广安意民煤电有限公司蔡山洞煤矿复产复工工作方案根据广安市前锋区人民政府办公室关于印发广安市前锋区煤矿复产复工验收工作方案的通知（前府办发[2013]44号）、广安市前锋区煤矿企业兼并重组工作领导小组办公室《关于加强煤矿复产复工隐患整改及验收工作的通知》（广前煤重组办[2013]7号）文件及2013年10月16日前锋区煤矿复产复工工作会议精神。
为确保我矿复产复工安全有序开展，结合本矿实际情况，特制定本复产复工工作方案。
一、成立煤矿复产复工领导小组组长：王君东(矿长）副组长：何证（生产副矿长）、彭立祥（总工程师）、陈建华（安全副矿长）、龙承云（机电副矿长）成员：王会春、刘东林、周密、张有清、严肃、煤矿“五科”科长领导小组下设复产复工办公室，由彭立祥兼任办公室主任，负责复产复工具体工作。
验收办公室下设专业组：一通三防组：由彭立祥负责，成员有谢伟、王会春。
机电运输组：由龙承云负责，成员有秦泽华、周密、李平。
采掘组：由何证负责，成员有陈传学、王一平。
地测防治水组：由张有清负责，成员严肃、王敬全。
地面综合组：由王君东负责，成员有刘东林、谌贵元。
后勤保障组：由陈德树负责，成员有叶光明、唐润华。
二、工作开展程序1、通知人员返矿：由矿行政办公室在10月17日通知所有人员全部返矿。
（我矿已于10月11日通知人员返矿，现人员已基本到位）。
2、召开三会：由矿长组织开好“三会”（领导班子会、职工收心会、矿工培训会）重新学习煤矿岗位责任制和管理制度及各作业头面的作业规程。
认真分析井下可能存在的问题，并提出针对性检查、整改方案。
提前做好准备工作。
所有管理人员必须收心，把工作重点放在矿井安全管理上。
并进行为期72学时的上岗前全员安全培训，培训工作由安全副矿长陈建华负责，培训地点设在矿安全培训中心，培训人员为全矿井下作业人员。
培训工作由广安市安全培训中心负责。
培训内容为煤矿基本培训教材，煤矿安全知识，煤矿管理制度，《煤矿矿长保护矿工生产安全七条规定》、《煤矿安全规程》、《煤矿安全质量标准化标准》、岗位责任制及技术操作规程、并开展警示教育，培训时间每天上午8：00至12：00，下午13：00至17：00。
培训组织工作由各队队长负责，严格考勤。
培训结束后由安全培训中心统一组织考试。
培训合格，取得区煤管局颁发的《上岗证》、《实习证》后，方可入井。
考试不合格者不得上岗。
同新工人签定劳动合同。
此项工作在10月19日前完成。
3、开展地面系统检查。
在10月17日完成地面主要系统安全检查，主要包括供电系统、备用电源、地面变配电设备、供电线路、防雷电设施、主要通风机、风井主控风门和瓦斯监控系统、人员定位系统等进行全面检查并及时检修，保证供电系统、瓦斯监控系统、人员定位系统正常运行。
4、开展入井侦察。
煤矿停产时间较长，井下各种情况都可能发生较大变化，在正式入井排查隐患之前必须进行入井侦察。
入井侦察必须制定安全技术措施，报区煤矿复产复工验收小组批准后，由煤矿熟悉井下情况的管理人员带领广安市矿山救护大队救护队员进行。
入井侦察前，煤矿必须向救护队提供真实的矿图、介绍停产前后矿井有关情况，矿山救护队要在充分调查了解的基础上，会同煤矿制定入井侦察方案，经矿方和救护队共同签字确认后开展入井侦察。
侦察结束后，形成侦察报告报区煤矿复产复工验收办公室。
此项工作在10月20日前完成。
5、排放瓦斯。
根据侦察情况，请区煤管局协调市矿山救护队实施矿井瓦斯排查。
瓦斯排放工作结束后，矿山救护队对瓦斯排放情况形成书面报告报区煤矿复产复工验收办公室。
此项工作在10月22日前完成。
