1、第一栏：出口方此栏不得留空，填写出口方的名称、详细地址及国家（地区）。
若经其它国家或地区需填写转口商名称时，可在出口商后面加填英文VIA，然后再填写转口商名称、地址和国家。
例如：SINOCHEM INTERNATIONAL CHEMICALS CO.,LTD.NO.40,FUCHENG ROAD,BEIJING,CHINAVIA HONGKONG DAMING CO.LTD.NO.656,GUANGDONG ROAD,HONGKONG此栏不得填写国外出口商名称。
2、第二栏：收货方应填写最终收货方的名称、详细地址及国家（地区），通常是外贸合同中的买方或信用证上规定的提单通知人。
但往往由于贸易的需要，信用证规定所有单证收货人一栏留空，在这种情况下，此栏应加注“TO WHOM IT MAY CONCERN”或“TO ORDER”，但不得留空。
若需填写转口商名称时，可在收货人后面加填英文VIA，然后再填写转口商名称、地址、国家。
3、第三栏：运输方式和路线海运、陆运填写装货港、到货港及运输路线。
如经转运，还应注明运地。
例如通过海运，于1995年7月1日由上海港经香港转运至鹿特丹港，英文为“FROM SHANGHAI TO HONGKONG ON JUL.1,1995,THEN TRANSHIPPED TO ROTTERDAM BY VESSEL”，或“FROM SHANGHAI TO ROTTERD AM BY VESSEL VIA HONGKONG”。
4、第四栏：目的地国家（地区）即货物最终运抵港，一般应与最终收货人或最终目的港国别一致，不能填写中间商国家名称。
