骨髓间充质干细胞对大鼠佐剂性关节炎防治的实验研究许键炜;申长清;赵芳芳;舒莉萍;何志旭【摘要】目的:探讨骨髓间充质干细胞(MSCs)对大鼠佐剂性关节炎(AA)的治疗、干预作用。

方法收集培养扩增SD大鼠骨髓MSCs。

SD大鼠注射弗氏完全佐剂造模之后行MSCs治疗及干预。

造模后定期测量大鼠体质量、踝关节周长和后足爪体积。

治疗后第21天,行四肢关节X线片及病理检查。

结果模型大鼠经M SCs治疗后关节肿胀减轻,X线片示骨质破坏明显修复。

病理HE染色见关节滑膜病变减轻。

干预组经MSCs治疗,原发性炎症和继发病变均不明显,关节X线片和病理检查均未见明显异常,与模型组大鼠相比,体质量、足爪体积以及踝关节周长差异有统计学意义(P<0.05)。

结论 MSCs治疗可有效减轻并逆转弗氏完全佐剂诱导的大鼠AA的进展。

Mesenchymal stem cells have the above advantages in the treatment of osteoarthritis without serious complications.This paper reviews the clinical research progress of several mesenchymal stem cells in the treatment of osteoarthritis.[Key words] Mesenchymal stem cells; Osteoarthritis; Articular cartilage; Clinical trials骨关节炎(osteoarthritis, OA )临床表现为疼痛、活动 度受限、活动时捻发音、关节不稳及肿胀等叫预计到2032 年,45岁以上人群中30%患有OA 叭OA 可影响患者的肢体功能及生活质量,在发达国家,用于OA 的治疗费用可达 国内生产总值的2.5% [3]o人工关节假体置换术是治疗晚期OA 的金标准,但假体 使用寿命有限且术后会产生诸多并发症[41o 故应注重OA 的 早中期治疗,理想的治疗方案是改善患者的临床症状并促进软骨再生。

