【性状】 本品为白色疏松体粉剂。
【适应症】 移植用免疫抑制剂:预防和治疗器官排异反应。 预防造血干细胞移植术后的急性和慢性移植物抗宿主病(GvHD)。 治疗激素耐受的移植物抗宿主病 (GvHD)。 血液学疾病:治疗再生障碍性贫血。
【规格】 25mg/瓶 (每瓶含兔抗人胸腺细胞免疫球蛋白25mg,复溶后体积5ml。)
• 再生障碍性贫血: 2.5~3.5mg/kg/日,连续5日。相应的累积剂量12.5~17.5mg/kg。 此产品用于治疗再障的临床对照试验暂缺。
剂量调整 对于包括血小板减少症和/或白细胞减少症(特别是淋巴细胞减少症和中性粒细胞减少 症)的血液学影响,通过调整药物剂量,可逆转上述不良反应。当血小板减少和/或白细 胞减少不是由基础疾病所致或者与使用兔抗人胸腺细胞免疫球蛋白相关时,建议采用下 列剂量调整(见【用法用量】): 如果血小板计数在50,000~75,000/mm3或者白细胞计数2,000~3,000/ mm3必须考虑减少用
颗粒物,轻轻旋转瓶,直至颗粒消失。如果颗粒物持续存在,则丢弃。 推荐配制后即刻使用。每瓶药物只可以单次使用。根据每日剂量配制相应瓶数的兔抗
人胸腺细胞免疫球蛋白。根据需要,选择合适的瓶数配制。 为了避免不慎输入混有颗粒的溶液,推荐在输注时采用0.2μm过滤器进行在线过滤。 用等渗稀释液(0.9%生理盐水或5%葡萄糖溶液)稀释每日剂量药物至50~500ml(通常
比例50ml/瓶)。 配制的溶液应该处理。

终止用药时,无需逐渐减量。

治疗器官排异:2.5-5毫克/kg/日,至临床症状消失和生物学指标改善。

激素耐受和急性移植物抗宿主病治疗:剂量【不良反应】滴注兔抗人胸腺细胞免疫球蛋白时和用后有如下副反应的报道:全身性副反应为寒战、发热、心跳过速、呕吐和呼吸困难。

局部副反应有输液处局部疼痛及末梢血栓性静脉炎的报道。

罕见迟发性过敏反应,如初次使用后7-15日,可能会发生血清病(发热、搔痒、皮疹伴有关节痛)。

速发严重过敏反应极为罕见。

常见和极严重的副反应发生在第一次滴注后。

有些副反应的发生机理是与细胞分裂释放有关。

应用皮质醇和抗组胺制剂进行预防治疗并减慢滴速或增加稀释液量(等渗0.9%生理盐水或5%葡萄糖溶液)可降低或减轻【禁忌】急性感染时,禁用免疫抑制治疗。

对兔蛋白或本品其他成分过敏者。

【样本要求】新鲜活检或外科样本组织,甲醛固定,固定时间8-24小时要求取材、脱水、石蜡包埋并制成蜡块。

【检验方法】1、人体组织样本先经甲醛固定。

2、固定后的组织经过脱水等一系列操作制备成蜡块。

3、用切片机把组织蜡块切成切片。

4、手工操作免疫组化实验。

常规推荐步骤如下:4.1. 将需要进行实验的组织切片取出,插入玻片架上后放入60-65℃的烤箱中,烤片1-1.5h。

4.2. 将切片置于以下染色缸中梯度反应:二甲苯10分钟×2次二甲苯5分钟无水乙醇5分钟×2次95%乙醇5分钟85%乙醇5分钟PBS清洗3分钟×3次4.3. 抗原修复(方法见一抗供应商,或采用常规高压、微波、酶修复等)4.4.根据组织大小直接滴加内源性过氧化物酶阻断剂到组织上,通常为100uL, 直至完全覆盖组织。

室温10分钟,PBS清洗3分钟×3次。

此步骤通常在水浴法、酶法等温和抗原修复后加入。

或者组织本身含有较多内源性过氧化物酶中加入。

4.5. 根据组织大小滴加一抗到组织上,通常为100uL,直至完全覆盖组织。

抗人T细胞兔免疫球蛋白Kang Ren T Xibao Tu MianyiqiudanbaiAnti-human T Lymphocyte Rabbit Immunoglobulin本品系由人T淋巴细胞免疫家兔后,取其血清或血浆经去除杂抗体、纯化、浓缩后,再经病毒灭活处理并加入适宜稳定剂后冻干制成。

不含防腐剂和抗生素。

1 基本要求生产和检定用设施、原料及辅料、水、器具、动物等应符合“凡例”的有关要求。

2 制造2.1 免疫血清2.1.1 免疫抗原免疫用抗原为人胸腺细胞,或符合“血液制品生产用人血浆”中供血浆者标准的健康人血液分离的人淋巴细胞。

胸腺供体为HBsAg、HCV抗体、HIV-1/HIV-2抗体和梅毒血清学检查应为阴性。

分离后T淋巴细胞数应不低于总细胞数的90%,红细胞数应不高于总细胞数的5%。

2.1.2 免疫动物免疫用家兔至少应符合普通级实验动物的要求(附录ⅩⅢ B和附录ⅩⅢ C),体重为2000~2500克,检疫合格者方可使用。

2.1.3 免疫方法按批准的免疫程序免疫。

2.1.4 采血及分离血清加强免疫后,淋巴细胞毒试验效价达1∶400时即可采血。

分离的血清置-20℃以下保存。

保存期不应超过2年。

2.2 原液2.2.1 混合血清经56℃水浴30分钟灭能,辛酸-硫酸铵盐析分离纯化或经批准的其他分离纯化法,杂抗体吸收,再用DEAE-sephadex A-50色谱纯化抗人T细胞兔免疫球蛋白。

杂抗体吸收用的人红细胞、人血小板、人胎盘组织及人血浆的供给者应符合“血液制品生产用人血浆”中供血浆者标准。

2.2.2 原液检定按3.1项进行。

2.3 半成品制备2.3.1 配制加入适量麦芽糖或其他适宜稳定剂。

按成品规格以灭菌注射用水稀释至所需蛋白质浓度,并适当调整pH值及钠离子浓度。

2.3.2 半成品检定按3.2项进行。

2.4 成品制备2.4.1 分批应符合“生物制品分批规程”规定。

2.4.2 分装及冻干应符合“生物制品分装和冻干规程”规定。

Background:The protein encoded by this gene is a plasma protease inhibitor and a member of the serpin superfamily.This protein inhibits thrombin as well as other activated serine proteases of the coagulation system,and it regulates the blood coagulation cascade.The protein includes two functional domains:the heparin binding-domain at the N-terminus of the mature protein, and the reactive site domain at the C-terminus.The inhibitory activity is enhanced by the presence of heparin.More than120mutations have been identified for this gene,many of which are known to cause antithrombin-III deficiency.[provided by RefSeq,Jul2009].
