ITIL V2服务支持流程 – 故障管理
•An incident is an event which is not part of the standard operation of a service and which causes, or may cause an interruption to, or a reduction in the quality of that service（故障是非正常的操作） •To restore normal service operation as quickly as possible with minimum disruption to the business, thus ensuring that the best achievable levels of availability and service are maintained（尽快把由于故障所造成的非正常的 服务恢复正常，从而把故障对商业服务的影响最小化）

索克曼-先控UPS使用说明书
1，基本工作原理双路市电配置静态开关2，开关的作用15-40kVA范围开关的功能输入开关Q2输入开关提供对UPS的主要电源。
在正常操作条件下它应该是在位置1 "ON"。
位置0 "OFF" 将导致电池放电。
在双路市电的配置下，开关只中断整流器电源。
输出开关Q6Q6 有三个位置，作用如下:位置0 "OFF"在任何一种操作模式下，它能通过取消来自应用的电压，完全隔绝UPS的输出。
它被用来进行系统紧急停工的操作(内部 E.S.D.)顺时针转一下是位置1 "UPS"该位置为UPS 的正常运行，对负载连续供电。
顺时针转两下是位置2 "MANUAL BYPASS"“手动旁路”该位置只应被选为普通或非凡维护操作(手动旁路)；负载被直接地连接到市电供电。
它可被用于UPS 从辅助市电给应用供电有故障的情形下，在等候技术人员的干预时使用。
电池开关Q13，面板介绍高级模拟面板D2: 表意文字显示区D3: 文本和图形显示区域 BL: 亮光状态条 P1: EXIT 退出 P2: UP 上翻 P3: DOWN 下翻P4: ENTER 回车/OK 确定图标含义BLMASTERYS 3/3 30KVALOAD PROTECTED BATTERY OK P1P2P3P4D3D2亮光条含义光亮条对负载供电的情况提供一个直接的指示显示:红=当前无电源或临近停工。
黄=电源存在但不稳定或临时。
绿=电源安全且稳定。
模拟面板菜单下段描述了模拟面板上可用的菜单以及它们的作用。
如图所示，显示被组织入菜单和子菜单。
访问菜单按ENTER回车键，返回到更高一级菜单，按ESC 。
使用UP 上翻/ DOWN下翻键滚动每级可用的资料信息。
当UPS 被并联连接时，模拟面板显示的系统信息好象这里只有唯一一台UPS 。
选择UPS 数量，显示关于各台机器的信息。
查看报警在正常模式下查看系统报警：从面板操作，按ESC退到原始界面如下：然后按回车键进入SYSTEM按回车进入>ALARM按回车进入>ACTIVE ALARM按回车进入报警详情如：A00：general ALARM。

Lockout/tagout is a specific approach to performing set-up and maintenance work that involves isolating equipment from its energy source(s), locking out these energy source(s), and dissipating any stored energy before work begins.TRAINING OBJECTIVETo familiarize you with the lockout/tagout program and procedures so you will be able to understand when these procedures must be used and how to do it safely. DEFINITIONSAffected Employee: An employee whose machine may need to be worked on (e.g., set-up, routine maintenance, repair) by an authorized employee. Affected employees do not participate in the lockout/tagout work at any time. They are simply “affected” by the lockout/tagout work being done by someone else.Authorized Employee: An employee who has been trained and has permission to lock out or tag out machines and equipment to service or maintain them.Energy Isolating Device(EID): A mechanical device that physically prevents the release or transmission of energy. The written procedures must identify the information that authorized employees must know in order to control hazardous energy during service or maintenance.8 EASY LOCKOUT/TAGOUT STEPS1.KNOW THE EQUIPMENTAn Authorized employee must know all energy sources of the equipment to be worked upon and how to control each of them. The specific written lockout/tagout procedure for the equipment will indicate what energies are involved and what energy isolating devices to lock out. Even if you have been on a particular piece of equipment or machine for a long time do not assume that you know every energy source supplying power to that piece of equipment or machine. Things change. Check the procedure. 2. NOTIFY OTHERSThe lockout/tagout you perform may affect other employees. To avoid accidental start-up, or other problems, notify all Affected employees who might have anything to do with the machine before you start the procedure. If you hav en’t contacted the individual responsible for the machine, be sure to find that person and inform them.3. SHUT DOWN THE MACHINETurn off all controls or put them in a neutral position. This is usually a push button, switch or lever. Tag the controls as a warning so others will not attempt to turn them on before you are done with your lockout procedures. It is important to turn off electricpower at the machine before shutting off the disconnect switch or other isolating device because such electric control devices can explode when operated under load.4. DISCONNECT AND LOCK OUT ALL ENERGY SOURCESLock and tag out all primary or main energies at their source. For example, use a manually operated circuit breaker or line valve to physically isolate the equipment from its primary power source.5. CONTROL OR DISSIPATE ANY RESIDUAL ENERGIESResidual energies are not always obvious. Stored energy can unexpectedly make equipment