汉语名称缩写名 词 解 释 和 临 床 意 义红细胞RBC<1>尿沉渣RBC参考范围（医院设定）男性0--12/ul 女性0--24ul均一型RBC isomorphic RBC<2>RBC数量增高，主要见于泌尿系结石，结核，肿瘤，肾炎，血管畸形，出血性疾病等，亦可见于月经污染不均一型(异型)RBC dysmorphic RBC<3>利用RBC形态鉴别血尿来源（肾性和非肾性血尿）： 70%RBC-Fsc<=80ch,称为异型RBC（肾性）；混合型RBC mixed RBC 70%RBC-Fsc>=100ch称为均一型RBC（非肾性）；介于两者之间称为混合性血尿。
白细胞WBC<1>尿沉渣WBC参考范围（医院设定）男性0--12/ul 女性0--26ul<2>WBC数量增高，多见于泌尿系感染，结石，结核和肿瘤，亦可见于肾移植早期排异。
<3>同时检测WBC和BACT是判断泌尿系感染和区分污染的重要指标，结合WBC形态所在的图型区域，亦可判断泌尿系感染的类型：a.当WBC>60/ul并呈高WBC-Fsc/低WBC-FI图型时，可考虑急性泌尿系感染（活白细胞）b.当WBC>10/ul并呈低WBC-Fsc/高WBC-FI图型时，可考虑慢性泌尿系感染（死白细胞）上皮细胞EC正常尿液中可见少量鳞状和移行EC，若数量增多亦可见SRC，提示泌尿道感染或肾实质损害，若EC数量>3/ul时，显示警告和复查信号。
小圆上皮细胞SRC管型CAST <1>正常尿可见极少量透明管性，当数量增多或有病理管型时，表明有肾实质损害，当管型数量>2.5/ul（医院设定），提示警告和复查信号。
病理性管型Path. CAST<2>当数量>0.5/ul时，显示警告和复查信号。
<3>对急慢性肾炎，肾病综合症有较特异的诊断意义。

尿液中的红血球/红细胞（RBC）通常比白细胞要小的多并缺少内部结构，红细 胞在外观上由于中心显著的灰白色而呈指环状。
注意: 生化 Blood 检测为阳性。
红细胞 (RBC)
尿液中发现的白血球/白细胞 (WBC) 多为炎症或传染病程中应答反应产生的嗜 中性粒细胞。通过其大小及内部结构即核叶结构可将其与红细胞区分开来。

2002年，美国IRIS公司推出了iQ200 全自动尿有形成分分析仪，在 Yellow IRIS基本原理基础上，采用流式细胞技术和自动粒子识别系统 的全自动尿有形成分分析仪。真正实现了两者的自动化，是镜检图像 式和流式细胞式两种理念的结合，引领尿沉渣检测进入标准化、规范 化和自动化的新时代。

尿沉渣自动分析仪
一、测定项目（参数和信息）：
⒈定量参数：RBC（红细胞），WBC（白细胞），EC（上皮细胞），CAST（管型），BACT（细菌）。
据文献报道：每高倍镜视野的报告方式报告结果，其CV值接近40%以上。
非定量结果是一个模糊的概念，特别当结果处于临界状态时，容易引起误诊识判。
定量报告方式能让临床及时了解药物疗效，进行疗效的动态观察和比较。
两者的换算关系：1/HPF=5.56ul，1/LPF=0.34ul。
⒉提示性参数：Path. CAST（病理管型），SRC（小圆上皮细胞）YLC（酵母样细胞），X’TAL（结晶），SPERM（精子）。
Path. CAST和SRC阳性提示肾脏疾病，应显微镜复查与尿蛋白综合分析；YLC 、X’TAL 和SPERM阳性提示可能有这些物质的存在。
⒊红细胞信息：（红细胞信息），Microcytic（小红细胞），Normocytic（正常红细胞），Non-classified（混合性红细胞）。
⒋其它分析参数：Total Count（颗粒总数：当Total Count＞4万：⑴明显的病理标本⑵污染的标本、药物的干扰），OTHERS（其它颗粒），Fsc2（细菌的Fsc均值），Conductivity（电导率）。
⒌红细胞分析参数：RBC-P70Fsc（红细胞直方图70%面积处红细胞大小），
RBC-Fsc-DWC（红细胞分布宽度）。
二、参数和信息在临床中的应用：
1．血尿来源的判断（文献报道敏感性100%，特异性92.54%）：Microcytic（小红细胞），提示血尿来源为肾性血尿，同时RBC-P70Fsc＜80ch，RBC-Fsc-DWC 均值15.7ch。
Normocytic（正常红细胞）RBC-P70Fsc＞100ch，RBC-Fsc-DWC均值15.7ch，Non-classified提示为混合性红细胞。
红细胞大小均一的RBC-Fsc-DWC＜50ch红细胞大小不均一的RBC-Fsc-DWC＞50ch。
2．尿道感染的判断：根据白细胞数目判断病情为急性或慢性。
一般情况下，急性：WBC ＞60/ ul慢性：WBC＞10/ ul，同时仪器对细菌结果的提示基于三个参数：阳性：BACT ＞8040/ul，Fsc2＞18ch，WBC＞10/ul。
低可信度：BACT在2573~8040 /ul之间，Fsc2＜18ch，WBC＞10/ul。
也有报导：以105/ul作为尿道感染的判定值，其敏感性95%，特异性90%。
3．电导率的应用：电导率即尿液的离子浓度，仪器设置范围：5~38mS/cm。
电导率和渗透压、比重有一定的相关性，在鉴别低渗透压尿、高渗透压尿，低比重尿、高比重尿待等相关性疾病对肾功能的变化，尿崩症的临床诊断有较大意义。
4．干扰的问题：⑴大量结晶、细菌引起RBC↑或↓。
⑵酵母菌引起RBC↑或↓，WBC↑或↓。
⑶脂肪小体引起RBC↑。
⑷柱状上皮细胞、肾小管上皮细胞、滴虫引起WBC↑。
⑸白细胞引起EC↑或SRC↑。
⑹非细菌微小粒子引起BACT↑。
⑺粘液丝引起CAST↑。
5．复检条件：计数错误Total Count＞4万；电导率错误Conductivity≤5mS/cm或≥38mS/cm；界标错误RBC-Fsc-DWC≥40ch；设定RBC、WBC、EC、CAST、BACT 超出Negative Limits限值，或称阳性报警，为数量超限；设定RBC 、WBC、EC、CAST、BACT定量参数的超出复检范围的异常结果数值，为必须镜检的Review Limits 样本限值。
6．与干化学的交叉复核：RBC与红细胞（潜血）；WBC与白细胞（酯酶）；BACT 与亚硝酸盐；CAST与蛋白质。
7．随着研究的进一步深入，可扩大到其它体液如脑脊液、浆膜腔液、羊水、支气管灌洗液等的检测，此有待于临床实验中进一步实践和研究。
