清洁灌肠后保留灌肠多层螺旋CT扫描在直肠占位中的应用【摘要】目的:探讨清洁灌肠后保留灌肠多层螺旋ct扫描方法在直肠占位检查中的诊断价值。

方法:对51例疑有直肠占位的患者行清洁灌肠后保留灌肠ct扫描,观察直肠肠腔及肠周结构显示效果。

结果:51例患者病变显示阳性率100%,ct征象有:肠壁增厚,腔内肿块,肠腔狭窄,以及肠周邻近结构侵犯情况。

结论:清洁灌肠后保留灌肠多层螺旋ct检查对直肠占位的诊断有很大价值,良好的肠道准备及扫描方法是多层螺旋ct对直肠恶性病变诊断的关键。

方法选取2021年7月—2023年5月新沂市人民医院收治的89例肺结节患者为研究对象,对所有研究对象予以胸部螺旋CT平扫和靶重建技术,收集并分析不同检测方法的准确度、灵敏度及特异度。

结果 CT靶重建技术准确度为97.75%(87/89)、特异度为97.14%(34/35)、灵敏度为98.15%(53/54),高于胸部螺旋CT平扫,差异有统计学意义(χ2=19.264、12.857、7.053,P<0.05);CT靶重建技术影像特征符合率高于胸部螺旋CT 平扫,差异有统计学意义(P<0.05)。

结论在针对肺结节良恶性进行诊断时,选择CT靶重建技术进行检查能够进一步提高检测的准确度,能够更准确地检测出病灶影像特征,检测图像清晰度更高。

Methods A total of 89 patients with pulmonary nodules in Xinyi People's Hospital from July 2021 to May 2023 were enrolled. Chest spiral CT plain scan and target reconstruction were performed on all subjects, the accuracy, sensitivity and specificity data of different detection were collected and analyzed. Results The accuracy, specificity and sensitivity of CT target reconstruction were higher than those of CT plain scan, and the difference was statistically significant (χ2=19.264, 12.857, 7.053, P<0.05). The accuracy of CT target reconstruction was 97.75% (87/89), the specificity was 97.14% (34/35) and the sensitivity was 98.15% (53/54). The coincidence rate of CT target reconstruction was higher than that of chest spiral CT plain scan, and the difference was statistically significant (P<0.05). 多层螺旋CT血管成像动态图像后处理对脑血管畸形病变的诊断价值研讨王丽,周田,刘斯咪[摘要]目的研究多层螺旋CT血管成像动态图像后处理运用于脑血管畸形病变患者中的效果。

方法随机选取2020年10月—2023年10月聊城市人民医院接诊的70例疑似脑血管畸形病变患者为研究对象,将数字减影血管造影结果作为诊断金标准,并且提供多层螺旋CT血管成像动态图像后处理,应用CT多平面重组、三维容积漫游及联合诊断分析诊断效能。

结果数字减影血管造影阳性检出率为48.57%,CT多平面重组诊断阳性检出率为42.86%,三维容积漫游诊断阳性检出率为44.29%,联合检查阳性检出率为51.43%。

