Intro d uctionBaseline-ZERO™ DNase* is ideal for use when you need to be certain that ZERO DNA remains. Baseline-ZERO DNase hydrolyzes both double-stranded (ds) and single-stranded (ss) DNA to mononucleotides with the highest efficiency (Fig. 1). In the presence of Mg 2+, cleavage of each strand of a dsDNA substrate proceeds independently.1Baseline-ZERO DNase must be inactivated prior to the addition of Baseline-ZERO DNase-treated RNA to reverse transcription reactions. To inactivate the enzyme, incubate the completed reaction at 65°C for 10 minutes in the presence of 1X Stop Solution.2.Product Designations and Kit Components3. SummaryObjective.Thesurgicaltreatmentofbasilarbifurcationaneurysmsischallenging,andmanyoftheseaneurysmsarecurrentlytreatedbyendovascularmeans.However,thecompleteclosureoftheaneurysmbysurgicalclippingstillremainsthebestandmostpermanentcurefortheaneurysm.The‘‘goldstandard’’,subtemporalapproachwasestablishedandintroducedbyDrakeandithasbeenadaptedbytheseniorauthorHernesniemi.Wedescribeourpresentmodifiedtech-niqueofthisapproachbasedonclinicalexperience.Methods.ThesubtemporalapproachtobasilarbifurcationaneurysmshasbeenregularlyusedbytheseniorauthorHernesniemiinrecent15yearsinover200operationsinKuopioandHelsinki,Finland.Results.Thisapproachissuitableinmostbasilarbifurcationaneurysmsexceptforthosehighabovetheposteriorclinoidprocess.Toavoidtemporallobedamage,cerebrospinalfluiddrainageisnecessary.Benefitsofsubtemporalapproachareshortoperativeandretractiontimes,andnoneedforskullbaseresection.Conclusion.Thesubtemporalapproachissimpleandsafeinexpe-riencedhands,andshouldbeconsideredthestandardmethodtoapproachmostbasilarbifurcationaneurysms.

IntroductionThesurgicaltreatmentofbasilarbifurcationaneur-ysms(BBAs)remainsoneofthemostdi‰culttasksinneurosurgerybecauseoftheanatomicalandtechnicaldi‰culties.Recently,manyoftheserareaneurysmslocatedinanarrowspaceinfrontofthebrainstemhavebeentreatedbyendovascularproceduresinmanyinstitutions.However,closureofthebaseoftheaneurysmbyclippingstillremainsthemoste¤ectivecurefortheaneurysm[12,20].Severalsurgicalap-proaches,suchassubtemporalapproach,pterionalapproach,supraorbitalapproach,orbitozygomaticapproach,transpetrosalapproachandcombinedap-

proachesarecurrentlyusedforthetreatmentofBBAs[1–6,8–11,13–19].Thesubtemporalapproachistheoldestone,whichwaswidelyusedbyDrakeandPeer-lessinmorethan1000patients[2–4].Theseniorau-thorHernesniemiadoptedtheusethesubtemporalapproachinthe80’safterstudyingchapterswrittenbyDrakeandPeerlessindi¤erenttextbooks.Thisexperiencewaslaterrefinedbycoworkingwiththeseauthors[4,7].Wedescribeourrecentlymodifiedoper-ativetechniqueofthesubtemporalapproach.

