仙灵骨葆对去势大鼠骨量和生物力学性能的影响邢磊焦颖华1耿丽华孙尧(河北联合大学附属医院老年病科,河北唐山063000)〔摘要〕目的探讨仙灵骨葆对骨质疏松(OP )大鼠骨量、骨代谢和生物力学性能的影响。

方法3月龄雌性SD 大鼠24只分为3组,每组8只:正常对照组(N )、卵巢切除组(OVX )、卵巢切除+仙灵骨葆治疗组(XLGB )。

除N 组外,其余两组行卵巢切除术,6w 后XLGB 组给予药物干预:250mg ·kg -1·d -1,OVX 组给予等量生理盐水,8w 后处死所有大鼠。

留取尿液、血清检测血PINP 值、尿DPYD /Cr 、NTX /Cr 值。

取左侧股骨行骨密度测定,取左侧胫骨制备硬组织不脱钙切片,备行骨组织形态计量学检测,取右侧股骨行三点弯曲试验,检测其最大载荷。

结果OVX 组血PINP 、尿DPYD /Cr 、尿NTX /Cr 值显著高于N 组,XLGB 能显著降低血PINP 、尿DPYD /Cr 、尿NTX /Cr 值,但仍显著高于N 组。

OVX 组股骨全长及近、中、远三段骨密度均显著低于N 组,XLGB 组近、远端骨密度显著高于OVX 组。

BV /TV 在OVX 组显著低于N 组,XLGB 组显著高于OVX 组;OVX 、XLGB 组骨吸收指标Oc.N 、Er.Pm 均显著高于N 组,XLGB 组Oc.N 、Er.Pm 显著低于OVX 组,BFR /BV 显著高于OVX 组。

最大载荷三组之间无显著差别。

结论仙灵骨葆灌胃可抑制卵巢切除大鼠骨量丢失,其机制与促进骨形成、抑制骨吸收,降低骨转换水平,进而维持骨量及微观结构有关。

仙灵骨葆胶囊干预老龄骨质疏松模型大鼠骨力学特性的变化李鹏;李新颖;李正伟;李亚军【摘要】BACKGROUND: The effect of Xianling Gubao Capsule for the elderly female osteoporotic animal models assessed by compression and bending mechanical properties is little reported.OBJECTIVE: To investigate the curative efficacy of Xianling Gubao Capsule, alendronate sodium, calcium carbonate and parathyroid hormone (3-34) for an osteoporotic rat model through compressive and bending tests at the femur.METHODS: The rat models of osteoporosis were established by bilateral ovariectomy, and were then subjected to the treatment of Xianling Gubao Capsule, alendronate sodium, calcium carbonate and parathyroid hormone (3-34),respectively, for 20 weeks. Afterwards, the rats were killed by cutting the abdominal aorta, and then the bilateral femurs were removed to undergo compressive and bending tests.RESULTS AND CONCLUSION: The compressive test: the elastic stress, elastic load, elastic strain, elastic displacement,maximum load, maximum strain, maximum stress, maximum displacement and elastic modulus in the model group were significantly lower than those in the blank control, Xianling Gubao Capsule, alendronate sodium and parathyroid hormone (3-34) groups (P < 0.05). The maximum load, maximum strain, maximum stress, and maximum displacement did not differ significantly between Xianling Gubao Capsule and blank control groups (P > 0.05). All indexes in the compressive and bending tests showed no significant differences between model and calcium carbonategroups (P >0.05). Bending test: the maximum load, maximum stress, maximum strain, maximum bending moment and bending section modulus in the model group were significantly lower than those in the blank control, Xianling Gubao Capsule,alendronate sodium and parathyroid hormone (3-34) groups (P < 0.05). The bending mechanical indexes showed no significant differences between Xianling Gubao Capsule and blank control groups (P > 0.05). 仙珍骨宝干预泼尼松诱发大鼠的骨丢失许碧莲;陈广斌;陈文双;张新乐;吴铁;崔燎【摘要】BACKGROUND: Xianzhengubao capsule made from Cnidium, Epimedium and others has a certain protective role in glucocorticoid and retinoic acid-induced osteoporosis.rnOBJECTIVE: To study the effect of Xianzhengubao on different bones in prednisone-treated rats by bone histomorphometry and bone biomechanics methods.rnMETHODS: Thirty-two Sprague-Dawley rats were randomly divided into control, prednisone, high- and low-dose Xianzhengubao groups. Rats in the latter groups were given prednisone at 3.5 mg/kg-d first to establish osteoporosis models. High- and low-dose Xianzhengubao groups were administrated with Xianzhengubao capsule at 0.95 g/kg-d and 0.475 g/kg-d, respectively, after prednisone treatment.rnRESULTS AND CONCLUSION: Compared with the control group, bone mineral density and biomechanical properties (elastic load, maximum load, break load) of the femur were significantly decreased in the prednisone group. Percent labeled perimeter, bone formation rate per bone surface and per unit of bone volume of the proximal tibial metaphysis were decreased in the prednisone group, but there was no significant difference both in static parameters of proximal tibial metaphysis and in tibial shaft. Compared with the prednisone group, bone mineral density and biomechanical properties of the femur, percent labeled perimeter and bone formation rate per bone surfaceof the proximal tibial metaphysis were increased, but there was no significantchange in tibial shaft in the high-dose Xianzhengubao group. Compared with the prednisone group, there was no significant change in the low-dose Xianzhengubao group. 仙灵骨葆治疗骨质疏松的临床疗效目的评价仙灵骨葆治疗骨质疏松症的临床疗效和安全性,为骨质疏松症的治疗找到一种安全、方便、有效的途径。

