塞来昔布【药物名称】中文通用名称:塞来昔布英文通用名称:Celecoxib其他名称:塞来考昔、西乐葆、Celebrex。

【临床应用】1.用于缓解骨性关节炎的症状和体征。

2.用于缓解成人类风湿关节炎的症状和体征。

3.用于幼年型类风湿关节炎(国外资料)。

4.作为常规疗法(内镜监测、手术等)的辅助治疗,可减少家族性腺瘤息肉(FAP)患者腺瘤性结直肠息肉量。

5.用于强直性脊柱炎、原发性痛经、急性疼痛。

【药理】1.药效学本药是昔布类非甾体类抗炎药(NSAIDs),能特异性抑制环氧酶-2(COX-2)。

炎性因子可诱导COX-2生成,导致炎性前列腺素类物质的合成和聚集,尤其是前列腺素E2,从而引起局部炎症、水肿和疼痛。

而本药可通过抑制COX-2阻止炎性前列腺素类物质的产生,达到抗炎、镇痛及解热作用。

体外及体内试验表明,本药与基础表达的环氧酶-1(COX-1)的亲和力极弱,治疗剂量下不影响由COX-1激活的前列腺素类物质的合成,因此不干扰组织中与COX-1相关的正常生理过程,尤其是在胃、肠和肾等组织中。

2.药动学本药口服吸收迅速而完全,相对生物利用度约为99%(相对于口服混悬液),口服后约3小时达血药峰浓度。

食物可延缓其吸收。

药物吸收后广泛分布于全身各组织。

稳态分布容积约为400L,血浆蛋白结合率约为97%。

本药主要在肝脏经细胞色素P450 2C9(CYP 2C9)代谢,已证实血浆中有3种代谢产物(醇、相应的羧基酸和其葡糖苷酸结合物),代谢产物对COX-1、COX-2无抑制活性。

服用单剂放射性核素标记的药物后,57%从粪便排出,27%随尿液排泄,粪便及尿液中的绝大多数代谢产物为羧基酸(给药剂量的73%)。

少量葡糖苷酸从尿液排泄,仅有少于3%给药量的药物以原形从粪便及尿液中排泄。

多次给药无蓄积作用。

血液透析不能有效清除药物。

【注意事项】1.禁忌症 (1)对本药过敏者。

(2)对阿司匹林或其他NSAIDs过敏者(可诱发哮喘、荨麻疹或过敏反应,有严重甚至致死的过敏反应报道)。

规格:(1)0.1g (2)0.2g用法用量:在决定使用本品前,应仔细考虑本品和其他治疗选择的潜在利益和风险。

根据每例患者的治疗目标,在最短治疗时间内使用最低有效剂量(见【注意事项】-警告)。

骨关节炎和类风湿关节炎,根据个体情况决定本品治疗的最低剂量。

进食的时间对此使用剂量没有影响。

骨关节炎:本品缓解骨关节炎的症状和体征推荐剂量为200mg,每日一次口服或1OOmg每日两次口服。

类风湿关节炎:本品缓解类风湿关节炎的症状和体征推荐剂量为1OOmg至200mg,每日两次。

急性疼痛:推荐剂量为第1天首剂400mg,必要时,可再服200mg;随后根据需要,每日两次,每次200mg。

家族性腺瘤息肉(FAP):FAP患者在接受本品治疗时,应继续其常规的治疗。

用于FAP患者减少腺瘤性结直肠息肉数目治疗时,推荐剂量为口服400mg (200mg胶囊两粒)每日两次,与食物同服。

特殊人群肝功能受损患者:中度肝功能损害患者(Child-Pugh II级)本品的每日推荐剂量应减少大约50%。

不建议严重肝功能受损患者使用本品(见[药代动力学]-特殊人群)。

西乐葆(塞来昔布胶囊)

劲通(双氯芬酸钠气雾剂)【药品名称】商品名称:劲通通用名称:双氯芬酸钠气雾剂英文名称:Diclofenac Sodium Aerosol【成份】每克含双氯芬酸钠12.5毫克。

