产品认证申请书（模版）
产品认证申请书（模版）第一篇：产品认证申请书（模版）产品认证申请书申请产品认证的条件1.申请组织应持有工商行政管理部门颁发的法人营业执照或注册文件。
2.申请认证的产品应符合国家有关标准或行业标准的规定，医疗器械产品应取得产品注册，产品已定型成批生产。
3.申请组织应该按ＣＭＤ的《产品认证实施规则》建立质量管理体系，当同时在CMD申请质量管理体系认证时，按拟申请认证的标准建立质量管理体系，并正式运行。
生产三类医疗器械产品的企业，质量体系运行时间不得少于6个月，其他产品的生产企业，体系运行时间不得少于3个月。
并至少进行一次内审。
4.申请认证的产品应正常批量生产，以保证生产现场审核的正常进行，并能提供充分的质量记录。
5.在认证申请前一年内，申请认证的产品应无重大质量事故。
申请组织应向CMD报送以下材料：1.申请组织授权代表签署的产品认证申请书；2.申请组织营业执照和医疗器械生产企业许可证（复印件）；3.申请组织的质量手册，必要时提供有关的程序文件；4.申请认证产品的注册产品标准、标准审批核标准历次修改的审批文件；5.医疗器械产品注册证（复印件）；6.产品生产流程、特殊过程和关键过程说明；7.产品主要外购件和外协件清单；8.近两年产品销售情况及用户反馈信息； 9.使用说明书；10.如同时申请质量管理体系认证，相同材料可只提交一份。
申请、获证组织的权利：1.申请组织有权询问认证机构的资格、能力和有关认证的背景和索取有关材料；2.申请组织有权对CMD派出的审核组、审核计划提出意见；3.申请组织有权对由于CMD过失造成的失密事件追究法律责任； 4.申请组织有权对审核员工工作能力和服务态度提出意见；5.经评定批准获证的组织可获得CNAB认可的CMD产品认证证书，并在CMD注册和公告；6.获证组织有权早认证证书有效期内，按规定在认证产品上使用CMD标志及在广告、宣传资料中介绍所或证书及CMD标志。
申请、获证组织的义务：1.申请组织需提交产品认证申请书、如实填写组织基本情况并提供营业执照、相关资质证明及本申请书要求提交的资料、2.获证组织在持证期间每年最少接受一次认证后的产品监督检验和现场监督审查。