方法选择84例骨质疏松患者,随就诊先后时间将84例患者平均分为治疗组与对照组,治疗组选择仙灵骨葆胶囊治疗,对照组选择乐力胶囊治疗,比较两组疗效。

结果两组药物均对骨质疏松症有明显的疗效,服用仙灵骨葆的治疗组在治疗原发性骨质疏松的各方面数据均优于服用乐力胶囊的对照组。

两组不良反应发生率时比差异有显著性(P<0.05)。

结论运用纯中药仙灵劳葆治疗骨质硫松,能有效缓解甘质疏松临床症状、增加骨密度,乐力胶囊疗效对某些人而言有不良反应。

标签:仙灵骨堡;骨质疏松;临床试验骨质疏松是由于种原因引发的钙盐流失而导致全身骨病,其中原因以年龄和女性内分泌水平低下最为多见。

临床症状多为易骨折、产生疼痛、乏力及下肢麻木等。

临床治疗主要以补钙或加用少量激素治疗,但疗效低下,虽能缓解症状,但停药后症状很快复发,且副作用较大。

我院选择中药制剂仙灵骨葆治疗骨质疏松,疗效显著,现报道如下。

1 资料与方法1.1一般资料选择84例骨質疏松患者,均为我院2010年1月~2011年12月门诊收治,所有患者均经Hologic QDR-1000型双能DEXA线测定、影像学检查及临床症状确诊。

男性56例,女性28例,年龄45~82岁,平均年龄(60.85±6.32)岁,其中女性均为50岁以上。

随就诊先后时间将84例患者平均分为治疗组与观察组,两组患者性别构成比、平均年龄、骨质疏松程度等无明显差异,具有可比性。

1.2方法为研究效果准确,两组患者饮食结构、锻炼等均进行统一安排,足量用药4 w为1疗程。

1.2.1治疗组选择仙灵骨葆胶囊(批准文号:国药准字Z20025337;厂家:桂林同济堂制药有限公司;规格:0.5 mg/粒)口服治疗,饭前空腹服用,3粒/次,2次/d。