辅料为:月桂氮卓酮、乙醇、丙二醇、抛射剂。

【适应症】用于缓解肌肉、软组织和关节的轻至中度疼痛。

如缓解肌肉、软组织的扭伤、拉伤、挫伤、劳损、腰背部损伤引起的疼痛以及关节疼痛等。

也可用于骨关节炎的对症治疗。

【用法用量】外用,将药瓶直立喷于患部。

一次喷药时间不超过2秒钟(喷药约2克),一日3次,一日用药总量不超过12克。

【不良反应】1.偶可出现局部不良反应:过敏性或非过敏性皮炎如丘疹、皮肤发红、水肿、瘙痒、小水泡、大水泡或鳞屑等。

2.局部使用本品而导致全身不良反应的情况少见,若将其用于较大范围皮肤长期使用,则可能出现:一般性皮疹、过敏性反应(如哮喘发作、血管神经性水肿、光敏反应等)。

如发生这种情况,应咨询医师。

【禁忌】1 对其他非甾体抗炎药过敏者禁用。

2 对丙二醇过敏者禁用。

【注意事项】1.由于本品局部应用也可全身吸收,故应避免长期大面积使用。

2.肝、肾功能不全者以及孕妇、哺乳期妇女使用前请咨询医师或药师。

3.不得用于破损皮肤或感染性创口。

根据每例患者的治疗目标,在最短治疗时间内使用最低有效剂量(见【注意事项】-警告)。

1)用于缓解骨关节炎的症状和体征。

2)用于缓解成人类风湿关节炎的症状和体征。

3)用于治疗成人急性疼痛。

(见【临床试验】)4)作为常规疗法(如:内镜监测,手术)的一项辅助治疗,可减少家族性腺瘤息肉(FAP)患者的腺瘤性结直肠息肉的数目。

目前尚不清楚FAP患者腺瘤性结直肠息肉数目减少的临床益处。

停用本品后其治疗效果是否能维持还不明确。

尚无本品治疗FAP患者超过6个月的疗效和安全性的研究(见【临床试验】、【注意事项】-警告和注意事项)。

根据每例患者的治疗目标,在最短治疗时间内使用最低有效剂量(见【注意事项】-警告)。

骨关节炎和类风湿关节炎,根据个体情况决定本品治疗的最低剂量。

进食的时间对此使用剂量没有影响。

骨关节炎:本品缓解骨关节炎的症状和体征推荐剂量为200mg,每日一次口服或1OOmg每日两次口服。

类风湿关节炎:本品缓解类风湿关节炎的症状和体征推荐剂量为1OOmg至200mg,每日两次。

急性疼痛:推荐剂量为第1天首剂400mg,必要时,可再服200mg;随后根据需要,每日两次,每次200mg。

家族性腺瘤息肉(FAP): FAP患者在接受本品治疗时,应继续其常规的治疗。

用于FAP患者减少腺瘤性结直肠息肉数目治疗时,推荐剂量为口服400mg (200mg胶囊两粒)每日两次,与食物同服。

特殊人群肝功能受损患者:中度肝功能损害患者(Child-Pugh II级)本品的每日推荐剂量应减少大约50%。

不建议严重肝功能受损患者使用本品(见[药代动力学]-特殊人群)。

.塞来昔布【药物名称】中文通用名称:塞来昔布Celecoxib英文通用名称:其他名称:塞来考昔、西乐葆、Celebrex。

【临床应用】1.用于缓解骨性关节炎的症状和体征。

2.用于缓解成人类风湿关节炎的症状和体征。

3.用于幼年型类风湿关节炎(国外资料)。

4.作为常规疗法(内镜监测、手术等)的辅助治疗,可减少家族性腺瘤息肉(FAP)患者腺瘤性结直肠息肉量。

5.用于强直性脊柱炎、原发性痛经、急性疼痛。

【药理】1.药效学本药是昔布类非甾体类抗炎药(NSAIDs),能特异性抑制环氧酶-2(COX-2)。

炎性因子可诱导COX-2生成,导致炎性前列腺素类物质的合成和聚集,尤其是前列腺素E,从而引起局部炎症、2水肿和疼痛。

而本药可通过抑制COX-2阻止炎性前列腺素类物质的产生,达到抗炎、镇痛及解热作用。

1 / 48.体外及体内试验表明,本药与基础表达的环氧酶-1(COX-1)的亲和力极弱,治疗剂量下不影响由COX-1激活的前列腺素类物质的合成,因此不干扰组织中与COX-1相关的正常生理过程,尤其是在胃、肠和肾等组织中。