CT多平面重组诊断、三维容积漫游诊断的灵敏度、特异度、准确度相比,差异无统计学意义(P均>0.05)。

联合诊断的灵敏度、准确度显著高于CT多平面重组诊断,差异有统计学意义(P均<0.05)。

三维容积漫游诊断、联合诊断的灵敏度、特异度、准确度比较,差异无统计学意义(P均>0.05)。

结论多层螺旋CT血管成像动态图像后处理中,CT多平面重组联合三维容积漫游诊断运用于脑血管畸形病变患者中具有较高的诊断效能,诊断价值高。

Methods Seventy patients with suspected cerebro⁃vascular malformation treated in Liaocheng People's Hospital from October 2020 to October 2023 were ran⁃domly selected as the study objects. The results of digital subtraction angiography were used as the diagnostic gold standard, and dynamic image processing of multi-slice spiral CT angiography was provided, application of CT multiplanar reconstruction, three-dimensional volume roaming and combined diagnosis to analyze diagno⁃sistic performance. Results The positive detection rate of digital subtraction angiography was 48.57%, the posi⁃tive detection rate of multi-plane CT recombination diagnosis was 42.86%, the positive detection rate of three-dimensional volume wandering diagnosis was 44.29%, and the positive detection rate of combined examination was 51.43%. There was no significant difference in sensitivity, specificity and accuracy between CT multi-plane recombination diagnosis and three-dimensional volume roaming diagnosis (all P>0.05). The sensitivity and accuracy of combined diagnosis were significantly higher than that of CT multi-plane recombination diag⁃nosis, and the differences were statistically significant (all P<0.05). 腰椎狭部裂的螺旋CT诊断价值【摘要】目的探讨ct在腰椎椎弓峡部不连诊断中的临床价值及提高检出率,提高对该病的认识和诊断水平,方法搜集腰椎螺旋ct容积扫描,经后处理重建检出的腰椎峡部裂55例,分析其mpr、vr成像的ct表现,并与常规椎弓ct轴位像进行对照,评价对峡部裂及其继发病变的显示情况。

结果 mpr冠状面、经椎弓矢状面重建结合vr成像可清晰显示55例108处峡部裂; mpr结合vr图像重建对峡部裂特征以及裂隙骨赘、椎间孔狭窄、椎体滑脱、椎间盘后突的相关继发病变征象显示均有明显优势。

多排螺旋CT在骨关节创伤中的诊断价值【摘要】目的:探讨16层螺旋ct三维重建技术在骨关节创伤性疾病中的诊断价值。

方法:骨关节创伤92例,伤患处均行16层螺旋ct横断面扫描,在工作站行三维重建技术后处理,即表面遮盖显示(ssd)、多平面重建(mpr)及容积重建(vr)技术,与x线平片比较,评价3种重建方法对骨折或脱位的显示效果。

结果 16层螺旋横断面检出91例,mpr重建对所有骨折的部位及形态全部明确诊断,获ssd重建技术明确诊断89例,获vr重建技术明确诊断90例。

经16层螺旋ct横断面扫描可发现细小骨折,但立体信息不足;三维重建技术可多角度、全方位观察骨折部位的骨结构空间关系,ssd能立体、逼真地显示骨折及脱位情况,mpr能兼顾软组织的改变,在骨折线走行和移位程度具有优势,在显示细小的骨折线上更具优势。

结论:各种三维重建技术各有优势,与16层螺旋ct联合使用可提高对骨关节损伤诊断的准确性,为临床诊治提供更多准确、直观的信息。

方法随机选取2020年1月—2023年10月济宁市第三人民医院收治的80例结肠癌患者为研究对象,均予多层螺旋CT检查,以手术病理结果为依据,评估CT对结肠癌术前TNM分期的诊断准确率及与病理结果的一致性。

结果在结肠癌术前分期诊断中,多层螺旋CT对T1、T2、T3、T4期诊断符合率80.00%、91.67%、100.00%、93.33%,T分期准确率93.75%;对N0、N1、N2期诊断符合率88.00%、85.29%、95.24%,N分期准确率88.75%;对M分期诊断准确率91.25%;对TNM分期诊断结果与手术病理结果一致性较好(kappa= 0.91、0.83、0.81)。

结论结肠癌患者术前予多层螺旋CT检查TNM分期诊断准确率高,且与手术病理结果一致性较好。

Methods A total of 80 patients with colon cancer admitted to the Third People's Hospi⁃tal of Jining from January 2020 to October 2023 were randomly selected as the research objects. All patients were examined by multilayer spiral CT. Based on the surgical pathological results, the diagnostic accuracy of CT for preoperative TNM staging of colon cancer and the consistency with pathological results were evaluated.Results In the diagnosis of preoperative staging of colorectal cancer, multilayer spiral CT had a diagnostic con⁃cordance rate of 80.00%, 91.67%, 100.00%, and 93.33% for T1, T2, T3, and T4 stages, and an accuracy rate of 93.75% for T staging. The diagnostic compliance rate for N0, N1, and N2 stages was 88.00%, 85.29%, and 95.24%, and the N staging accuracy rate was 88.75%. The diagnostic accuracy rate for M staging was 91.25%; the diagnostic results for TNM staging were in good agreement with the surgical pathology results (kappa=0.91, 0.83, 0.81). 186泌尿系梗阻性疾病在临床发病率很高,泌尿系梗阻性疾病发作时,常因病因诉说不清,或临床症状不典型而容易延误诊断,随着CT 技术的进步,MSCT 大范围扫描,多层螺旋CT 尿路造影(MSCTU)的应用,可多方位观察双肾、输尿管及膀胱等病变的形态,能在定位、病程发展和并发症方面给泌尿系梗阻性疾病的诊断提供更多的影像学依据,为临床选择恰当的治疗方案提供影像学依据。

