Form PER – 2: Resume of Proposed Personnel 人事表—2：推荐人的简历：The Bidder shall provide all the information requested below:投标人应当提供以下要求的所有信息：Summarize professional experience in reverse chronological order. Indicate particular technical and managerial experience relevant to the project.（时间）倒序专业经历概要。

表明相关项目的特殊技术和管理经验。

Site Organization现场组织(Note: Evaluation of the Bidder’s Site Organization will include an assessment of the Bidder’s capacity to mobilize key personnel for the Contract consistent with its proposal regarding work methods, scheduling, and material sourcing in sufficient detail and full y in accordance with the requirements stipulated in Section 6 (Employer’s Requirements)).（注:投标人的现场组织的评估应包括评估投标人调动那些与合同所拟定的相关工作方法相一致的关键人员的能力、计划调度能力、充分详细的材料源和完全按照第6节规定的要求(雇主的需求)）Method Statement施工方案Bidder’s Qualification投标人资格To establish its qualifications to perform the contract in accordance with Section 3 (Evaluation and Qualification Criteria) the Bidder shall provide the information requested in the corresponding Information Sheets included hereunder.投标人应当在相应的信息表中提供要求的信息，按照第3节(评价和资格标准) 建立其履行合同的资格。

Form ELI - 2: JV Information Sheet 表ELI-2 合资信息表Each member of a JV and Specialist Subcontractor must fill in this form 合资成员和专业分包商必须填此表Specialist Subcontractor is a specialist enterprise engaged for highly specialized processes which cannot be provided by the main Contractor.专业分包商是主要承包商不能提供的所从事高度专业化程序的专业企业。

Form LIT – 1: Pending Litigation and Arbitration表LIT – 1:未决诉讼和仲裁Each Bidder or member of a JV must fill in this form if so required under Criterion 2.2 of Section 3 (Evaluation and Qualification Criteria).如果需要，每个投标人或合资成员企业必须按照第3章2.2的标准(评价和资格标准) ,填写这张表格。

❑No pending litigation没有未决诉讼❑Below is a description of all pending litigation and arbitration involving the Bidder (or each JV member if Bidder is a Joint Venture).下面是对所有涉及投标人(或每个合资企业成员，如果投标人是合资企业)未决诉讼和仲裁的描述。

Form FIN - 1: Historical Financial Performance表FIN - 1: 历史财务业绩Each Bidder or member of a JV must fill in this form每个投标人或合资成员必须填写此表Information from Balance Sheet资产负债表的信息Information from Income Statement损益表的信息Form FIN - 2: Average Annual Construction Turnover 表财-2：年平均建设营业额Each Bidder or member of a JV must fill in this form每个投标人或合资成员必须填写此表The information supplied should be the Annual Turnover of the Bidder or each member of a JV in terms of the amounts billed to clients for each year for work in progress or completed, converted to US Dollars at the rate of exchange at the end of the period reported.提供的信息应该是投标人或每个合资成员企业的年营业额，在数量方面每年为客户所完成的工作量或进度工作量，用报告期结束时的汇率转换成美元计。

Form FIN – 3: Availability of Financial Resources 表财-3 金融资源可用性Specify proposed sources of financing, such as liquid assets1, lines of credit, and other financial resources (other than any contractual advance payments) available to meet the financial resources requirement indicated in Form Fin-4..规定建议的资金来源,如流动资产,信贷额度和其它金融资源(除了任何合同预付款)可用来满足表格财务表-4资金来源要求的可用性资金。

1Liquid Assets mean cash and cash equivalents, short-term financial instruments, short term available-for-sale-securities, marketable securities, trade receivables, short-term financing receivables and other assets that can be converted into cash within one year.2流动资产指着现金和现金等价物,短期金融工具,,在一年内可以转化为现金短期可售债券、有价证券、应收款项、短期融资应收账款和其他资产。

Form FIN- 4: Financial Resources Requirement 财务表—4：财政资源需求Bidder (or each JV partner) should provide information indicated below in order to calculate the aggregated financial resources requirement, which equals the sum of: (i) the Bidder’s (or each JV partner’s) current commitments on all contracts that have been awarded, or for which a letter of intent or acceptance has been received, or for contracts approaching completion, but for which an unqualified, full completion certificate has yet to be issued and (ii) financial resources requirement for subject contract as determined by the Employer. Bidder must also disclose any other financial obligations that could materially affect the implementation of subject contract if such contract were to be awarded to the Bidder.投标人(或每个合资人)为计算合计金融资源需求应提供下列信息,这总和等于:(i)投标人的当前(或每个合资人)对所有现有被授予合同的承诺,或者对意向书或已经收到验收证书,或接近完成的合同,但对于一个不合格的,全部完工证书尚未发放的除外,(ii)金融资源要求主体合同由雇主决定。