2.药动学本药口服吸收迅速而完全,相对生物利用度约为99%(相对于口服混悬液),口服后约3小时达血药峰浓度。

食物可延缓其吸收。

药物吸收后广泛分布于全身各组织。

稳态分布容积约为400L,血浆蛋白结合率约为97%。

本药主要在肝脏经细胞色素P 2C9(CYP 2C9)代谢,已证实血浆中有3种代谢产物(醇、相应的450羧基酸和其葡糖苷酸结合物),代谢产物对COX-1、COX-2无抑制活性。

服用单剂放射性核素标记的药物后,57%从粪便排出,27%随尿液排泄,粪便及尿液中的绝大多数代谢产物为羧基酸(给药剂量的73%)。

少量葡糖苷酸从尿液排泄,仅有少于3%给药量的药物以原形从粪便及尿液中排泄。

多次给药无蓄积作用。

血液透析不能有效清除药物。

【注意事项】1.禁忌症(1)对本药过敏者。

(2)对阿司匹林或其他NSAIDs过敏者(可诱发哮喘、荨麻疹或过敏反应,有严重甚至致死的过敏反应报道)。

(3)对磺胺类药过敏者。

(4)冠状动脉搭桥手术(CABG)围手术期疼痛患者。

(5)重度肝功能不全者。

(6)进行性肾脏疾病患者。

2 / 48.2.慎用(1)有支气管哮喘、过敏性鼻炎、荨麻疹史者。

(2)肾功能不全者。

(3)高血压或心脏疾病患者(可能因体液潴留或水肿而加重)(国外资料)。

(4)有胃肠道溃疡、出血、穿孔史者(国外资料)。

(5)有肝功能不全史者(国外资料)。

3.药物对儿童的影响国内尚无18岁以下患者用药的安全性及有效性资料。

4.药物对老人的影响老年患者用药的安全性及有效性与年轻患者相比尚未见明显差异。

但有报道老年患者出现致命的胃肠道不良反应与急性肾衰竭多于年轻患者。

5.药物对妊娠的影响尚无孕妇用药的研究。

妊娠早中期用药应权衡利弊。

妊娠晚期用药可致胎儿动脉导管提前闭合,应避免使用。

美国食品药品管理局(FDA)对本药的妊娠安全性分级为C级。

6.药物对哺乳的影响本药是否分泌入乳汁尚不明确,哺乳妇女用药应权衡利弊。

7.用药前后及用药时应当检查或监测(1)长期使用NSAIDs者,应定期进行全血细胞计数及血生化检查。

(2)用药期间应密切监测血压、肝功能。

(3)长期用药者出现贫血或失血的症状及体征应检查血红蛋白、血细胞比容。

【不良反应】1.心血管系统可使严重心血管血栓事件、心肌梗死、脑卒中的发生风险增加,甚至可能致死,此风险可能随用药时间的延长而增加,3 /48.尤其是心血管疾病或存在心血管疾病危险因素者风险更大。

还可见高血压或加重高血压、体液潴留、水肿、充血性心力衰竭。

2.胃肠道NSAIDs可使严重胃肠道不良反应(胃或肠道出血、溃疡、穿孔)发生的风险增加(但本药所致的危险较其他NSAIDs低),且可在用药期间的任何时间内发生,无警示症状,老年患者更易发生,长期用药还可增加以上不良反应的风险。

