肝血管瘤的超声诊断体会

论著·临床辅助检查CHINESE COMMUNITY DOCTORS 中国社区医师2018年第34卷第20期影像学检查在肝血管瘤的检查中非常重要,超声是肝血管瘤诊断中最常用的无创伤性检查方法,作为肝血管瘤的初筛手段,在基层医院广泛使用,对初筛不能确诊的血管瘤患得可以建议去上一级医院,可在超声引导下穿刺、活检。
亦可做超声造影或CT 检查。
回顾我院2014年2月-2016年10月门诊检查的患者资料,经整理现将这几年肝血管瘤的超声诊断总结如下。
资料与方法2014年2月-2016年10月收治肝血管瘤患者350例,经病理、手术及CT 确诊。
其中男138例,女212例;年龄20~86岁,平均40岁。
多数患者无明显临床症状,在超声检查肝脏时被偶然发现,亦有少数病例肝区或右上腹疼痛,体积较大压迫胃肠道发生食欲不振、消化不良、饭后饱胀、恶心、呕吐等症状。
极少数肝包膜下血管瘤可破裂出血而呈急腹症症状。
方法:仪器设备:使用本院超声诊断仪,GE LOGIQ S8,PHILIPS 飞凡,探头频率3.5MHz,为了保证图像的清晰显示,检查前1d 嘱患者晚上少吃或不吃过多油腻食物,晨起空腹(10~12h),当天如果同时检查胃肠钡餐透视,应先行超声检查,患者如若腹内积气积便较多,宜于前夜服用泻药以促使排出粪便和消化道内的积气,并开始禁吸烟,患者常规仰卧于右侧检查床,双臂上举,使肝区体表投影位置充分暴露于视野中,探头置于右侧肋缘下,上下晃动倾斜,仔细检查,显示胆囊、肾脏及第一、第二肝门等结构。
探头再置于剑突下纵切,左右两边依次扫查,必要时嘱患者深吸气后屏气,避开肋骨及肺气干扰,观察记录肝脏的形态、内部回声等,是否有脂肪肝,是否有肝硬化等。
再详细记录病灶部位、大小、数目、内部回声及边界回声,彩色多普勒观察病灶内部及周边血流情况。
扫查步骤的总的原则是有次序的全面观察,尽量不漏掉病变。
注意事项:①应特别注意在邻近肝脏表面及底面的包膜下区。

Thus, the impressed voltage to the separating voltage is made low when an original is in black solid or close to it, or made high when in white solid or close to it, improving separating performance. 方法:回顾并对照分析41例声像图为点状强回声、无结节的甲状腺疾病和15例声像图无点状强回声、无结节的桥本甲状腺炎及相应的病理学特征。
结果:10例伴点状强回声的桥本甲状腺炎病理学改变以纤维增生为主,15例不伴点状强回声桥本甲状腺炎病理学改变以淋巴细胞浸润为主,纤维轻度增生。
31例弥漫性硬化型乳头状癌病理学改变以砂粒体为主,亦见不规则钙化、胶质凝集体等。
伴点状强回声的桥本甲状腺炎、弥漫性硬化型乳头状癌在病理学改变中的纤维增生(χ2=10.146,P =0.001)、胶质凝集(χ2=4.603,P =0.032)与砂粒体(χ2=35.757,P =0.000)的差异有统计学意义。
结论:砂粒体、胶质凝集体、纤维索、营养不良性钙化等均可形成甲状腺声像图上的点状强回声,在检查过程中应注意识别其成因。