1 资料与方法1.1 一般资料 选取本院2005年2月—2011年6月行MSCT 检查的泌尿系梗阻性疾病患者62例为观察组,男42 例,女20例,男女之比为2:1,<30岁23例,30~50岁25例,>50岁14例,平均年龄38.4岁。

1.2 仪器 西门子16排螺旋CT 机,电压为120kV,电流为150mA。

1.3 MSCT 扫描方法 自第12胸椎至耻骨联合水平,层厚:5mm,1次闭气完成扫描。

根据病人状态可选择以下几种方法:①非增强的螺旋CT 检查法:在1次摒气下,连续扫描多层,不需要对比剂;②静脉注入对比剂法:平扫和三期动态增强扫描。

增强方式:从上肢静脉经高压注射器注入欧乃派克80~100mL,注射速率2.5m/sL,分别于开始后30s(皮质期)、90s(实质期)、180s(排泌期)进行三期增强扫描,必要时增加延时15~30min 扫描。

1.4 图像后处理技术 将各期原始数据1mm 重建,分别采用最大密度投影(MIP )、多平面重建(MPR )、曲面重建(CPR )和容积再现(VR )等方法进行图像处理。

2 结 果62例病例中38例诊为肾盂或输尿管结石,平扫时即可发现,CT 值:150~1800Hu,CTU 表现为腔内高密度影,梗阻上方输尿管及肾盂不同程度扩张。

9例为输尿管狭窄,CTU 表现为输尿管狭窄段呈突然狭窄或渐进性,但无明显管壁增厚,伴有狭窄段上方输尿管扩张积水;6例为肾盂或输尿管肿瘤,MSCTU 检查表现为肾盂内乳头状软组织影,输尿管壁局限性增厚、狭窄,伴有输尿管扩张积水,增强扫描实质期病变中度强化;5例为膀胱肿瘤,病变位于膀胱三角区附近,增强扫描实质期肿块略有强化,延迟期膀胱内可见肿块样充盈缺损;2例肾盂输尿管畸形;2例输尿管外压性梗阻,均为转移瘤所致鸟嘴样狭窄,多层螺旋CT三维尿路造影(MSCTU )诊断泌尿系梗阻性疾病临床价值吴涛(辽宁中医药大学附属二院医学影像科,辽宁 沈阳 110034)摘 要:目的:探讨螺旋CT 对临床表现泌尿系梗阻性疾病的诊断价值。

.6中国医疗器械信息 | China Medical Device Information论著Article隐匿型肋骨骨折是指用常规X 射线检查难以发现或难以及时发现的骨折,经过一段时间治疗或者用其他影像学检查方法才能发现的骨折。

在影像学中,肋骨骨折的X 射线片和CT 片的成像特点不同,由于X 射线检查的灵敏度和肋骨解剖学的结构特殊性,对于有错位的骨折可诊断明确,比较轻微和重叠区的骨折难以及时发现,而且由于患者体位、投照位置和角度、以及条件限制,传统X 射线片难以充分地展示骨折的部位和数目,易发生漏诊;CT 检查优于X 射线检查之处在于其密度分辨率高,图像清晰,解剖关系明确,能提供没有组织重叠的横断面图像,且三维重建可进行冠状面、矢状面以及三维立体图像的重建,可以更逼真地显示病变的部位图像,弥补了X 射线片和常规CT 检查的不足,提高了骨折诊断的准确性[1]。

为了研究探讨隐匿型肋骨骨折在64排螺旋CT 三维重建的检查与应用价值,为临床诊疗及法医鉴定提供准确的诊断依据。

对2015年1月~2016年12月来本院诊治的96例胸部外伤患者相关资料进行分析,报道如下。