还可见腹痛、腹泻、消化不良、胃肠胀气、恶心等。

3.中枢神经系统可引起头痛、头晕、失眠等。

4.呼吸系统可引起上呼吸道感染(8.1%)、咽炎(2.3%)、鼻炎(2.0%)、鼻窦炎(5.0%)等。

支气管炎、支气管痉挛、支气管痉挛恶化、咳嗽等发生率小于2%。

5.泌尿生殖系统(1)可出现肾功能不全,见于老年患者、原有心肝肾疾病和同时服用多种药物的患者。

(2)长期使用NSAIDs可致肾乳头坏死,停药后通常可恢复至治疗前水平。

(3)可见血尿素氮(BUN)升高。

6.代谢/内分泌系统可见下肢浮肿(2.9%)、低磷血症。

7.肝脏有罕见的严重肝脏反应的报道,包括黄疸性及致命性急性重型肝炎、肝坏死、肝功能衰竭。

可见丙氨酸氨基转移酶(ALT)及天门冬氨酸氨基转移酶(AST)升高。

8.血液可见贫血。

4 / 48.9.过敏反应可见磺胺过敏反应,常见皮疹、瘙痒、荨麻疹等。

还可能出现致命的严重不良反应,如剥脱性皮炎、史-约综合征、中毒性表皮坏死溶解,可在没有征兆的情况下和有磺胺过敏史者中出现。

10.眼可见视力障碍。

11.其他可见背痛、皮疹、流感样症状、尿道感染、牙病等。

本药不抑制血小板聚集也不延长出血时间。

[国外不良反应参考]1.血液系统有研究表明,用药期间严重血栓栓塞性疾病(包括心肌梗死、肺栓塞、深静脉血栓形成、不稳定型心绞痛、短暂性脑缺血发作、缺血性脑血管意外)的发生率为1.2%;也有导致血栓形成的报道。

在推荐剂量下,本药可能不会影响血小板计数、凝血酶原时间、部分凝血活酶时间或血小板聚集。

2.心血管系统有2.1%的患者可出现体液潴留与外周水肿。

也有心脏异常的患者用药后出现尖端扭转型室性心动过速的个案报道。

3.中枢神经系统可出现头痛、头晕。

有谵妄、精神紊乱、定向力障碍、幻听、幻视的个案报道。

4.泌尿生殖系统(1)对依靠肾前列腺素维持肾血流的患者,NSAIDs 可能引起急性肾衰竭。

有引起急性肾衰竭或慢性肾衰竭加重的报道。

充血性心力衰竭者、肝肾功能不全者、使用利尿药或其他肾毒性药物者、老年患者是发生肾衰竭的高危人群。

停用NSAIDs后,肾功能通常会恢复到治疗前水平。

肾乳头坏死及其他肾损伤也与NSAIDs有关。

(2)本药导致钠钾潴留与萘普生相似。

首次给药后,本5 / 48.药可一过性降低肾小球滤过率与有效肾血流,萘普生对两者均无影响。

(3)有引起间质性肾炎的个案报道。

5.胃肠道(1)可出现消化不良(8.8%)、腹泻(5.6%)、腹痛(4.1%)、胀气(2.2%)。

消化不良与腹痛是导致停药的最常见原因。

(2)与传统NSAIDs(如双氯芬酸、布洛芬或萘普生)相比,本药导致严重胃肠道不良反应(如出血、穿孔)的发生率显著降低。

(3)有炎症性肠道疾病加重的报道。

(4)有80岁以上患者用药后48小时内出现急性胰腺炎症状的个案报道。

6.呼吸系统可出现上呼吸道感染(8.1%)、副鼻窦炎(5%)、咽炎(2.3%)、鼻炎(2%)等。

7.眼有治疗两个月出现橙色幻视(双侧视野内出现橙色点)的个案报道,停药后3日症状消失。

8.皮肤(1)与耐受剂量的NSAIDs(双氯芬酸和布洛芬)相比,治疗类风湿关节炎和骨性关节炎,使用最大治疗剂量2-4倍的本药(一次400mg,一日2次)导致皮肤不良反应(皮疹、瘙痒、荨麻疹)的发生率更高。