1.2.2对照组选择乐力胶囊,复合氨基酸螫合钙(商品名:国药准字H20093675;厂家:四川维奥制药有限公司,500 mg/片)咀嚼服用,1片/次,2次/d。

1.6 统计学处理 各组大鼠所有观察结果均以均数±标准差(±s)表示,两组均数之间比较采用t检验。
2 结 果
2.1 DEXA骨密度结果 如表2~3所示大鼠卵巢切除术后3个月,全身骨密度显著低于假切除组(P<0.05),提示骨质疏松症动物模型成立。大鼠卵巢切除术后6个月,骨密度明显低于假切除组(P<0.025)。应用仙灵骨葆治疗后可以明显改善骨丢失加速的过程,提高骨量,与假切除组比较无明显差异(P>0.05),同应用倍美力治疗的结果相似,各组间无差异。
1 材料与方法
1.1 药物 仙灵骨葆胶囊(96黔卫药准字100019,贵州仙灵药业有限公司产品,每粒含生药0.75g),倍美力(96卫药准字J-35号,美国惠氏-艾尔斯特公司大药厂产品,片剂,合成雌激素类)。
1.2 实验动物及饲养 10月龄健康雌性SD大鼠48只,体重350±20g,由上海中医药大学实验动物部提供(上海市医学实验动物管理委员会实验动物合格证号01597)。各组大鼠分笼饲养,自由摄取标准大鼠饲料,自由饮用蒸馏水,室温22±2℃。

1.5 大鼠股骨干重及骨钙含量检测 处死大鼠后,取出股骨,剔除干净肌肉及结缔组织,生理盐水冲洗干净髓腔,120℃烘干lh,阿基米德原理,万分之一天平称重量,并测定密度。然后将股骨置于脱脂液中(三氯甲烷:甲醇=2:1v/v)72h,120℃烘6h,冷却称重,该重与体积相比即为干重。硝酸镧法测定大鼠股骨骨钙含量,精称0.1g灰化骨,4ml盐酸溶解,0.2%硝酸镧溶液稀释1000倍,原子分光光度计测试。

仙灵骨葆胶囊治疗骨质疏松症的疗效及其对骨代谢及骨转换指标的影响分析摘要:目的探析研究仙灵骨葆胶囊治疗骨质疏松症的疗效及其对骨代谢及骨转换指标的影响。

方法在本院2015年5月-2017年6月间收治的骨质疏松症患者中选择70例进行分组治疗,对照组采取常规治疗,观察组则加以使用仙灵骨葆胶囊进行治疗,各35例。

结果2组对比治疗效果,显示观察组总有效率为94.3%,对照组为77.1%,对比差异明显(P<0.05)。

对比两组骨密度(BMD)、骨钙素(OC)、I型胶原交联C-末端肽(CTX-1),提示观察组更佳(P<0.05);对比血清钙(Ca)、磷(P)以及碱性磷酸酶(ALP)等指标则提示差异不明显(P>0.05)。

治疗期间2组均未见明显不良反应。

结论仙灵骨葆胶囊治疗骨质疏松症的效果显著,可有效改善骨转换状态及骨密度,值得推广。

药物保守疗法是临床治疗该病症的常用方法,钙尔奇、福善美等是临床常用药物,其虽可在一定程度上改善患者的临床症状,但其引发的副作用也需要注意[1-2]。

近年来仙灵骨葆胶囊在临床推广应用,为探究该药物治疗骨质疏松症的疗效及其对骨代谢及骨转换指标的影响,现将在本院2015年5月-2017年6月间收治的骨质疏松症患者中选择70例进行分组治疗,现将2组患者的治疗结果做下述分析报道。

1资料与方法1.1病例资料在本院2015年5月-2017年6月间收治的骨质疏松症患者中选择70例进行分组治疗,2组各35例。

纳入标准:①经检查确诊为骨质疏松症。

②患有其他严重疾病。

③对研究随用药物无使用禁忌。

④对研究知情同意。

排除标准:①病历资料不完整。

②患者患有精神疾病或意识障碍,无法进行正常交流。

对照组:男性19例,女性16例。

患者年龄:56-78岁,平均年龄为(70.4±4.7)岁。

新仙灵骨葆胶囊对大鼠骨折愈合的影响研究目的:研究新仙灵骨葆胶囊对大鼠骨折愈合的影响,为寻求“减量增效(减量等效)”新组方提供参考。

方法:将大鼠随机分为假手术组(蒸馏水)、模型组(蒸馏水)、仙灵骨葆胶囊组(350 mg/kg)和新仙灵骨葆胶囊低、中、高剂量组(53、105、210 mg/kg),每组14只。