骨髓间充质干细胞在重度烧伤大鼠心肌损伤中的保护作用牟欢;刘洋;李俊杰;刘剑;周新立;强廷会;杜兴国;马平【摘要】目的探讨骨髓间充质干细胞(BMSCs)对严重烫伤大鼠心肌的保护作用及作用机制.方法①取10只健康雄性SD大鼠,采用全骨髓贴壁法体外分离大鼠BMSCs,流式细胞仪检测细胞表面分子标志物.②24只健康雄性SD大鼠,随机分为:对照组、模型组、治疗组.模型组及治疗组大鼠背部暴露于95℃热水浴18s造成30%TBSAⅢ度烫伤,对照组暴露于37℃水浴18s.水浴后即刻模型组及治疗组大鼠分别腹腔注射生理盐水10 mL(50 mL/kg),水浴后3h模型组经尾静脉注射100 μL 生理盐水,治疗组经尾静脉注射100 μL(2.5× 107/mL) BM-SCs.处理后48 h收集3组大鼠心脏组织标本及腹主动脉血,ELISA法检测血清肌酸激酶(CK)、乳酸脱氢酶(LDH)含量,实时荧光定量聚合酶链式反应(RT-PCR)法检测心脏组织中炎性细胞因子TNF-α、IL-1β、IL-10及凋亡相关分子caspase-3、Bcl-2、Bax的mRNA表达水平.结果流式细胞术检测提示,所培养的BMSCs表达CD44、CD90、CD105的阳性率分别为96.8%、99.72%、95.93%,而CD34、CD45阳性率分别为1.42%、2.17%;模型组大鼠血清CK、LDH含量显著高于对照组(P<0.05).治疗组CK、LDH含量显著低于模型组(P<0.05).模型组大鼠心脏组织中caspase-3、Bax 的mRNA表达量显著高于对照组(P<0.05).治疗组大鼠心脏组织中caspase-3、Bax的mRNA表达量显著低于模型组(P<0.05).模型组大鼠心脏组织中Bcl-2的mRNA表达量为显著低于对照组(P<0.05);治疗组大鼠心脏组织中Bcl-2的mRNA表达量显著高于对照组(P<0.05).模型组大鼠心脏组织中TNF-α、IL-1β的mRNA表达量显著高于对照组(P<0.05).治疗组大鼠心脏组织中IL-1β、TNF-α的mRNA表达量显著低于模型组(P< 0.05).模型组大鼠心脏组织中IL-10的mRNA 表达量显著低于对照组(P<0.05);治疗组大鼠心脏组织中IL-10的mRNA表达量显著高于模型组(P<0.05).结论 BMSCs可显著降低烧伤后大鼠CK、LDH水平,减轻心肌组织损伤,减少心肌细胞凋亡,降低炎性因子IL-1β和TNF-α表达、促进抑炎因子IL-10的表达,对严重烧伤大鼠心肌具有保护作用.%Objective To explorethe effects and related mechanism of bone marrow stromal stem cells (BMSCs) on protecting rats myocardial injury induced by severeburns.Methods (①)Ten healthy male SD (Sprague-Dawley)rats were usedto separate BMSCs through whole bone marrow adherent.The biological markers were assayed with flowcytometry.②)Twenty-four healthy male SD rats were randomly divided into control group,model group and treatment group.Except for control group,abdominal skin of rats were exposed to 95℃ water for 18 seconds which led to the third degree,30% TBSA burns while the rats in control group were exposed to 37℃ water for 18 seconds.Ratsin burn model group and treatment group were given 10 mL(50 mL/kg) saline intraperitoneally immediately after burns.Three hours later,Saline (100 μL)or BMSCs (100 μL,2.5×107/mL) were given through caudalvein.Forty-eight later,rats were sacrificed and the myocardium and blood from aorta abdominalis were collected.The expression levelsof ereatine kinase (CK),lactic dehydrogenase (LDH) were detected by ELISA.The mRNA level of caspase-3,Bcl-2,Bax,TNF-α,IL-1βand IL-10 were detected by RT-PCR.Results The positive rates of CD44,CD90,CD105 were 96.8%,99.72%and 95.93% separately in BMSCs.However,the positive rates of CD34 and CD45 were 1.42% and 2.17% separately.The expression level of CK and LDH in rats in model group were significantly higher than that in control group (P < 0.05).The expression level of CK and LDH in treatment groupwere higher than those in control group (P < 0.05).The mRNA level of caspase-3 and Bax in the myocardium in treatment group were significantly decreased compared with control group (P < 0.05).The mRNA level of Bcl-2 in myocardium of treatment group was higher than that in control group (P < 0.05).The mRNA levels of TNF-α and IL-1β in model group were higher than that in control group (P < 0.05).However,in treatment group,these two were lower than in the model group (P <0.05).The mRNA level of IL-10 was significantly lower than that in control group (P < 0.05).While in treatment group,the mRNA level of IL-10 was significantly higher than that in model group (P < 0.05).Conclusion BMSCs could protect against myocardial injury induced by severe burns.The expression of CK and LDH decreased,indicating that the injury of myocardial decreased.During which,the inflammatory factors,IL-1β and TNF-α decreased and anti-inflammatory factor IL-10 increased.【期刊名称】《中国医药导报》【年(卷),期】2017(014)012【总页数】5页(P12-16)【关键词】烧伤;心肌损伤;细胞凋亡;骨髓间充质干细胞【作者】牟欢;刘洋;李俊杰;刘剑;周新立;强廷会;杜兴国;马平【作者单位】陕西省汉中市中心医院骨关节外科一病区,陕西汉中723000;第四军医大学附属西京医院全军烧伤中心,陕西西安710032;第四军医大学附属西京医院急诊科,陕西西安710032;第四军医大学附属西京医院神经外科,陕西西安710032;陕西省汉中市中心医院骨关节外科一病区,陕西汉中723000;陕西省汉中市中心医院骨关节外科一病区,陕西汉中723000;陕西省汉中市中心医院骨关节外科一病区,陕西汉中723000;陕西省榆林市中医医院烧伤整形外科,陕西榆林719000【正文语种】中文【中图分类】R644严重烧伤患者早期即存在缺血缺氧性心肌损害,导致心功能减退[1-2] ,炎性反应、细胞凋亡等因素与严重烧伤早期心肌细胞损害密切相关。

