收学校证明及书面的个人转学申请,教导处分管主任审核,报分管校长同意,由学籍管理员开具转学证明并为其办理学籍档案(盖章加封)转出手续,学生学籍卡片仍由我校留存。

(三)学生转出(出国)或退学,若入校时缴纳择校费,根据济南市教育局济教基〔2005〕4号文件《关于高中阶段学校因学生转学、退学退还其所交费用的暂行规定》,确定退还比例。

(四)学生出国,一般由家长亲自以书面形式提出申请,待签证批准后再办理退学、退费手续,出国之前语言进修或待签阶段按请假办理,并以书面形式确认家长的监护责任。

(五)我校原则上不办理市内转学手续。

未经我校同意,因学生原所在学校单方面办理转学手续造成学生失学的,我校不承担责任。

(六)新生原则上不办理转入手续,学生处分期内一般不办理转学手续,高三一般不办理转学手续。

(七)学生转学一般于学期初、末办理。

三、借读(一)我校一般不接收学生借读。
