六、违约责任1. 若甲方违反乙方的规定或损坏场馆设施，应承担相应的赔偿责任。
2. 若乙方未按照约定提供服务，应按照会员费用的__%向甲方支付违约金。
七、协议变更与解除1. 本协议的任何变更或补充需经双方书面协商一致，并签署相关协议。

第三章会员卡3.1 甲方发给乙方会员卡，作为乙方享受会员服务的凭证。
3.2 乙方应妥善保管会员卡，不得出租、出借、转让或买卖。
3.3 会员卡如有遗失或损坏，乙方应及时向甲方报告，甲方有权收取工本费后予以补办。
第四章会员服务4.1 甲方为乙方提供以下会员服务：4.1.1 健身设施使用权；4.1.2 健身课程参加权；4.1.3 健身教练指导权；4.1.4 甲方提供的其他增值服务。
4.2 乙方应按照甲方规定的时间、地点使用会员服务，遵守甲方制定的健身俱乐部规章制度。
4.3 乙方在享受会员服务时，应遵守以下规定：4.3.1 不得在健身区域内吸烟、饮酒；4.3.2 不得在健身区域内大声喧哗，影响他人健身；4.3.3 不得损坏健身设备，如有损坏，应承担相应赔偿责任；4.3.4 不得在健身区域内进行非法活动。
第五章费用与支付5.1 乙方应按照本协议约定的费用标准支付会员服务费用。
5.2 乙方可以选择以下支付方式：5.2.1 现金支付；5.2.2 银行转账；5.2.3 信用卡支付。
5.3 乙方在支付会员服务费用后，甲方应及时为乙方开具正规发票。
5.4 乙方如需退会，应在协议期限届满前30日向甲方提出书面申请，甲方在扣除乙方已享受服务部分的费用后，退还剩余费用。

2、因器械、设备(设施)检修、维护，俱乐部有权在某一时间段对某一项目或某类项目采取停用或限用措施。
三、训练区域：1、为了大家的卫生环境，请勿在健身房内携带食品。
2、进行力量锻炼前、后，须轻拿轻放，会员使用完器械后须放回原位。
3、自由力量区域内的大重量练习必须在有保护的情况下进行，且须使用器械上的安全卡子。
