X线成像原理

X线是一种电磁波，具有穿透性、 荧光性和摄影效应，能够穿透人 体组织并留下影像。

X线成像的原理是利用不同组织对 X线的吸收程度不同，通过对比度 差异来呈现人体内部结构。
肺部的正常形态

两肺形态对称
人体有两个肺，左右两侧 形态对称，呈圆锥形，底 面向上，尖端向下。
肺野广阔
肺野是充满空气的肺部区 域，在X线片上表现为高 透亮、低密度的影像，占 据胸腔的大部分空间。
肺门结构清晰
肺门是支气管、血管和淋 巴管等结构进出肺部的开 口，在X线片上表现为结 构清晰的影像。
纵隔的正常形态
纵隔的正常形态可分为前、中、后三 个部分。前纵隔位于胸骨后方，中纵 隔位于心脏和大血管的周围，后纵隔 则位于胸椎前方。
在前后位片上，前纵隔表现为心影后 方的三角形透明区，中纵隔则呈现出 心脏和大血管的形状和位置，而后纵 隔则与胸椎影像重叠。
在侧位片上，前纵隔表现为心影后方 的一条浅沟，中纵隔则呈现出心脏和 大血管的轮廓，而后纵隔则表现为胸 椎前方的致密影。
了解这些血管和器官的位置和形 态对于诊断和治疗胸部疾病具有
重要的意义。

心脏的血管和心腔
主动脉发自左心室，向右前上至 主动脉弓转折为升主动脉，经肺 动脉起始部至肺动脉分叉处为左、
右肺动脉。肺静脉自肺门Fra bibliotek心房汇入左、右 心房。
腔静脉窦位于右心房的后部，接 受上、下腔静脉回流入心。

心脏位于胸腔中部，纵隔内，夹于两侧胸膜囊之间，前方对胸骨体和第2-6肋软骨，后方 平第5-8胸椎。
心脏外形与分叶
心脏外形似倒置的圆锥体，大小约相当于本人的拳头。心脏可分为一尖、一底、两面、三 缘和三沟。

气管位于纵隔内，向下分为左右主支 气管，再逐级分支形成各级支气管。

先天性变异
如肺隔离症、先天性支气 管囊肿等，多与胚胎发育 过程中发育异常有关。
获得性异常
如肺炎、肺结核、肺癌等 ，多由于感染、炎症、肿 瘤等因素导致胸部结构和 功能异常。

• 气管以软骨、肌肉、结缔组织和粘膜构成。软骨为“C”字形的软骨 环，缺口向后，各软骨环以韧带连接起来，环后方缺口处由平滑 肌和致密结缔组织连接，保持了持续张开状态。管腔衬以粘膜， 表面覆盖纤毛上皮，粘膜分泌的粘液可粘附吸入空气中的灰尘颗 粒，纤毛不断向咽部摆动将粘液与灰尘排出，以净化吸入的气体。
胸廓
• 胸廓(thoracic cage)是胸腔壁的骨性基础和支架。胸廓由12个胸椎， 12对肋(ribs)，和1个胸骨(sternum)借关节、软骨连结而组成。
• 成人胸廓呈前后略扁的圆锥形。胸廓入口由第1胸椎体、第1肋和 胸骨柄上缘围成，是颈部与胸腔之间的通道。胸廓下口较大，由 第12胸椎体、第12肋和11肋前端、肋弓和剑突围成。相邻两肋之 间的间隙称肋间隙，共11对。两侧肋弓之间的夹角称胸骨下角。 新生儿的胸廓呈桶状；老年人的胸廓则扁长；成年女性的胸廓短 而园。
• 上纵膈内主要有胸腺、出入心的大血管、迷走神经、膈神经、气管、食管、 胸导管等。前纵膈仅含有少量结缔组织和淋巴结，中纵膈主要含有心包、心 及出入心的大血管根部，后纵膈内含有胸主动脉、奇静脉及其属支、主支气 管、食管、胸导管、迷走神经、交感神经和淋巴结等。

纵隔：有四分法、六分法、七分法、九分法等
在胸部侧位片上将纵隔分为前、中、后及上、中、下 九个区（如图），前纵隔系胸骨之后，心、升主动脉和 气管之前的狭长三角区。中纵隔相当于心、主动脉弓、 气管及肺门所占据的区域。食管前壁为中、后纵隔的分 界线。食管以后和胸椎旁区为后纵隔。自胸骨柄、体交 界处至第4胸椎下缘连一水平线，其上为上纵隔，肺门 下缘的连线，约第8胸椎下缘水平，其下至膈为下纵隔。 两条水平线之间为中纵隔。
