When such patients fail to respond to empiric therapeutic measures, they often present problems in terms of precise diagnosis and rational management. paque markers (2, 3) or scintigraphy (4), anorectal m a n o m e t r y (3, 5), and evacuation proctography (6-10). Colonic transit studies in patients who complain of infrequent defecation have identified patients who consciously or unconsciously misinterpret bowel habits by documenting normal passage of radiopaque markers and have identified several patterns associated with prolonged colonic transit (2, 3). Anorectal manometry has demonstrated that constipated patients often exhibit diminished perception of rectal balloon distension (2, 5) and abnormal expulsion dynamics (11-13). H o w e v e r , the significance of anorectal abnormalities in such patients is not entirely clear. Evacuation proctography, which measures the evacuation of barium thickened with a variety of substances in order to approximate the consistency o f stool, has detected a high frequency of anorectal abnormalities in constipated patients, including rectal intussusception, enterocele, rectocele, inappropriate contraction of the puborectalis muscle during defecation, megarectum, and poor emptying of the contrast medium (7-9). H o w e v e r , since similar structural abnormalities have also been reported in normal volunteers (14), the relationship of such findings to patient complaints is uncertain. The aim of this study was to correlate evacuation proctography with anorectal manometric parameters and the clinical characteristics of patients with chronic constipation and/or defecatory difficulty. The primary goal was to determine if evacuation proctography is helpful in delineating the pathogenesis of complaints of constipation and defecatory difficulty and, more specifically, to correlate the presence of various abnormalities with impaired rectal emptying in such patients.

We prospectively evaluated 36 patients who complained o f chronic constipation and~or defecatory difficulties to determine the role of anorectal manometry and evacuation proctography in delineating the pathogenesis o f these complaints. Twenty patients with constipation also underwent a colonic transit study with radioopaque markers, which identified one group with normal transit (N = 10) and another with slow transit (N = 10). Nine o f 36 patients (25%) had inappropriate puborectalis muscle contraction or exhibited weak expulsion efforts during evacuation proctography, and these correlated highly with poor rectal emptying o f barium paste (20 + 6% vs 61 +- 5% in patients with normal relaxation; P < 0.01). However, poor rectal emptying did not correlate with the presence o f high-grade intussusceptions, large rectoceles, anorectal angles at rest or with straining, rectal diameter, clinical features, or colonic transit. Moreover, abnormal expulsion patterns as seen with anorectal manometry correlated poorly with the presence o f inappropriate puborectalis contraction and decreased rectal emptying by proctography. Although anatomic abnormalities occurred frequently in patients with constipation and~or defecatory difficulties, they were also prevalent in asymptomatic controls. In view o f these findings, surgical intervention to correct anatomic abnormalities in patients with constipation and~or defecatory difficulties should be considered only with great caution.
