免疫全项检测各项的临床意义
1、DNP抗体:
对人血清中与SLE相关的抗脱氧核糖核蛋白(anti-DNP)抗体进行定性。

几乎所有活动期的SLE均可检出此种ANA,多种自身免疫病时此种抗体检出率也达20~30%。

[局限性]
①硬皮病、类风湿性关节炎、皮肌炎患者及各种结缔组织病患者血
清在SLE玻片上也可能产生凝集反应。

②象肼苯达嗪、异烟肼、普鲁卡因胺以及抗惊厥药物所引起系统性
红斑狼疮综合症。

③污染的、血脂高的或明显溶血的血清可能产生非特异性反应。

④极高水平的抗体会影响凝集反应程度。

2、ANA抗体:
ANA抗体是以细胞的核成分为靶抗原的自身抗体的总称。

对SLE等自身免疫疾病有重要的诊断价值,SLE初诊阳性率为91.4%,活动期可达100%(内科学讲SLE中约80~95%病例ANA呈阳性),多次检测ANA抗体阴性可排除SLE。

ANA抗体阳性的其它疾病:肝脏疾患、溃疡性结肠炎、巨球蛋白血症、高球蛋白血症性紫癜、淋巴瘤、特发性自身免疫性贫血、恶性贫血、淋巴细胞性甲状腺炎、恶性肿瘤、重症肌无力、血管炎、结核等。

ANA抗体阳性与激素的关系:SLE病人用激素治疗后,ANA 变化37.5%转为阴性,而62.5%的病人虽然阳性但效价大大降低。

ANA抗体滴度:SLE 92.5% ANA在1∶40以上,活动期在1∶160以上(1∶60 ~ 1∶2560之间)。

SLE以外疾病78%在1∶10左右,其它结缔组织病的ANA滴度都在1∶80以下。

3、as-DNA:
抗DNA抗体检测是诊断SLE的特异性较强的试验,高滴度的DNA抗体几乎仅见于SLE病人。

as-DNA抗体的特异性较差,除系统性红斑狼疮外,其它结缔组织病、药物诱导的狼疮、慢性活动性肝炎也可检出。

DLE等未见阳性。