·综述·间充质干细胞治疗骨关节炎的研究进展与展望贺曦吕红斌【摘要】骨关节炎是最常见的关节疾病之一,其病理呈现出关节软骨退变、软骨下骨重塑、关节间隙变窄及边缘骨赘增生的过程。

本文综述了近年来用于骨关节炎治疗的间充质干细胞(mesenchymalstem cells,MSCs)的来源及特性、动物实验及临床试验方面的进展,介绍了MSCs治疗骨关节炎及关节软骨缺损的优势,总结了目前MSCs治疗骨关节炎的现状及局限性,提示MSCs治疗骨关节炎是一项应用前景广阔的技术。

【关键词】骨关节炎;间充质干细胞;研究进展骨关节炎(osteoarthritis,OA)是最常见的关节疾病之一,其病程主要表现为关节软骨退变、软骨下骨重塑、关节间隙变窄以及边缘骨赘增生,并最终发展为关节功能障碍的过程[1]。

目前,OA是老年人中最常见的活动障碍原因之一,随着老年人口增多,OA的发病人数逐年上升。

然而,目前临床上关于OA的治疗却远远未取得令人满意的效果,对OA的干预主要是应用非甾体类抗炎药、激素等控制关节内炎症,减少疼痛,应用透明质酸[2⁃4],进行康复性训练等。

这些保守治疗方式并不能修复关节软骨。

关节软骨作为一种无血管组织,其自身修复潜能低下[5],关节镜下微骨折手术只适用于小到中型的软骨缺损,且其术后关节炎的控制并不理想,所修复的软骨也会在数年内退变[6]。

骨软骨移植虽然也能减轻疼痛,但远期效果并不理想[7,8]。

尽管自体骨软骨细胞移植或基质诱导骨软骨细胞移植可能获得较好的远期效果,但因其在培养过程中会加速细胞衰老和去分化,所以对于老年膝关节OA病人并不是常规建议的治疗方式[8,9]。

因此,鉴于供区损害、软骨细胞的去分化以及侵袭性操作等问题,对新的OA治疗方案的需求迫在眉睫。

干细胞治疗成为再生医学领域的热门。

从人体很多组织,包括骨髓、脂肪组织、脐带血、滑膜等都能分离出相当数量的间充质干细胞(mesenchymal stem cells,MSCs)[10],这成为软骨组织工程学中最有希望的祖细胞来源,是治疗OA的热点。

Sox9 is a transcription factor that is essential for chondrogenesis and has been termed as a“master regulator”of the chondrocyte phenotype. OBJECTIVE:To study the therapeutic effects of Sox9-transfected bone marrow mesenchymal stem cel s on knee osteoarthritis. METHODS:The bone marrow mesenchymal stem cel s were transfected with Lenti-Sox9-EGFP in vitro. The model of murine knee osteoarthritis was established by cutting off the anterior cruciate ligament. Thirty model mice were randomly divided into three groups, as normal saline group, bone mesenchymal stem cel group and Sox9-transfected bone mesenchymal stem cel group. 0.1 mL of normal saline, 0.1 mL of normal saline containing non-transfected bone marrow mesenchymal cel s (non-transfected group), or 0.1 mL of normal saline containing Sox9-transfected bone marrow mesenchymal cel s (Sox9-transfected group) was injected into the knee joint cavity of mice in the corresponding group, respectively. After 4, 8, 12 weeks, the repair of articular cartilage lesions was evaluated by toluidine blue and immunohistochemical staining. RESULTS AND CONCLUSION:The lesions of articular cartilage were more serious in the normal saline group, compared with the other two groups, and the difference became moreobvious over time. Damaged articular cartilage was improved in the non-transfected group, but the improvement was less than that in the Sox9-transfected group. Immunohistochemistry staining revealed that in theSox9-transfected group, the positive type II col agen expression was stronger than that in the other two groups, but this positive expression was decreased over time in al the three groups. These results suggest that Sox9-transfected bone marrow mesenchymal stem cel s promote the repair of damaged cartilage in mice with knee osteoarthritis.%背景:相关研究认为,骨髓间充质干细胞具有良好的增殖及分化能力,Sox9转录因子在软骨形成及维持软骨细胞表型方面起关键作用。