move and cause injury. Lower suspended parts, block movable parts, vent air pressure from pneumatic lines, drain or bleed hydraulic lines to relieve pressure, release or block spring energy, secure machine products if necessary. Only when you have dissipated or controlled secondary energies is the machine in a zero energy state.6. VERIFY THE LOCKOUTDon’t assume anything. Try the machine’s controls to be sure that there is no movement and that no indicator lights show power. Visually check the machine to be sure all moveable parts are stable. Return controls to the off position after your test. This step is only to be done by the authorized lockout/tagout employee. Nobody but the person doing the lockout/tagout shall verify its effectiveness.7. KEEP THE LOCKOUT IN FORCEIf you need to remove a lockout to test the equipment, be sure to put the lock back on before making any further adjustments, no matter how minor they may seem. Always put the machine back to a completely safe state before putting any part of your body ina dangerous position. Remember, it only takes a second for an injury to happen.8. REMOVE THE LOCKOUTWhen you have completed your work, be sure all tools and equipment have been removed from the machine and all guards are in place. Make sure no one is in a danger zone of the machine. Notify all affected personnel that you are about to reenergize the equipment. If you have used warning tags at control switches, leave them in place while you remove the locks and reenergize the machine. Only the authorized employee may remove the lockout. Never ask anyone else to do it. When you are sure everything is working properly, remove the tags and tell the other employees the machine is ready for operation.SOME GENERAL REQUIREMENTS1. If several Authorized employees are working on a job, each person must put theirown lock and tag onto each EID.2. Never ask or allow anyone else to remove your locks and tags for you.3. Never rely on someone else’s lockout. Always place your own locks and tags onevery EID necessary to protect you while you are working on the equipment.4. Use only the authorized equipment that has been provided for lockout/tagout.5. If it comes to the end of your shift and your lockout job is not yet complete, you mustkeep the lockout in place until at least one Authorized employee from the next shift comes and places his locks and tags on the EIDs. Then and only then may you remove your lock. Similarly, the next shift employee should never assume that everything is fine when they take over the job. Always review the lockout/tagout procedure for that piece of equipment and physically verify that each EID was properly locked and tagged out. When that is done, it is safe to continue the job.6. Always use the proper lockout device for the job. For example, if the lockout deviceyou have is f or a 10” to 13” valve handle, but the valve you have to lock out is 15”, don’t try to get by with the wrong size device. Take the time to get the correct device so it will work properly.7. When throwing an electrical disconnect switch, keep your face turned away from thepanel door and use whichever hand that will keep your body clear of the door.8. Never use tags in place of a lock. 4.责任
油漆车间高级经理负责执行本流程。
屏蔽现有制造生产过程的过程方法必须得到油漆车间高级经理、质量经理批准。
车间工程师负责：
确定和维护允许屏蔽的过程清单（见附件1《屏蔽清单》）；
在PFMEA中评价和记录屏蔽过程的失效模式；
根据需要控制计划；
定义屏蔽过程的方法，完成标准化指导书的编写。
生产线人员（包括T/L、G/L、S/L）负责：
生产T/L提出屏蔽申请，记录屏蔽的开始和结束断点，生产S/L负责验证屏蔽恢复完毕
后的签字审批；
生产T/L与G/L协助工程师完成屏蔽的PFMEA评价；
生产G/L至少每2小时对屏蔽工位进行一次分层审核。
5.规定
所有生产制造过程按照批准的PPAP中定义的执行。当现行情况不能满足期望要求，实施
制造屏蔽过程，且事先要按要求进行定义、评审和批准该屏蔽过程。
《屏蔽清单》（详见附件1）需由工程师制订和维护（定期更新）。车间经理、质量经理
对脱离现有生产过程的屏蔽原始清单、过程方法以及更改进行批准。
制造工程牵头的多功能小组（包括质量和生产）应确保屏蔽过程PFMEA的RPN值和正常
工艺的RPN值比较时在合理的水平。
当发生修改、增加和删除时，工程师、生产和车间质量代表将共同确认和评审，确定质
量风险已经适当的降低，确保这个屏蔽过程是可行的。
生产部门需拥有一份正处于屏蔽状态的清单，以及恢复正常生产的反应计划，在每天的
生产会议上进行交流评审这些反应计划的状态。并提交每天的快速响应会议评审，以便强
调能尽快恢复正常生产。《屏蔽跟踪表》（详见附件2）。
生产线记录屏蔽过程开始和结束的断点。
执行屏蔽过程的操作者必须经过培训使其合格上岗操作。
在屏蔽过程进行时，需在该工位旁张贴信息提示表（如变化点目视牌等）。
当屏蔽现有生产过程时，其原因需记录在《屏蔽检查表》（详见附件3）中。
当屏蔽过程结束重新恢复正常生产之前，工程师需在生产人员的协助下，基于实际的情
况对原有过程的PFMEA和RPN重新评估。
当屏蔽过程结束恢复正常生产过程之前，由生产线牵头维修、质量人员根据文件要求对
过程参数和设备设置进行验证。
生产值班长签字批准恢复正常生产。
质量经理签字确认最终的屏蔽检查表中所有步骤的执行过程。
根据各车间实际情况建立自己的屏蔽清单。