OperativeprocedureCraniotomy,approachtotentorialedgeandbasilarbifurcation

Thepatientispositionedintheparkbenchposition.Asmallsubtemporalcraniotomythroughalinearincisionisdone.Ordinarily,thenon-dominant,rightsideisused,unlesstheprojectionorcomplexityoftheaneurysm,scarringfromearlieroperations,severean-atomicalrotationofthebasilarbifurcation,leftoculo-motorpalsy,leftsidedblindnessorrighthemiparesis,makesanapproachfromthedominantsidenecessary.Thesubtemporalapproachcanbeconvertedintoapterionalorposteriortemporalboneflap,which,how-ever,israrelyneeded.Theskinincisionisatrightangletothezygomaabout1cminfrontoftheearandiscar-rieddowntothelevelofthezygomaticarch(Fig.1A).Here,caremustbetakennottoinjuretheupperbranchofthefacialnerve,anditisadvisabletoretractthelowerpartoftheincisiondownwardswitha‘‘fishhooks’’(Fig.1A,C).Afterincisingthefasciaandtem-poralmusclealongitsfiberstothearch,itisimportant32J.Hernesniemietal.todetachthemusclewithitsfasciaposteriorlyfromthetemporalrootofthezygoma(Fig.1D).Anteriorly,bothleavesofthetemporalfasciaareseparatedfromthearchonecentimeter’sdistancewithbluntdissec-tion,nottoinjurethenerve(Fig.1D).Thisallowswiderspreadingofthetemporalmusclewithacurvedretractorandfishhooks(Fig.1D).Asmallboneflapsized3Â3cmisdonewithoneburrholeandcranio-tometakingcarenottoinjurethedurawhichmaybeheavilyattachedtotheskullatthemostbasedpartofthecraniotomy(Fig.1D,E).Resectionofapartofthezygomaticarchcanbedonewithadiamonddrill,butisnotoftennecessaryorveryhelpful(Fig.1F,G).Thetrickoftheproperuseofthesubtemporalapproachliesingettinginquickly,withoutheavycom-pressionofthetemporallobe,andwithjustenoughspacetoreachthetentorialedge.Lumbardrainageinstalledbytheneurosurgeonbeforesurgery,isthekeytosuccessfulsurgery(Fig.1B).Modernneuro-anesthesiawithMannitolmaybehelpfultoaccelerateslackbrain,butwithoutCSFdrainagewewouldnotberecommendtostartasubtemporalapproach.Inthepresenceofhydrocephalusafrontalventriculardrainagehasbeenapplied,andwhenwideningtheopeningintoapterionalapproachlaminaterminaliscanalsobeopened.Withspinaldrainageof50–100mlofCSF,thebrainwillbeslackwhentheduraisopened,eveninearlysurgeryforrupturedaneurysmsingoodgradepatients(Fig.1B,H).AfteropeningthedurainV-shapedform,basedownwards,andafterverycarefulfixationofthedurawithstitchestothesurroundingmusclesordraping,themicroscopeshouldbebroughtintoposition(Fig.1H).Fibringluehasprovedtobeextremelyusefulinstoppingpossibleepiduralbleed-ings.Absolutehemostasisisdonebeforeproceedingdeeper;otherwisebloodmaybepouredontheopera-tionfieldatthemostcriticalmomentsofdeepsurgery.Theretractionofthetemporallobeshouldbeslowlyincreased.Thebrainiscoveredwithoxidizedcelluloseandlargewetcottonoids.Thelineofretractiongoesfirstslightlydownwards,thenacrossthefloorofthemiddlefossaandafterthatupwardstothetentorialedge.Thecourseofcreatingtheoperativerouteisliketakingthetemporallobeinyourretractinghandandpullingupwardstoseethetentorialedge.AbroadAes-culapo;Rretractorisused,verygently,andanothermayassisttoholdthetemporalpoleanteriorly.Theangleofthesubtemporalapproachordinarilyisnearlyperpendiculartothesagittalplane,buteventhroughasmallcraniotomytheanglecanbechanged.AnoppositeP1hiddenbehindalargesacwithitsper-foratorscanusuallybeseenbyanglingtheretractorforwardafewdegreesunderthetemporalpole,chang-ingtheangleoftheoperatingmicroscopeandthendis-placingthewaistofthesacposteriorly.Thismaneuverisparticularlyappropriatewhenalargerboneflaphasbeenused.Associatedipsilateralintactcarotidaneur-ysmsareeasilyexposedforclippingbymovingtheretractortipforwardafewcentimetersunderthetem-poralpole.Oneortwotemporallobebridgingveinscrossingtothefloorofthemiddlefossainthelineofapproachmustbedivided,butusuallyotherveinsoneithersidecanbespared.Theadjacentbridgingveinsareunderstretchbuttheymaybedissectedafewmillimetersfromarachnoidtogetmorefreedomforretraction.ThejunctionoftheveinofLabbe´withthelateralsinusisfurtherawayposteriorlylocatedandnotinjeopardyinthisapproach.Itiscriticalthatthisveinisnotinjured,toavoidmajortemporallobehemorrhagicinfarction.Astheedgeofthetentoriumcomesslowlyintoview,havingthetemporallobeinyour‘‘hand’’,theuncuswillleaveitspositioninsidetheedgeofthetentorium.Theuncusisthelandmarkforthisretraction,asitsel-evationbytheretractortipexposestheopeningintotheinterpeduncularcistern.Amedialtemporalveinmaybedividedbeforethefinalretractorplacementonthetemporallobemesiallyatthebaseorunderthetipoftheuncus.Astheuncusisraisedwiththetipoftheretractor,thethirdnerveiselevatedwithit,withoutplacingdis-turbingangulartensiononit.Itisusuallypossibletoworkbelowthethirdnervetocliptheaneurysmexceptforthecaseofahigherbasilarbifurcationoragiant