骨关节炎( osteoarthritis,OA) 患 者 关 节 软 骨 损 伤、变性是由慢性退行性病变导致,BM-MSCs 对其 治疗 效 果 及 作 用 机 制 的 研 究 报 道 不 多。 Centeno 等[4]曾报 道 1 例 采 用 自 体 BM-MSCs 治 疗 膝 关 节 OA 的病例,术后 24 周 MRI 显示患者膝关节软骨 较术前明显 改 善,关 节 活 动 度 以 及 疼 痛 评 分 也 得 到了较好的改善。koh 等[5]报道 18 例膝 关 节 OA 患者在实施关节镜清理后采用关节腔内注射自体 BM-MSCs进行治疗,随访 2 年左右的结果显示,患 者 WOMAC( The Western Ontario and McMaster Universities) 、Lysholm 及 VAS( visual analogue scale) 疼 痛 评 分 都 得 到 改 善 ,但 由 于 样 本 量 较 小 ,影 响 患 者 主观 感 受 的 因 素 较 多,上 述 研 究 所 得 结 果 偏 倚 较大。

骨髓间充质干细胞 ( bone marrow mesenchymal stem cells,BM-MSCs) 作为一种多向分化潜能的组织 工程种子细胞,已有多项研究证实其在组织修复中 能够起到很好的效果。Lee 等[1]应 用 关 节 腔 内 注 射 BM-MSCs 透明质酸悬液治疗小型猪股骨髁软骨 缺损,注射后 6 周及 12 周进行检测,与单纯应用透 明质酸及生 理 盐 水 对 照 组 相 比,实 验 组 软 骨 缺 损 部位组织愈 合 及 形 态 学 上 均 有 较 好 的 改 善,并 且 在缺损修 复 部 位 可 见 大 量 的 BM-MSCs,提 示 注 入 的 BM-MSCs 能 够 直 接 修 复 损 伤 软 骨。 而 Mangi 等[2]和 Tang 等[3]则认为,BM-MSCs 除了直接参与 损伤组织修 复 外,更 为 重 要 的 作 用 是 其 能 够 分 泌 多种因 子,抑 制 局 部 瘢 痕 组 织 形 成,减 少 细 胞 凋 亡 ,促 进 损 伤 部 位 血 管 再 生 ,从 而 达 到 组 织 修 复 的 目的。