除假手术组外,其余各组大鼠均在麻醉状态下制作右侧股骨中段骨折模型。

待大鼠术后清醒,各组大鼠灌胃相应药物10 mL/kg,每天给药1次,连续4周。

末次给药后,测定大鼠体质量,观察骨折处骨痂形成及组织病理学变化,进行骨折侧生物力学(骨折应力、骨压碎力)测量,并采用酶联免疫吸附法检测血清中炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)] 水平。

结果:与假手术组比较,模型组大鼠体质量和骨折侧骨折应力、骨压碎力明显降低(P<0.05或P<0.01),血清中TNF-α、IL-1β水平明显升高(P<0.01);模型组大鼠骨折周围可见明显骨痂形成,骨小梁不成熟、排列混乱。

与模型组比较,新仙灵骨葆胶囊高剂量组和仙灵骨葆胶囊组大鼠体质量明显增加(P<0.05),新仙灵骨葆胶囊中、高剂量组和仙灵骨葆胶囊组大鼠骨折侧骨折应力明显升高(P<0.05或P<0.01),各给药组大鼠血清中TNF-α、IL-1β水平均明显降低(P<0.01);且新仙灵骨葆胶囊各剂量组与仙灵骨葆胶囊组比较上述指标差异均无统计学意义(P>0.05)。

各给药组大鼠骨痂面积均变小,骨折线变模糊,骨折痕迹基本消失,骨小梁数量增多;骨折处皮质层增厚,骨髓腔见大量毛细血管植入。

结论:新仙灵骨葆胶囊具有明显的促进骨折愈合作用,可有效提高骨生物力学强度,抑制炎症反应。

ABSTRACT OBJECTIVE:To study the effects of new Xianling gubao capsule on fracture healing in rats,and to provide reference for seeking new formula of “to reduce stockpiles and increase efficiency (to reduce stockpiles and constant efficiency)”. METHODS:Rats were randomly divided into sham operation group (distilled water),model group (distilled water),Xianling gubao capsule group (350 mg/kg),new Xianling gubao capsule low-dose,medium-dose and high-dose groups (53,105,210 mg/kg),with 14 rats in each group. Except for sham operation group,right middle femur fracture model was established under the anesthetic state. Rats were given relevant medicine 10 mL/kg intragastrically after woken,once a day,for consecutive 4 weeks. After last administration,body weights of rats were determined;the formation of callus and histopathological changes in fracture were observed;biomechanics of fracture side (fracture stress and bone crushing force)was measured. The levels of inflammatory factors (TNF-α,IL-1β)in serum were detected by ELISA. RESULTS:Compared with sham operation group,body weight,fracture stress and bone crushing force of fracture side were decreased significantly (P<0.05 or P<0.01),while the serum levels of TNF-α and IL-1β were increased significantly (P<0.01). In model group,the scab was visible around the fracture,and the trabecular bone was not mature and arranged in confusion. Compared with model group,body weights of rats were increased significantly in new Xianling gubao capsule high-dose group and Xianling gubao capsule group (P<0.05);fracture stress of fracture side were increasedsignificantly in new Xianling gubao capsule medium-dose and high-dose groups and Xianling gubao capsule group (P<0.05 or P<0.01). The serum levels of TNF-α and IL-1β were decreased significantly(P<0.01). There was no statistical significance in above indexes among new Xianling gubao capsule groups and Xianling gubao capsule group (P>0.05). The area of the callus in each group was smaller;the fracture line became blurred;the fracture trace disappeared and the number of bone trabeculae increased. The cortical layer was thickened,and a large number of capillary implants were found in the bone marrow cavity. CONCLUSIONS:New Xianling gubao capsule has a significant role in promoting fracture healing,and can effectively improve the strength of bone biomechanics and inhibit the inflammatory reaction.KEYWORDS New Xianling gubao capsule;Rats;Fracture;Bone biomechanics;Inflammatory factors仙灵骨葆胶囊为治疗骨质疏松症类非处方药品,2009年开始被收录在《国家基本药物目录》中,是防治骨质疏松症的首选药物。