(2)有Sweet综合征、迟发型皮肤过敏反应、假性皮肤卟啉病、史-约综合征、多形红斑的个案报道。

(3)另有用药后出现过敏性血管炎,皮肤活检为弥漫性毛细血管白细胞碎片性和血栓性血管炎,最终死于多器官功能衰竭和弥漫性皮肤坏死溶解的个案报道。

9.过敏反应可见血管神经性水肿、发热等。

【药物相互作用】6 / 48.·药物-药物相互作用1.与氟康唑(一次200mg,一日1次)同服,本药的血药浓度升高2倍,其作用机制为氟康唑可抑制本药经CYP 2C9代谢。

接受氟康唑治疗者应给予本药最低推荐量。

2.与锂剂合用时,可使血浆锂浓度升高,锂中毒(无力、震颤、烦渴、精神紊乱)的危险增加。

3.与阿司匹林合用时,可增加胃肠道出血的危险。

但使用本药时不能停用因防治心血管病所需服用的小剂量阿司匹林(一日80-150mg)。

4.与华法林或其同类药合用时,有增加出血的危险,需监测抗凝活性。

有联用后因凝血酶原时间延长而导致出血不良反应的报道。

5.与含铝和镁的抗酸药合用时,本药血药峰浓度降低37%,曲线下面积减少10%。

6.与血管紧张素转换酶抑制药、袢利尿药、噻嗪类利尿药合用时,可使以上药物的降压及利尿作用降低。

7.尚未发现本药与格列本脲、酮康唑、甲氨蝶呤、苯妥英钠、甲苯磺丁脲之间存在有临床意义的相互作用。

·药物-食物相互作用高脂饮食可使本药血药浓度达峰时间延迟1-2小时,但总吸收量可增加10%-20%。

本药剂量达一次200mg,一日2次时,服药时间不受进食时间的影响。

达一次400mg,一日2次时,应于进食时服药以增加吸收。

7 / 48.【给药说明】1.本药用于镇痛抗炎的对症治疗,无根治风湿病的疗效,.不能代替皮质激素或治疗皮质激素缺乏,患者如突然停用皮质激素可导致其控制的疾病的恶化。

长期使用皮质激素治疗者停药需逐渐减量。

2.如非感染性疼痛与感染同时存在,本药的抗炎及解热作用可能减弱对感染的诊断依据。

3.使用本药前,应权衡使用本药与其他治疗选择的利弊。

根据患者个体的治疗目标,在最短治疗时间内使用最低有效剂量。

4.如出现皮疹或过敏反应、肝功能或肾功能异常持续存在或加重应停药。

5.药物过量的症状:急性NSAID过量症状通常限于疲倦、嗜睡、恶心、呕吐、上腹痛,经对症及支持治疗后可缓解。

也可见胃肠道出血、过敏反应。

罕见高血压、急性肾衰竭、呼吸抑制、昏迷。

6.药物过量的处理:本药尚无特性解毒药,过量后应给予对症及支持治疗,包括洗胃(服药4小时内者)、催吐、导泻、口服活性炭(成人顿服60-100g,儿童剂量为1-2g/kg)、输液。

对严重过敏反应者宜给肾上腺皮质激素及支持治疗。

有胃肠出血等急腹症者按急腹症处理,不宜给予洗胃、催吐、导泻等。

本药采用利尿、碱化尿液、血液透析或血液超滤可能无效。

【用法与用量】成人8 / 48.·常规剂量·口服给药1.骨性关节炎:推荐量为一日200mg,一日1次,或一次100mg,一日2次。

2.类风湿关节炎:推荐量为一次100-200mg,一日2次。

3.FAP:FAP患者接受本药治疗时,应继续其常规治疗。

推荐量为一次400mg,一日2次,进餐时服用。

4.急性疼痛:推荐剂量第1日首剂400mg,必要时可再服200mg,随后根据需要,一次200mg,一日2次。

·肾功能不全时剂量轻度肾功能不全者无需调整剂量,中度肾功能不全者应减量,重度肾功能不全者尚无临床使用经验。

·肝功能不全时剂量轻度肝功能不全者无需调整剂量,中度肝功能不全者(Child-Pugh分类标准Ⅱ级)一日推荐量应减少50%。