[摘 要]目的: 研究不同浓度骨髓间充质干细胞( bone marrow mesenchymal stem cells,BM-MSCs) 对骨关节炎( osteoarthritis,OA) 的治疗效果,并探究其治疗机制。方法: 8 周龄近交系 SD 大鼠 32 只随机分成 4 组,每组 8 只,均采用 自身双侧对照研究: 对照组、高浓度组( 1 × 107 / mL BM-MSCs) 、低浓度组( 5 × 106 / mL BM-MSCs) 及高 / 低浓度对比 组。应用改良 Hulth 方法诱导膝关节 OA。对照组一侧行手术,另一侧行假手术,其余各组均行双侧手术。术后 4 周处死对照组大鼠采集双侧膝关节标本。各试验组大鼠膝关节内注入相应浓度的 BM-MSCs 或磷酸盐缓冲液 ( phosphate buffered solution,PBS) ,切断大鼠双侧坐骨神经及股神经使下肢制动,注射 3 周后处死各组大鼠并收取 双侧膝关节标本。应用 Mankin 组织学评分评价 OA 病变程度,RT-PCR 检测软骨Ⅱ型胶原 mRNA 表达,荧光显微 镜观察荧光蛋白标记的 BM-MSCs 在膝关节内分布情况。结果: 高浓度组、低浓度组 BM-MSCs 侧软骨组织标本 Mankin 评分均明显低于对照侧( 5. 40 ± 0. 51 vs. 9. 60 ± 0. 51; 6. 60 ± 0. 40 vs. 10. 00 ± 0. 32; P 均 < 0. 05) ,高 / 低浓度 对比组高浓度侧 Mankin 评分略低于低浓度侧( 6. 40 ± 0. 51 vs. 7. 60 ± 0. 75,P > 0. 05) 。RT-PCR 结果显示,高浓度 组、低浓度组 BM-MSCs 侧软骨Ⅱ型胶原 mRNA 含量分别为对照侧的 108% ± 1% 和 106% ± 1% ,高 / 低浓度对比组 高浓度侧Ⅱ型胶原 mRNA 含量是低浓度侧的 102% ± 1% 。荧光显微镜显示,在软骨表面未见绿色荧光蛋白表达, 而在滑膜组织内可见绿色荧光表达。结论: 关节腔内注入 BM-MSCs 可能通过间接机制对 OA 软骨病变起到保护作 用,两种浓度治疗效果无差异。 [关键词] 骨关节炎; 间充质干细胞; 注射,关节内 [中图分类号] R684. 3 [文献标志码] A [文章编号]1671-167X( 2015) 02-0211-08
ABSTRACT Objective: To investigate the efficacy of single time intra-articular different concentration of allogeneic bone marrow mesenchymal stem cells ( BM-MSCs ) injection in the treatment of SpragueDawley ( SD) rat model of osteoarthritis ( OA) . Methods: In the study,32 SD rats were equally randomized into 4 groups: control group,high concentration group ( 1 × 107 / mL BM-MSCs) ,low concentration group ( 5 × 106 / mL BM-MSCs) and high vs. low concentration group. The two knees of each rat were set up to a pair. The induction of OA was performed surgically randomly at one side in model group, and bilaterally in the other groups,which were through anterior cruciate ligament transaction ( ACLT) and medial meniscus excising. After the operation,the SD rats were allowed free movement. Four weeks later,different concentrations of allogeneic BM-MSCs isolated from the SD rats,expanded in vitro and suspended in phosphate buffered solution ( PBS) were delivered in the articular cavity of both knees; PBS was used as the control. After injection,we excised the femoral nerve and sciatic nerve to disuse the low limb. The cartilage histological sections of knees were scored by Mankin scoring system to assess the severity of the pathology. mRNA of collagen Ⅱ was detected by real time polymerase chain reaction ( RTPCR) . eGFP was detected by fluorescence microscope. Assessments were carried out 4 weeks after the operation in model group,and 3 weeks after injection in the other groups. Results: Mankin scores of the BM-MSCs side and control side were 6. 60 ± 0. 40 vs. 10. 00 ± 0. 32 in low concentration group ( P < 0. 05) ,and 5. 40 ± 0. 51 vs. 9. 60 ± 0. 51 in high concentration group ( P < 0. 05) . Mankin scores of high vs. low concentration group were 6. 40 ± 0. 51 vs. 7. 60 ± 0. 75 ( P > 0. 05) . mRNA expression of collagen Ⅱ of the BM-MSCs side in low concentration group was 106% ± 1% in contrast to the control side. As in high concentration group it was 108% ± 1% ,and 102% ± 1% in high vs. low concentration group. Labeled BM-MSCs were detected unexpectedly in the synovial membrane but not in cartilage tissue three weeks from injection. Conclusion: BM-MSCs could promote cartilage repair and inhibit OA