Results: The pathological changes of 10 Hashimoto ’s thyroiditis cases with hyperechoic foci were mainly fibrous hyperplasia and 15 Hashimoto ’s thyroiditis cases without hyperechoic foci were mainly lymphocyte infiltration accompanied by mild fibrous hyperplasia. The pathological changes of 31 diffuse sclerosing thyroid papillary carcinoma cases were mainly psammoma bodies, and dystrophic calcification as well as colloid also can be seen. Conclusion: The appearance of thyroid ’s hyperechoic foci on B-ultrasound can be caused by psammoma bodies, colloid, fiber and dystrophic calcification and so on. The ovarian scans were defined as follows: (1) ovulation: normal follicular development with ultrasound evidence of ovulation; (2) follicular cyst: persistent, bizarre growth of follicle >25 mm in diameter which failed to rupture; (3) luteinized unruptured follicle (LUF): normal follicular growth up to 25 mm with normal serum estradiol and progesterone levels without follicular rupture; (4) immature follicles: small follicles c 10 mm in diameter seen in both ovaries but with no evidence of ovulation on ultrasonography. 2.3. Collection of blood samples Blood samples (approximately 5 ml) were collected once daily during one pretreatment, two treatment and one post-treatment cycle from day 8 to day 24 of the menstrual cycle and thereafter on alternate days until the onset of menstruation. Serum was separated and kept at -20°C until further processed. Serum levels of estradiol, progesterone, luteinizing hormone and folliclestimulating hormone were measured by specific radioimmunoassay as described earlier [l]. 3. Results A total of 32 treatment cycles were studied, 16 in each group. Table 1 shows the effect of oral and intranasal NET on ovarian follicular growth as evaluated by ultrasonography. 3.1. Intranasal group All eight women (eight cycles) had ultrasonographic evidence of normal ovulation in the pretreatment cycle. Their follicular diameter ranged from 17 to 25 mm before rupture. A total of 16 treatment cycles were studied. Treatment with NET had no apparent effect on ovarian follicular growth in seven cycles (Table l), during which normal follicular growth and ovulation were observed. In six of these cycles, the opposite ovary appeared normal but without follicles, whereas in one cycle (second cycle of volunteer no, 9) with normal ovulation in the left ovary, a growing follicular cyst was seen in the right ovary. The size of the cyst grew to 29 mm before it disappeared in the middle of the post-treatment cycle. The remaining nine cycles were anovulatory. In seven cycles a growing follicular cyst was seen, the

Ultrasonographic monitoring of ovarian follicles in women . using norethisterone for contraception
In earlier studies the effect of NET on ovulation was mainly judged according to the endocrine profile. Now that high-resolution ultrasound scanning techniques are available, ovarian follicular events can be better studied when combined with hormonal parameters [lo]. Although an in-depth study was carried out to examine the effect of oral and intranasal NET on ovarian folliculogenesis, endocrine profile (plasma gonadotropins, estradiol and progesterone), cervical mucus and endometrial histology, this paper describes the ovarian function with serial ultrasound monitoring of the follicles. 2. Materials and methods Sixteen healthy, sterilized women aged 28-39 years, weighing between 46 and 54 kg and having regular menstrual cycles of 24-32 days volunteered for the study. Each woman underwent a complete gynecologic (including Pap smear) and medical examination. Blood samples were analyzed to assess hepatic and renal function. Women receiving intranasal NET also had an otorhinolaryngological examination. 2. I. Drug dose regimen NET 300 pg was administered orally (n = 8) or intranasally (n = 8) once daily starting from day 1 of the menstrual cycle and lasting for two consecutive cycles until the last day of the cycle. 2.2. Ultrasonography Each woman was scanned during four consecutive menstrual cycles comprising one pretreatment (control), two treatment and one post-treatment cycle. During control and posttreatment cycles, no drug (NET) was administered. Serial pelvic ultrasonography was performed beginning on day 7 of the menstrual cycle using a Philips scanner (model SDR 1550 X R, Bombay, India) with a 3-MHz sector transducer. The scans were performed with a full bladder until the growing follicle collapsed/ disappeared, or throughout the menstrual cycle if a follicular cyst was observed. The size of the follicle was reported as the mean of the two largest diameters. Follicular rupture was diagnosed when the follicle disappeared or decreased in diameter by more