6、排查隐患和制定隐患整改方案。
瓦斯排放结束后，煤矿根据入井侦察情况，立即制定《煤矿隐患排查及处置工作方案》和《井下密闭管理台帐》，一并报区煤矿复产复工验收办公室提出隐患排查申请。
我矿完成隐患自查后，向区煤管局报送《煤矿隐患排查报告》，请区煤管局安排专家到矿进行隐患排查。
排查工作在10月25日前完成。
7、进行隐患整改。
煤矿根据批复的整改方案落实好整改资金、材料、设备、工具、人员等，组织全体参与隐患整改的人员学习隐患整改方案措施，并严格按整改方案和安全技术措施进行隐患整改。
整改工作计划在10月30日前完成。
8、复产复工验收申请。
煤矿按照批准的整改方案完成全部整改内容经企业自查合格后，向区煤矿复产复工验收领导小组提出复产复工验收和采掘头面核定申请。
9、恢复正常生产。
验收合格后，按区煤管局批复组织正常生产作业。
三、隐患排查主要内容1、排查矿井是否存在证照不全、不在有效期内。
2、排查是否存在有《国务院关于预防煤矿生产安全事故的特别规定》（国务院446号令)中明确规定的15种重大安全生产隐患。
3、是否建立了安全管理机构，是否按要求配备“五职”矿长，并持有矿长资格证和矿长安全资格证，是否配齐“五科”、“五队”、“五长”等管理人员并持有安全资格证。
4、排查矿井一级和二级负荷机电设备是否使用正常，安全装置是否齐全，或井下电气设备是否存在失爆现象。
5、排查煤矿机构是否健全，是否按要求配备专业技术人员，是否至少配备了2名全日制煤炭院校毕业、煤矿主导专业工程技术人员，矿长和特种作业人中是否存在有未经过培训或配备不全，或从业人员应经过全员培训而未达到上岗培训要求上岗作业的。
排查是否组建防治水机构并配备足够的人员和设备。
6、排查是否按规定执行收尺制度，排查收尺结果与煤矿产量是否符合。
7、排查是否严格执行交换图制度，煤矿是否按测绘规程绘制和报送《矿井地质和水文地质图》、《井上下对照图》、《采掘工程平面图》、《通风系统图》、《井下避灾路线图》等11种矿图。
8、排查煤矿井下头面和同时最大下井人数进行是否经过前锋区煤炭管理局核定。
9、排查煤矿井下是否有隐蔽、违法和非法生产区域，是否有超层越界现象。
10、排查煤矿是否实现统一安全生产管理，是否有多个业主各自安排、逐级承包、层层转包、分片包干现象。
11、排查矿区环境治理情况，是否符合《四川省小煤矿安全生产基本要求》。
12、排查规章制度情况。
是否有建立健全的1）、安全生产责任制；2）、安全办公会议制度；3）、安全目标管理制度；4）、安全投入保障制度；5）、安全质量标准化管理制度；6）、安全教育与培训制度；7）、事故隐患排查治理制度；8）、安全监督检查制度；9）、安全技术审批制度；10）、矿用设备、器材使用管理制度；11）、矿井主要灾害预防管理制度；12）、煤矿事故应急救援制度；13）、安全奖罚制度；14）、入井检身与出入井人员清点制度；15）、安全操作规程管理制度；16）、煤矿领导干部井下跟班、带班制度等制度的实施；17）、安全操作管理制度；18）、安全责任追究制度等18种制度，和各工种岗位责任制及操作规程；是否有批准的年度矿井灾害预防和处理计划，事故应急救援预案；是否建立有安全隐患台帐，并在井口设立了安全隐患标示牌，整改隐患是否进行了公布；排查是否严格执行入井检身和出入井人员清点、登记制度，并在井口挂牌明示。

四、复工复产隐患排查期间安全技术措施1.隐患排查必须在矿复产复工领导小组领导下按技术措施要求进行。
2.隐患排查工作由煤矿安全管理人员负责，其它辅助人员必须经过培训并取得合格证。
