01 Is there a PPE hazard assessment record to determine if hazards that

require the use of personal protective equipment (for example, head,

eye, face, hand, or foot protection) are present or are likely to be present

for each work area, task, or group of tasks?存在危险或可能存在危险因

Criteria: A hazard assessment to determine personal protective

equipment requirements for a specific job, task, or area is required to be

documented and certified. This assessment should be available for

review, and should be used by the inspection team to assist in

02 Are employees in work areas notified and reminded by signs or postings

Criteria: Signs or postings indicating required PPE must be displayed to

03 Are protective goggles or face shields provided and worn where there is

Criteria: Goggles and/or face shields must be worn if specified by the

PPE assessment due to potential chemical splash or exposure, and/or

required to be worn at all times in areas where there is a risk of eye

injuries such as punctures, abrasions, contusions or burns?在眼部伤害

Criteria: Safety glasses with non-conductive frames and side shields

Criteria: Aprons, sleeves, gloves, or other specific PPE must be worn if

chemicals, materials, or physical conditions such as extreme heat or cold.

06 Are hard hats provided and worn where danger of falling objects exists?高

Criteria: Head protection PPE must be worn if specified by the PPE

assessment or otherwise specified due to potential impact from falling

objects or overhead obstructions or equipment.由个人防护评估或其他特
